Peer Specialists intentionally use language in the service of listening to understand and making space for complex personal stories of recovery and resilience. Advocacy and Support: • Advocates for and facilitate access to needed health care services, proactively addressing potential barriers such as missed appointments, transportation, fear and stigma, and communication with professional staff. • Advocate with and for program participants both in the larger community and on the team. • Facilitate individual and group-based educational, recovery, wellness and skill building opportunities including: wellness self-management; WRAP; peer support; harm reduction; and other social or leisure activities. • Collaborate with participants to clarify their personal visions and develop their wellness toolkit.   Support participants in strengthening their self-awareness, building life skills, and connecting to resources and community outside of the S:US team. • Work actively within the team itself to help build an environment that operates from a belief in the human potential to grow and an understanding of human diversity. • Accompany individuals to appointments and community/leisure activities. • Carry a small caseload, as needed. Pre-Crisis and Crisis Support: • Providing reassurance and support to an individual in distress or crisis. • Employing non-clinical crisis support, especially after periods of hospitalization or incarceration. • Assisting the individual with putting their wellness, crisis plans or recovery plan into action. • Accompanying, supporting, and advocating on behalf of an individual in an emergency room or crisis unit or preparing to be admitted to detox, residential or other service to deal with crisis. • Providing short-term post-crisis support to an individual who has been discharged from an emergency room, crisis unit, or crisis respite residence, or incarceration. Communication: • Peer Specialists maintain open and frequent communication with the team. This includes reporting safety and other concerns to team members. • Responsible for timely and accurate documentation including documentation of interactions with guests, services provided, critical information and events, contacts with collaterals and referrals, as required by S:US and program policies. • Actively participate in agency and staff team meetings, individualized/team supervision, coaching and professional development activities. • Attend all required trainings. Maintain the highest levels of confidentiality. • Participate in required training and professional development activities. • Perform other duties as required. Community Participation: • Utilizing shared personal experience to support skill development that guides the individual towards a more independent life in the community. • Exploring community spaces that promote integration, inclusion, and participation (bridging from the individual’s home to the community). • Supporting the individual in attending community activities and appointments when requested. • Assisting the individual to help connect to long-term natural supports that enhance the quality and overall security of life. III. S:US is seeking a bi-lingual (English/Spanish speaking) Case Manager for our tansitional shelter in Brooklyn.   This position is located at 1329-1331 Saint Johns Place Brooklyn, NY 11213       ESSENTIAL DUTIES & RESPONSIBILITIES:   • Conduct intake process with new clients within 24 hours of admission; includes but is not limited to explanation of resident rights and responsibilities, and grievance procedures • Conduct psychosocial, vocational/educational, and housing assessments and documentation of same • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing • Prepare housing applications and assist clients with the housing application/interview process. • Monitor and assess client progression/regression in behavior and/or appearance • Facilitate group and individual counseling, workshops, psych education, and recreational activities • Conduct room/dorm visits/inspections to assess client personal hygiene, room cleanliness, and overall ADL skills • Provide comprehensive case management services to an assigned caseload • Develop initial service plans and conduct service plan reviews as needed for an assigned caseload • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate • Ensure that clients attend meals and maintain appropriate nutritional intake • Provide crisis intervention and address all emergency situations as appropriate • Assist in building evacuation during fire drills and building emergencies • Monitor client medication compliance and collaborate with nursing staff monitor compliance • Refer clients to appropriate community resources as needed • Maintain CPR/First Aid Certification • Maintain up to date, accurate, complete, and timely client chart documentation in compliance with DHS, OTDA, other Funder and SUS standards • Facilitate discharge/transfer process and documentation of same • Attend staff meetings, case conferences, group supervision, trainings, and escort clients on housing interviews as needed • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes • A work schedule that includes some weekend and evening hours is required

SCOPE OF ROLE: As part of the SSVF team, this position serves as the primary initial S:US contact point for Veterans and their families.  This position delivers outreach and supportive services to veteran families by providing engagement and ongoing support.  Assists team members in the delivery of daily services, including providing positive modeling, peer counseling, support and encouragement to participants. Screens and assesses applicants applying for SUS Veterans Services and navigates applicants for program services through the intake process, documentation standards and necessary supports.     ESSENTIAL DUTIES & RESPONSIBILITIES: - Provide outreach through the distribution of materials, informational/educational presentations and marketing to community members and possible participants. - Address crowds of varying sizes and offer program presentations. - Provide resource and referral services to community members and possible participants, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and housing resources. - Develop, assess and monitor strategic outreach plan and resources in conjunction with SSVF staff and other SSVF programs. - Interface with representatives from referring agencies and partnering service providers in order to facilitate the delivery of program services and a steady flow of referrals. - Assist program participants with accessing needed community services (housing, employment, medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up.  - Maintain a small caseload of SSVF participants in accordance with SSVF standards. - Assist with the development of Housing Stability Plans and follow-up re-certifications with participants. - Act as advocate for participants as appropriate; encourage and support self-advocacy. - Maintain a strong working relationship with colleagues and program managers. - Document all services in compliance with agency standards. - Work as a member of the social service team in the development of on-site and off-site socialization, educational, risk reduction and skill building activities with a view towards permanent housing stability. - Liaison to community-based services and groups including civic, social service, government, businesses, landlords and non-profit organizations. - Participate in ongoing individual supervision, team meetings, case conferences, division meetings and trainings. - Participate in SUS Veterans Service Fair(s) and other events. - Completion of other tasks as assigned by manager.

  The Assistant Program Director (APD) is responsible for ensuring that our individuals have the supports and resources necessary to live meaningful lives, and that they align with their needs and aspirations within both their respective homes and communities.
