The Case Manager ensures the best quality of care and services for program consumers in a Certified Community Behavioral Health Clinic (CCBHC). The Licensed Practical Nurse will assist the Registered Nurse and be responsible for supporting the individuals we served by taking vital signs, such as blood pressure, temperature and weight, working with consumers in filling prescriptions and coordinating appointments as needed. The LPN will follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed.  The LPN will conduct home visits on a monthly basis visiting up to 25 consumers, refer residents to emergency care, coordinate treatment and discharge planning with hospital staff. Compliance with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes is required.

The Diversion Specialist will coordinate the intake and referral process for adult diversions at DHS identified shelters.    Essential Functions: • Assess shelter applicants directly referred from single and adult family intake centers; serve as the first point of contact. • Enter all Intake and referral information in CARES in a timely manner for Diversions and Aftercare. • Develop individualized housing plan. • Conduct home visits for furnished rooms/apartments. • Provide general case management including coordination and delivery of services relating to housing stability. • Entitlement referral and advocacy. • Meet with participants as needed for follow-up and recommendations. • Meet daily targeted quota for program. • Assist participants in identifying, locating, obtaining and exploring alternatives to shelter (reuniting with family/friends, return to previous living situation, refer to relevant treatment facilities, room rental etc) and retaining suitable housing. • Participate in meetings to promote the program. • Cultivate, develop and maintain productive relationships with referral agencies that are related to retaining housing. • Actively participate in internal and external training programs and committees. • Assure program participants’ rights are protected. • Perform such other duties as may be required. • Maintain case notes for each interaction with clients in CARES. • Complete all individual Diversion folders prior to submitting to Supervisor for review. • Participate in bi-weekly supervision. •Other duties as assigned.

We are looking for Wellness Coaches who have a passion to help others, to serve as case managers.  As a Wellness Coach you will assist consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network.    This is great opportunity for those looking to build a career in the not-for-profit social services arena.  With a passion to help others, you can easily progress into other positions such as Vocational Specialist, Clinical Coordinator, Program Director.   Responsibilities: - Engage people with mental illness and assist them in meeting the demands of community living and maintaining their housing by teaching them the necessary skills, and linking them to services that will support their recovery, independence and self-determination - Conduct Group Meetings on Wellness Management  - Complete service plan reviews for all residents - Regularly assess, correct and report apartment and personal living space inspections conditions as assigned - Provide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated.
