SCOPE OF ROLE: The Respite Specialist will work in the provision of hospital diversion model providing temporary services including residential/respite care so clients can stay in a warm, friendly, safe, and supportive home-like environment where they are taught to use new recovery and relapse preventions skills. This is achieved through 24-hour peer support, self-advocacy education, self-help training, and mutual understanding.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Develop a partnership with guests experiencing crisis utilizing trauma-informed, recovery oriented, intentional peer support, - Promote the provision of a safe, comfortable, secure and trusting environment where guests are listened to and respected by staff - Practice the Need Adapted Treatment Model (NATM) and Intentional Peer Support (IPS) after completing training provided as part of the Parachute NYC grant - Draw on knowledge and skills acquired from personal experience with the mental health system with a focus on guest’s personal recovery process - Create a sense of community through the mutual sharing of lived experiences and offers encouragement and support to participate in daily activities within the CRC and community - Assist with pre-registration process for both individuals referred from NA-MCT (Need Adapted Mobile Crisis Team) and those who self-refer (respite-only guests) - Assist with intake and registration of new guests, including an explanation of services and expectations, a tour of the premises, introduction to other staff and guests - Assist and collaborate with other Parachute NYC staff when appropriate to help guests identify, plan for and realize short-term personal goals and priorities while in the CRC - Facilitate individual and group-based educational, recovery, wellness and skill building opportunities including: wellness self-management; WRAP; peer support; harm reduction; and other social or leisure activities - Follow-up with guests to monitor and support progress toward goal-achievement - Advocates for and facilitate access to needed health care services, proactively addressing potential barriers such as missed appointments, transportation, fear and stigma, and communication with professional staff - Assist guests with steps required to prepare their own meals, manage their own medication, take care of their physical needs and participate in the maintenance of a clean-living environment  - Responsible for timely and accurate documentation including documentation of interactions with guests, services provided, critical information and events, contacts with collaterals and referrals, as required by SUS policies and Parachute NYC - Provide household upkeep duties as needed - Serve daily cold breakfast for guests and assist guests with accessing snacks, when needed - Encourage guest participation in relevant aspects of documentation process - Participates in data collection and recording related to outcomes, research and other agency and grant-based priorities - Participate in supervision, team meetings, interagency meetings, case conferences, NA-MCT meetings, and other relevant meetings as required by SUS and Parachute NYC - Provides flexible evening, night, weekend and holiday coverage     Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance; 403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays

This position is located at 240 E. 178 Street Bronx, NY 10457 The Residential Specialist is responsible for providing a warm ,compassionate welcome to new shelter residents. You will be tasked  with  promoting person-centered services, focusing on supporting the independence of the people we serve.  You will manage the primary coverage in the building 24 hours a day 7 days a week, 365 days a year and at times functioning independently using your judgement to guarantee the safety of the people we serve and the building. S:US Is seeking a Vocational Specialist for our families with children shelter located at 246 Echo Place Bronx, NY 10457.   Salary Minimum: $40k/Yr. Maximum: $42k/Yr.                                The Vocational Specialist will: - Conduct assessments interviews to elicit information relative to client’s skills, education, abilities and interest. - Identify barriers to employment, e.g. English as a Second Language (ESL), substance use disorder, mental health issues et al., and in collaboration with Case Manager makes appropriate referrals. - Assist engaged clients in the development of vocational educational service plans and monitors and facilitates implementation of same. - Document all counseling, assessment, testing and service plans in clients’ folder. - Implement and coordinate the EM Path economic mobility program - Maintain an active employment and vocational/educational training data bank and serves as program representative to all vocational/educational and employment placement service providers. - Provide career coaching/guidance and assists with resume/cover letter preparation, job search and filing applications. - Design and facilitate adult life skills workshops to include: budgeting, tenant behavior, how to maintain your home, and other related topics. - Develop direct job placement services through the development of program job bank. - Maintain tracking forms for job/skills training placements job retention, and reports as required. - Conduct vocational workshops. - Attend all training sessions and meetings as required. - Perform such other duties as may be required. - Any other duties as assigned by the Program Director, designee or as required.

SCOPE OF ROLE: The Care Coordinator provides services in the Clinical Home and in the community to HIV-positive clients living with complex medical conditions, severe mental illness, substance abuse and long-term care needs and a history or risk of over-utilizing medical and behavioral health services. ESSENTIAL DUTIES & RESPONSIBILITIES: - Help find, engage, and enroll clients from the NYS DOH list of eligible Brooklyn and Manhattan Medicaid beneficiaries who have chronic illness and possible mental illness and or substance abuse - Assure that clients reliably attend medical appointments at outpatient clinics - Assure that clients comply with prescription regiment - Write clear service plans and progress notes and maintain meticulous documentation of service visits - Carry a caseload of clients, each of which is to be visited once and at least one service provided during the course of a calendar month - Coordinate ancillary care for clients to ensure continuous, comprehensive care - Collaborate with all members of the team, primary care and specialty doctors and all partners on a regular basis - Initiate appropriate referrals to additional services including home care and notify appropriate support staff of referral to ensure authorization - Assist clients to obtain access to transportation - Research, evaluate and recommend community resources to meet non-medical needs of client (i.e. alternative resource programs, support groups and community support services.) - Work with Medical Home team inpatient providers, discharge planners and nurse partners to ensure efficient use of inpatient resources and a timely discharge to an appropriate setting - Facilitate and participate in client care conference for medical and peer review - Collaborate with mental health and substance abuse care providers to ensure coordination of all services and integration of services - Work collaboratively with the ValueOptions Field Care Coordinator to provide and link clients to services that facilitate illness management - Provide verbal and written illness management educational protocols - Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors.     The Developmental Disabilities Division believes that our role in supporting people with Intellectual and Developmental Disabilities is to live meaningful lives as determined by the person. The Shift Supervisor is responsible for ensuring that people supported have the support and resources necessary to live meaningful lives, and that they align with their needs and aspirations within their respective program and communities. He/she/they will monitor and ensure active adherence to the all plans for each person supported with intellectual/developmental disabilities. The Shift Supervisor will ensure proper supports are in place and necessary resources are made available to Direct Support Professionals to ensure proper adherence to person supported scheduled of activities. The Shift Supervisor serves as a role model to staff and people supported and will help to develop best practices to support both the person supported and Direct Support Professional. ESSENTIAL DUTIES & RESPONSIBILITIES:   Person Centered Planning • Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. • Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. • Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. • Ensures a positive climate that promotes respect and dignity for people supported. • Provides opportunities for people support to personalize their environment, schedule and experiences in program. • Supports the person’s independence, choice and outcomes.   Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. Regulatory • Provide habilitation services to each client as prescribed in their service plans (CFA, Individual Service plan, Habilitation plan). • Responsible for overall functioning of the facility during the absence of the Program Director. • In the absence of the Program Director, supervises and instructs all Direct Support Professionals on shift. • Responsible for people supported and agency funds and expenditures during shift (petty cash, recreation etc.). • Ensures coverage of a shift when a Direct Support Professional is absent. • Acts as a functioning member of the staff, meeting the needs of the people supported • Responsible for the physical condition of the program including agency vehicles; ensuring that they are maintained in a safe and sanitary condition. • Participates in staff meetings, supervisors’ workshops, interdisciplinary team (IDT) and committee meetings. • Channels approved information pertaining to staff and agency to Program Specialists. • Provides supervisory counseling and discipline to Direct Support Professionals as necessary • Identifies facility needs and makes the relevant staff aware. • Ensure optimal utilization of the resources available in order to achieve efficiencies. Quality Improvement • Carries out policies and procedures of SUS, in accordance with OPWDD and other funding sources. • Reports all incidents, ensuring consistent adherence to OPWDD and agency policies • Assesses and monitors staff productivity on an ongoing basis. • Responds appropriately to community, family and agency contacts. • Maintains, develops and supports the person support to staff assignment • Meets with Program Director on a regular basis to discuss pertinent shift and Direct Support Professional’s issues and successes. • Monitors Direct Support Professional’s time sheets on an as-needed basis. • Ensure fire drill completion on shift.

The Assistant Program Director (APD) is responsible for ensuring that our individuals have the supports and resources necessary to live meaningful lives, and that they align with their needs and aspirations within both their respective homes and communities. The APD is responsible for ensuring that the individuals’ wellness, nutritional, and healthcare needs are met. He/she will lead their assigned shift, providing hands-on coaching and role modeling best practices and behaviors for Direct Care Professionals (DSPs) as well as providing 24-hour assistance in periods of high demand.   In collaboration with a team member and under supervision of the Program Director, the APD is expected to promote an environment of respect, learning and independence for the individuals.  He/she will interact with individuals and colleagues in a manner that stems from dignity and with hopes of fostering a true home atmosphere for the individuals.  In addition, he/she will provide coaching to DSPs and Shift Supervisors so that their actions ensure the health and wellness of the individuals, support their independence, self-determination and right to take reasonable risks. The APD will maintain operational efficiency of the residence, including fiscal, regulatory, policies and procedures, staff management, and quality improvement.                                        This position is located at our East 34th St Program in Brooklyn,New York   ESSENTIAL DUTIES & RESPONSIBILITIES: Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. ·         Manage the holistic healthcare, nutrition, and wellness of individuals including tracking appointments, communicating with the CMS, Nurse, relevant doctors and managers.·         Ensures the coordination of all recreation and other community activities that promotes the participation and independence of individuals. ·         Implement, evaluate, and provide feedback on all individual-specific care plans and protocols as written by the team.·         Encourages the independence of individuals and promotes relationship building within the community and with natural supports.   ·         Manages and directs the overall flow of the shift including assigning staff to appropriate individual ratios. ·         Supervise staff (DSPs and Shift Supervisors), while promoting learning and development by providing continual coaching and mentoring. ·         Role model the necessary behaviors that support independence and the aspirational desires of the individuals.     S:US  Is seeking a Housing Specialist for their homeless prevention program.     This program is located at  516 West 181st Street (St. Nicholas Ave) New York, NY 10033     The Housing Specialist will provide aftercare services to  program participants and their families.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Interviews clients to assess their present housing needs and status and submits appropriate applications to community resources. - Conducts groups to discuss available housing options, how to secure an apartment and how to handle issues related to housing. - Accompanies residents to housing interviews, housing fairs, and to complete housing applications when necessary. - Provides housing resources (i.e. phone numbers to landlords, housing programs, etc) to clients and acts as a liaison between landlords and clients. - Refers residents to get PPD and physicals completed within two weeks of their entrance into the facility. - Obtains an accurate history of client’s housing situation to assess what services are needed. - Facilitates Tier II interviews and acts as a liaison between Tier II shelter and clients. - Submits housing applications for all housing programs available (i.e. Section 8, and NYCHA). - Handles requests for extensions to be submitted to HRA - Refers residents to Public Assistance and monitor their compliance with HRA and the ADVENT program. - Negotiates, assesses and assists client needs with respect to benefits and follows the resident’s case from entry to discharge - Assists and escorts residents in going to HRA/social service agency appointments when necessary. - Participates and attends staff, service plan review and clinical team meetings. - Maintains and ensures completion of accurate records and progress notes in client chart. - Other duties as assigned

  S:US is seeking a Residential Specialist for our women's shelter in the Manhattan.   This position is located at 205 W. 135th St. New York, NY 10030   The Residential Specialist is responsible for providing a warm ,compassionate welcome to new shelter residents. You will be tasked  with  promoting person-centered services, focusing on supporting the independence of the people we serve.  You will manage the primary coverage in the building 24 hours a day 7 days a week, 365 days a year and at times functioning independently using your judgement to guarantee the safety of the people we serve and the building. The ideal candidate will  be a person of character with humility ,integrity and can communicate skillfully with empathy and honesty.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct regular scheduled rounds to ensure safety and residents wellbeing. - Enter activity log information and shift notes in a timely fashion and accurate manner. - Complete necessary admissions paperwork. - Assist in escorting residents to services and activities outside of the facility, such as clinics, income maintenance centers and housing referral. - Meet with Shift Supervisor for individual and group supervision. - Planning, implementing and/or researching self-care and other social activities for residents conduct Resident Support groups - Meet with case managers on regular basis to assist in assigned tasks. - Provide initial orientation to program. - Provide administrative support in areas of office coverage, telephone and clerical work, food services. - Understand and follow emergency procedures, including fire drills. - Perform other duties as assigned by Shift Supervisor and Program Director. - Attend job related trainings and agency meetings as necessary. - Provide meal coordination
