The Shift Supervisor will ensure proper supports are in place and necessary resources are made available to Direct Support Professionals to ensure proper adherence to person supported scheduled of activities.   The Shift Supervisor serves as a role model to staff and people supported and will help to develop best practices to support both the person supported and Direct Support Professional.  This opportunity is available at our program located on PineGrove Street  in Queens,NY .   ESSENTIAL DUTIES & RESPONSIBILITIES: Person Centered Planning • Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. • Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. • Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. • Ensures a positive climate that promotes respect and dignity for people supported. • Provides opportunities for people support to personalize their environment, schedule and experiences in program. • Supports the person’s independence, choice and outcomes.   Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. Regulatory • Provide habilitation services to each client as prescribed in their service plans (CFA, Individual Service plan, Habilitation plan). • Responsible for overall functioning of the facility during the absence of the Program Director. • In the absence of the Program Director, supervises and instructs all Direct Support Professionals on shift. • Responsible for people supported and agency funds and expenditures during shift (petty cash, recreation etc.). • Ensures coverage of a shift when a Direct Support Professional is absent. • Acts as a functioning member of the staff, meeting the needs of the people supported • Responsible for the physical condition of the program including agency vehicles; ensuring that they are maintained in a safe and sanitary condition. • Participates in staff meetings, supervisors’ workshops, interdisciplinary team (IDT) and committee meetings. • Channels approved information pertaining to staff and agency to Program Specialists. • Provides supervisory counseling and discipline to Direct Support Professionals as necessary • Identifies facility needs and makes the relevant staff aware. • Ensure optimal utilization of the resources available in order to achieve efficiencies. Quality Improvement • Carries out policies and procedures of SUS, in accordance with OPWDD and other funding sources. • Reports all incidents, ensuring consistent adherence to OPWDD and agency policies • Assesses and monitors staff productivity on an ongoing basis. • Responds appropriately to community, family and agency contacts. • Maintains, develops and supports the person support to staff assignment • Meets with Program Director on a regular basis to discuss pertinent shift and Direct Support Professional’s issues and successes. • Monitors Direct Support Professional’s time sheets on an as-needed basis. • Ensure fire drill completion on shift.

  As Manager of Quality Improvement, you will be joining a dynamic department that protects the organization as well as the people we serve.   Specifically, your role will be to assist in framing a cohesive approach to quality improvement throughout the organization. Consistent with S:US’ core value of Continuous Quality Improvement, this effort will support the development, coordination and implementation of a quality improvement program that promotes the delivery of quality services to the individuals served across all divisions.  The position will be responsible for helping to build our capacity to further advance quality improvement efforts, and overseeing all quality improvement activities within the Quality Assurance Department.  Conduct multiple site visits every week to ensure compliance with all regulatory mandates, and to ensure that quality services are being provided to individuals served. Develop audit tools and protocols designed to review, measure, demonstrate and evaluate consumer care and outcomes.  Prepare for and participate in various internal program evaluation audits. Work collaboratively with program management to develop, review, and revise policies and procedures affecting consumer quality of care and health and safety. Support, promote, and encourage a culture of quality services and safety throughout the organization.   - ESSENTIAL DUTIES & RESPONSIBILITIES: - Responsible for developing, monitoring, evaluating, and documenting the day-to-day operations of a quality improvement program. - Oversee all quality improvement activities within the Quality Assurance Department. - Responsible for developing policies, procedures, protocols, and audit tools, to ensure compliance with all regulatory mandates. - Supervise the QI Consumer Advocate, as well as other potential Quality Assurance staff responsible for quality improvement activities. - Conduct multiple site visits every week to ensure compliance with all regulatory mandates, and to ensure that quality services are being provided to individuals served. - Conduct targeted site observations and other internal audits reviews, including: (i) determining what programmatic areas need to be reviewed in order to ensure that relevant regulations, policies and procedures are being implemented appropriately addressed; (ii) identifying and immediately addressing any health and safety issues with program management to protect the agency and the individuals we serve; and (iii) drafting internal audit reports to be used by program management to ensure quality services and compliance with relevant regulations, policies and procedures. - Conduct pre-certification audit reviews for programs, including: (i) determining the areas that need to be reviewed in preparation for external audits; (ii) meeting with program management to review findings and to provide guidance on compliance with regulatory mandates; and (iii) drafting internal audit reports to be used by program management in audit preparation activities. - Conduct reviews of the implementation of Plans of Corrective Actions (POCAs) to ensure that the areas identified in the POCAs have been addressed in order to protect the agency from receiving repeat audit citations. - Review, revise, and identify issues relating to Plans of Corrective Actions (POCAs) for government programs or audits prior to submission by S:US to external oversight agencies to ensure acceptance of the POCAs by said oversight agencies. - Participate on the Mortality Review Committee (MRC). - Create reports for the Chief Compliance Officer and VP of Quality Assurance and Compliance that document and measure Quality improvement activities. - Draft written reports and presentations related to quality improvement processes and outcomes. - Establish quality measurement and improvement activities, including methods to track the effectiveness and implementation of corrective action plans. - Plan and manage the process of accurate data collection, objective evaluation, and QI activities of specific programs and the agency as a whole. - Communicate quality improvement efforts and outcomes to all stakeholders. - Work collaboratively with program management to develop, review, and revise policies and procedures affecting consumer quality of care and health and safety. - Participate in staff/management meetings as they relate to quality improvement activities.

S:US Is seeking a Supervisor of Maintenance for our men's shelter in Brooklyn.    This position is located at 1000 Blake Avenue Brooklyn, NY 11208.     The Supervisor of Maintenance oversee the maintenance and cleanliness of facility and supervise all Porters, ensuring a safe and comfortable environment for residents.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Supervise and lead a team of Porters in maintaining cleanliness and orderliness of the shelter facility, including common areas, bedrooms, bathrooms, and outdoor spaces. - Develop and implement cleaning schedules and protocols, ensuring all areas are cleaned regularly and efficiently. - Conduct regular inspections to ensure that all maintenance issues are promptly addressed and resolved. -  Coordinate with external vendors and contractors for repairs and maintenance work. - Perform general maintenance tasks such as painting, plumbing, electrical work, carpentry, and other handyman duties as needed. - Train, mentor and supervise porters consistently on proper cleaning techniques and safety procedures. - Ensure compliance with health and safety regulations, including proper handling and disposal of hazardous materials - Maintain inventory of cleaning and maintenance supplies, and prepare purchase orders as necessary. - Assist in the recruitment and hiring process of new porters. - Collaborate with other staff members to create a supportive and positive environment for residents.

Housing Specialist is a member of the HLPS Housing Team that develops and executes housing plans, placement, and follow-up for referred clients. The HLPS Housing Specialist is responsible for collaborating with case managers and clients and the HLPS Housing Acquisitions Director to identify and secure permanent housing. Essential Duties and Responsibilities - Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords to identify new housing opportunities for clients. - Maintain and update a database of available housing options with relevant details and document status changes/findings/activities and provide updates on new and total inventory on a weekly or more basis. Maintain consistent, transparent communication and sharing of relevant information with HLPS program partners. - Ensure uniformity in maintaining fair housing standards in securing apartments. - Confirm landlord and management company compliance in meeting the established housing laws, building codes, and lease agreements. - Inform case management staff of unit readiness and/or problems with clients maintaining units. - Work with case managers to identify and problem-solve barriers to permanent housing and assist with preparing clients for the housing process. - Work with case management staff to resolve crises or conflicts with building owners, landlords, and management. - Provide accurate, complete, and timely data entry and other documentation of findings, apartment unit status, inspections, readiness, and as needed, interactions and assessments with clients. - Attend staff meetings, case conferences, group supervision, and professional development activities such as in-service trainings, seminars, webinars, and conferences as needed. S:US is seeking a Case Manager for our homeless prevention program.   This position is located at 516 West 181 Street (St. Nicholas Ave) New York, NY 10033 Salary:  The Case Manager will provide comprehensive case management services to an assigned caseload of  clients. Conduct intake process within 24 hours of admission. Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing. Facilitate group and individual counseling, workshops, psych education, and recreational activities. Conduct room inspections to assess client overall ADL skills. Conduct service planning, provide referrals to in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate. Provide crisis intervention and assist in building evacuation during fire drills. Monitor client medication compliance and escort clients on housing interviews as needed. A work schedule that includes some weekend and evening hours is required.

I. SCOPE OF ROLE: The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require limited on call coverage.   II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Complete medical assessments and communicable disease risk assessments within mandated time frames. • Coordinate delivery and reception of medications with pharmacy. • Track all medications received and administered as well as coordinate return of unused medications to pharmacy • Observe self-administration of medications for detoxification according to established protocols. • Refer for required medical services. • Provide education and consultation to individual clients and/or groups on their health needs. • Assess clients’ medical record and coordinate medical care with multidisciplinary team. • Assure clients’ compliance with prescribe medical regimen. • Complete timely and accurate documentation on medical services rendered. • Keep supervisor informed of all relevant information regarding individuals and the program. • Provide internal training to individuals/staff members. • Participate in weekly protocol meetings. • Other duties as requested by supervisor.

The ideal candidate will be a person of character with humility, integrity and who can communicate skillfully with empathy and honesty.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Plan trips, recreational and cultural activities appropriate to adult and children’s interest and developmental levels, prepare monthly schedule, conduct recreational activity groups. - Supervise the after school and summer program, provide respite care when necessary. - Maintain recreational space with age-appropriate toys, arts, and crafts - Accompany children and parents to events when necessary. - Provide guidance and information to clients around the daily activity schedule, Program Policies and Procedures - Responsible for cleaning program areas after usage. - Participate in ongoing individual supervision, team meetings, and training as appropriate. - A work schedule that includes weekend and evening hours may be required. Additional shifts required as needed. - Assist in escorting residents to services and activities outside of the facility, such as clinics, income maintenance centers and housing referral. - Planning, implementing and/or researching self-care and other social activities for residents Meet with case managers on regular basis to assist in assigned tasks. - Perform other duties as assigned by Senior Leadership, Program Director, Director of Social Services, or immediate supervisor. - Attending job-related training and agency meetings, as necessary.

SCOPE OF ROLE: The Respite Specialist will work in the provision of hospital diversion model providing temporary services including residential/respite care so clients can stay in a warm, friendly, safe, and supportive home-like environment where they are taught to use new recovery and relapse preventions skills. This is achieved through 24-hour peer support, self-advocacy education, self-help training, and mutual understanding.   ESSENTIAL DUTIES & RESPONSIBILITIES: Develop a partnership with guests experiencing crisis utilizing trauma-informed, recovery oriented, intentional peer support, Promote the provision of a safe, comfortable, secure and trusting environment where guests are listened to and respected by staff Practice the Need Adapted Treatment Model (NATM) and Intentional Peer Support (IPS) after completing training provided as part of the Parachute NYC grant Draw on knowledge and skills acquired from personal experience with the mental health system with a focus on guest’s personal recovery process Create a sense of community through the mutual sharing of lived experiences and offers encouragement and support to participate in daily activities within the CRC and community Assist with pre-registration process for both individuals referred from NA-MCT (Need Adapted Mobile Crisis Team) and those who self-refer (respite-only guests) Assist with intake and registration of new guests, including an explanation of services and expectations, a tour of the premises, introduction to other staff and guests Assist and collaborate with other Parachute NYC staff when appropriate to help guests identify, plan for and realize short-term personal goals and priorities while in the CRC Facilitate individual and group-based educational, recovery, wellness and skill building opportunities including: wellness self-management; WRAP; peer support; harm reduction; and other social or leisure activities Follow-up with guests to monitor and support progress toward goal-achievement Advocates for and facilitate access to needed health care services, proactively addressing potential barriers such as missed appointments, transportation, fear and stigma, and communication with professional staff Assist guests with steps required to prepare their own meals, manage their own medication, take care of their physical needs and participate in the maintenance of a clean-living environment  Responsible for timely and accurate documentation including documentation of interactions with guests, services provided, critical information and events, contacts with collaterals and referrals, as required by SUS policies and Parachute NYC Provide household upkeep duties as needed Serve daily cold breakfast for guests and assist guests with accessing snacks, when needed Encourage guest participation in relevant aspects of documentation process Participates in data collection and recording related to outcomes, research and other agency and grant-based priorities Participate in supervision, team meetings, interagency meetings, case conferences, NA-MCT meetings, and other relevant meetings as required by SUS and Parachute NYC Provides flexible evening, night, weekend and holiday coverage       Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance; 403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays
