SCOPE OF ROLE: The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community.  The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. S:US is seeking a Psychiatric Nurse Practitioner for our men's mica shelter in Brooklyn.   This position is located at 1000 Blake Avneue Brooklyn, NY 11208.     The Psychiatric Nurse Practitioner will provide direct psychiatric services for homeless individuals at three separate, general population treatment programs who are not connected to care. The PNP will document medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions and outcomes. The PNP will provide medication management including prescription refills and medication education, write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements and also provide in service staff training and consultation to the social service team. - Meets with Program Director, Assistant Director- Clinical, Program Nurse, and Social Service staff to coordinate shelter residents to receive ongoing psychiatric treatment in the community. - Meet with all new residents within the first 30 days for psychiatric evaluation. - Provide referrals for transitional Psychiatric Treatment to all Residents not currently in care. - Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Review health assessments completed by the Program Nurse and provide clinical feedback. - Provide medication management including prescription refills and medication education to consumers. - Administer medications including those administered by injection. - Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes. - Prepare Psychiatric Assessments for the purpose of HRA 2010e Housing Application and/or other applications for housing or needed services. - Provide training to other team members on topics including medications and drug interactions.   - Communicates and coordinates care with psychiatrists in the community, as needed. - Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.  - Provide in service staff training monthly and consultation as needed to the social service team.  - Provide crisis intervention and address all emergency situations as appropriate.  - Assist in building evacuation during fire drills and building emergencies. - Maintain CPR/First Aid Certification. - Maintain up to date, accurate, complete, and timely consumer chart documentation. - Attend staff meetings, case conferences, group supervision, and trainings as needed. - Attend face to face supervision with Collaborating Psychiatrist at least monthly. Make chart work available to Collaborating Psychiatrist for quarterly reviews. - Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.  - A work schedule that includes some weekend and evening hours will be required.

Projects will be developed together and may vary from week to week, based on overall agency need.   The internship will be a part‐time position (full‐time for a summer internship) and will include a bi‐weekly stipend, with the possibility to become a full‐time position when the internship is over.   ESSENTIAL DUTIES & RESPONSIBILITIES:  - Developing, managing, and updating project plans; - Preparing policy briefings, talking points, presentations, and proposals for internal and external meetings; - Assembling research and generating ideas on a variety of topics; - Preparing agendas and distributing meeting minutes; - Engaging with designated staff on open items and deliverables; and Tracking issues related to various initiatives.
