   S:US  Is seeking a Housing Specialist for our transitional housing shelter in Brooklyn.     This program is located at 1329-1331 Saint Johns Place Brooklyn, NY 11213     SCOPE OF ROLE;   The Housing Specialist is responsible for Housing duties related to all residents’ needs.  ESSENTIAL DUTIES & RESPONSIBILITIES: - Interviews clients to assess their present housing needs and status and submits appropriate applications to community resources. - Conducts groups to discuss available housing options, how to secure an apartment and how to handle issues related to housing. - Accompanies residents to housing interviews, housing fairs, and to complete housing applications when necessary. - Provides housing resources (i.e. phone numbers to landlords, housing programs, etc) to clients and acts as a liaison between landlords and clients. - Refers residents to get PPD and physicals completed within two weeks of their entrance into the facility. - Obtains an accurate history of client’s housing situation to assess what services are needed. - Facilitates Tier II interviews and acts as a liaison between Tier II shelter and shelters clients. - Submits housing applications for all housing programs available (i.e. Section 8, and NYCHA). - Handles requests for extensions to be submitted to HRA - Refers residents to Public Assistance and monitor their compliance with HRA and the ADVENT program. - Negotiates, assesses and assists client needs with respect to benefits and follows the resident’s case from entry to discharge - Assists and escorts residents in going to HRA/social service agency appointments when necessary. - Participates and attends staff, service plan review and clinical team meetings. - Maintains and ensures completion of accurate records and progress notes in client chart. - Other duties as assigned

Provide quality cleanliness oversight to assigned building(s).             - ESSENTIAL DUTIES & RESPONSIBILITIES:   - Maintain cleanliness in assigned areas within the facility. SAPT Peer Specialist TS-Wellness Works (Brooklyn, New York) Salary rate Range: Minimum - $19.23, Maximum - $21.63 per hour    I. SCOPE OF ROLE: The Peer Specialist facilitates engagement with shelters to reduce adverse substance use incidents in shelters (accidents, altercations, emergency services and overdoses) in order to improve client health and housing outcomes by providing and connecting at risk individuals to clinical services and peer supports.   II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Provide peer services on site to people living in the shelter that are at risk of harm from drugs and alcohol. • Provide recovery support services to individuals identified as at risk. • Provide coordination of treatment and transitional planning as needed. • Provide harm reduction education and awareness to shelter staff and clients on a regular basis: presentations, office hours, signage, etc. • Work collaboratively to encourage and support individuals identified as at risk. • Work collaboratively to meet project deliverables and achieve overall project goals. • Assist with making referrals to harm reduction, substance use, primary care, mental health care, pain management or other as needed. • Support linkage to care, return to care, retention in care. • Accompaniment to appointments and supporting access to tele-health services. • Treatment adherence support (reminders, check-ins etc). • Support discharge and transition planning. • Documentation and reporting of services delivered (in CARES). • Participate in Community of Practice and Learning of others in this role citywide (facilitated by DHS and OASAS). • Embracing Creative Narratives: Peer Specialists need to be able to understand, share and discuss multiple frameworks for understanding life experiences such as psychosis with participants and the rest of the S:US team. Peer Specialists intentionally use language in the service of listening to understand and making space for complex personal stories of recovery and resilience. • Advocacy: Peer Specialists advocate with and for program participants both in the larger community and on the team. • Co-Creating Support and Wellness Tools: Collaborate with participants to clarify their personal visions and develop their wellness toolkit. Along with other team members, Peer Specialists support participants in strengthening their self-awareness, building life skills, and connecting to resources and community outside of the S:US team. • Team Communication: Peer Specialists maintain open and frequent communication with the team. This includes reporting safety and other concerns to the team. Peer Specialists will also keep notes on visits with program participants as required by the provider agency. In the spirit of peer support, co-collaborating on reporting with the program participant is encouraged.

The Qualified Intellectual Disability Professional (QIDP) is responsible for the overall effective development, implementation and support of the person supported road map to leading the meaningful life they choose to live. The QIPD is responsible for ensuring each plan is developed and supports Person Centered Thinking and Planning.  The QIDP will help to lead teams in creating meaningful activities and valued outcomes for persons supported in the areas of community integration; socialization; recreation, volunteerism and mindful motivation. Promote, research and provide activities that further enrich people’s lives, while providing options in both the program setting and community environments.  The QIDP ensures that services provided by the DSPs are properly documented and adhere to all OPWDD data collection regulations to ensure proper billing of services render.   The QIDP is to support DSPs in increasing their knowledge and ability to capture service and experiences provided via training and modeling.  The incumbent will support Program Directors during periods of high demand and will provide ongoing coaching and opportunities for learning to the DSPs   This position has aminimum and a maximum salary of $58.500   Person Centered Planning - Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. - Ensures a positive climate that promotes respect and dignity for people supported. - Supports the person’s independence, choice and outcomes. - Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. - Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. - Provides opportunities for people support to personalize their environment, schedule and experiences in program. The Service Coordinator, SSVF will conduct comprehensive assessments of veterans/veteran families’ needs; develop, implement, monitor, and evaluate individualized housing stability plans with each client; provide short-term intensive case management to veteran families to achieve their housing stability goals, and to link each program participant with appropriate community supports to ensure their housing stability.     Responsibilities: - Engage program participants by maintaining regular contact to monitor and track progress and response to services, including home visits and field work. - Develop and implement individualized housing stability plans with veterans/veteran families to address identified housing barriers for Homelessness Prevention and Rapid Rehousing services. - Monitor and evaluate client progress and adjust housing plan and/or service delivery as necessary. - Complete all necessary documentation to ensure compliance with funding requirements, and agency quality assurance standards. - Performs comprehensive assessments on clients according to SSVF and agency standards. - Assist program participants in identifying and accessing healthcare, employment/income support supports and other supportive services as needed, including Department of Veterans Affairs’ services and benefits. - Advocate for veterans/veteran families at service providers and agencies in the community. - Coordinate Rapid Rehousing and Homelessness Prevention services with program participants and the social service team. - Assist veteran families with housing search and placement. - Coordinate the provision of Temporary Financial Assistance with third-parties and agency personnel.   - Cultivate positive relationships with relevant funding and monitoring entities, faith based organizations, social service providers, and other community partners. - Participate in appropriate community collaborations and activities. - Initiates and completes case closure when appropriate. - Participate in SSVF and Veterans Services team meetings.  - Available for after-hours crisis response for intensive case management as needed. - Respond to client needs promptly, accurately and with courtesy and respect.  Model appropriate behavior.  Represent the organization in a positive and appropriate manner to outside resources. - Maintain the highest levels of confidentiality for clients and staff, restricting information to those who need to know only.
