The Developmental Disabilities Division believes that our role in supporting people with Intellectual and Developmental Disabilities is to live meaningful lives as determined by the person.  All persons shall live a life with dignity conforming to each personal value system.  Our philosophy is for the people we serv e to be part of the community,  to live fulfilling lives driven by their choices and to be as independent as possible. Our mission is to accomplish this with well-trained staff, access to the quality health care, clinical supports, and a clean and safe home.     The  Shift  Supervisor  is  responsible  for  ensuring  that  people  supported  have  the  support  and resources necessary to live meaningful  lives, and that they align with their needs and aspirations within their respective program and communities. He/she/they will monitor and ensure active adherence to the all plans for each person supported with intellectual/developmental disabilities. The Shift Supervisor will ensure proper supports are in place and necessary resources are made available   to  Direct  Support  Professionals   to  ensure  proper  adherence   to  person  supported scheduled of activities.  The Shift Supervisor serves as a role model to staff and people supported and will help to develop best practices to support both the person supported and Direct Support Professional.   This position is located Decatur Street IRA in Brooklyn, NY . The salary is $18.oo per hour.     ESSENTIAL DUTIES & RESPONSIBILITIES: Person Centered Planning - Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. - Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. - Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. - Ensures a positive climate that promotes respect and dignity for people supported. - Provides opportunities for people support to personalize their environment, schedule and experiences in program. - Supports the person’s independence, choice and outcomes.  - Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. - Additional responsibilities as deemed necessary and appropriate by the Vice President of Day Services or designee. - Supports the mission, vision, values of S:US.   Regulatory - Provide habilitation services to each client as prescribed in their service plans (CFA, Individual Service plan, Habilitation plan). - Responsible for overall functioning of the facility during the absence of the Program Director. - In the absence of the Program Director, supervises and instructs all Direct Support Professionals on shift. - Responsible for people supported and agency funds and expenditures during shift (petty cash, recreation etc.). - Ensures coverage of a shift when a Direct Support Professional is absent. - Acts as a functioning member of the staff, meeting the needs of the people supported - Responsible for the physical condition of the program including agency vehicles; ensuring that they are maintained in a safe and sanitary condition. - Participates in staff meetings, supervisors’ workshops, interdisciplinary team (IDT) and committee meetings. - Channels approved information pertaining to staff and agency to Program Specialists. - Provides supervisory counseling and discipline to Direct Support Professionals as necessary - Identifies facility needs and makes the relevant staff aware. - Ensure optimal utilization of the resources available in order to achieve efficiencies.   Quality Improvement - Carries out policies and procedures of SUS, in accordance with OPWDD and other funding sources. - Reports all incidents, ensuring consistent adherence to OPWDD and agency policies - Assesses and monitors staff productivity on an ongoing basis. - Responds appropriately to community, family and agency contacts. - Maintains, develops and supports the person support to staff assignment - Monitors Direct Support Professional’s time sheets on an as-needed basis. - Ensure fire drill completion on shift.

    S:US  Is seeking a Housing Specialist for their men's shelter in Brooklyn     This program is located at 1000 Blake Avenue Brooklyn, NY 11208.   SCOPE OF ROLE:   The Housing Specialist is responsible for Housing duties related to all residents’ needs.    ESSENTIAL DUTIES & RESPONSIBILITIES: - Interviews clients to assess their present housing needs and status and submits appropriate applications to community resources. - Conducts groups to discuss available housing options, how to secure an apartment and how to handle issues related to housing. - Accompanies residents to housing interviews, housing fairs, and to complete housing applications when necessary. - Provides housing resources (i.e. phone numbers to landlords, housing programs, etc) to clients and acts as a liaison between landlords and clients. - Refers residents to get PPD and physicals completed within two weeks of their entrance into the facility. - Obtains an accurate history of client’s housing situation to assess what services are needed. - Facilitates Tier II interviews and acts as a liaison between Tier II shelter and shelters clients. - Submits housing applications for all housing programs available (i.e. Section 8, and NYCHA). - Handles requests for extensions to be submitted to HRA - Refers residents to Public Assistance and monitor their compliance with HRA and the ADVENT program. - Negotiates, assesses and assists client needs with respect to benefits and follows the resident’s case from entry to discharge - Assists and escorts residents in going to HRA/social service agency appointments when necessary. - Participates and attends staff, service plan review and clinical team meetings. - Maintains and ensures completion of accurate records and progress notes in client chart. - Other duties as assigned

SCOPE OF ROLE: The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. SCOPE OF ROLE: The ENGAGE Clinical Coordinator will lead a team of “can-do” people who continually challenge the status quo, merging cutting-edge innovation with groundbreaking research to decrease disparities in mental health and substance use problems by utilizing technology and training to provide high quality and accessible mental health assessments, immediate care, and referrals, as well as promote financial wellness among vulnerable populations. The Clinical Coordinator will be based at an integrated OASAS 822 and Article 31 Outpatient Mental Health Clinic serving adults living with substance use disorders and mental illness; including clinical program development, supervision, training and evaluation of professional staff.   Salary $80,000 - $87,000 Location: East Harlem   ESSENTIAL DUTIES & RESPONSIBILITIES: • Assists in establishing effective and efficient operations in accordance with ENGAGE regulations. • Lead in the clinical coordination of ENGAGE Community Mental Wellness Workers (CMWW). • Work collaboratively with the Project Coordinator on establishing workflows, developing initiatives, and mental health curriculums for staff. • Conducts clinical supervision of CMMW Outreach Specialists. • Directly supervises data reporting from the Team Leaders utilizing the Columbia Tool. • Work with the Columbia ENGAGE Team on Social Determinants of Health component of research that is relevant to served populations. • Issue clinical recommendations to Program Coordinator to ensure proper compliance of program guidelines. • Build relationships with S:US programs to develop inter-agency cross-collaboration. • Pending IPC Supervisory certification, provide counsel and expert advice to the Director and other staff for quality and performance improvement. • Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by S:US' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with S:US policies, OMH and OASAS regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services. • In coordination with relevant members of the SUS management team, conducts outreach and marketing to potential referral sources internal and external to SUS; active liaison with community service providers, hospitals, and other prospective referral sources is may be required. • Coordinates care with SUS nurse practitioners, psychiatrists and administrative staff; closely coordinates with clinicians external to SUS as appropriate. • Assist in managing client crises and onsite and offsite emergencies. • Perform other tasks as assigned.

S:US Is seeking a Director of Social Services for their adult families' shelter in Manhattan.   This position is located at 12 East 31 Street New York, NY 10016.     The Director of Social Services will manage and monitor daily program operations of a mental health shelter based on city, state, and federal regulatory requirements and DHS contractual requirements.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Plans, schedules, organizes, directs, and oversees the provision of all clinical services. - Provides supervisory oversight and training to the Senior Service Coordinators, Entitlements Specialist, Housing Specialist and Program Nurse. - Ensures the collaboration between the Senior Service Coordinators, Entitlements Specialist, Housing Specialist and Program Nurse around coordination of services/case management, barriers to services, and community-based referrals for job skills training, education, and employment as well as progression/regression of consumer behavior, mental health, chemical dependency, and any other issues that may present barriers to housing, employment, and job skills training.  - Meets weekly with the Program Nurse and Psychiatrist to coordinate the psychiatric and medical care of Shelter Residents. - Reviews all admissions and intake screenings. - Monitors daily census reports. - Develops groups, workshops, and recreational activities that will foster consumer self-sufficiency and self-determination, recovery, relapse prevention, daily living skills, socialization skills, and overall wellness.    - Monitors and oversees the assignment of caseloads, group and activity facilitation, housing and job placements, job skills training placements, and referrals for community based social services. - Monitors and oversees the results of room/dorm visits/inspections to assess consumer personal hygiene, room cleanliness, and overall ADL skills. - Responsible for Incident Management, crisis intervention, and all related documentation. - Assists in overseeing and managing building evacuation during fire drills and building emergencies. - Monitors program effectiveness, compliance and consumer outcomes, including but not limited to, productivity standards, reporting to management and funding source/s. - Monitors the establishment and maintenance of linkages with community-based job skills training and education programs in order to build a referral base for consumers to secure job skills training and education. - Monitors the establishment and maintenance of linkages with mental health, primary health care, and substance abuse service providers. - Monitors the establishment of linkages and maintenance of solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for consumers. - Attends staff meetings, case conferences, trainings, and management meetings when needed. - Acts as primary liaison for Care Coordination team and other departments within facility. - Monitors documentation for quality and timely completion; reviews and monitors the development of all Individual Living Plans and ensures they are updated every 30 days. - Develop plans for follow up with program graduates to monitor status for up to one year with documentation of same. - Provides direct services as needed and appropriate. - Complies with and enforces all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, NYC - DHS and NYS Dept. of Social Services Rules, Regulations, and Statutes.  - Performs other duties as assigned.

SCOPE OF ROLE: Provide quality cleanliness oversight to assigned building(s). This role is for scatter site program in Brooklyn.  ESSENTIAL DUTIES & RESPONSIBILITIES: - Maintain cleanliness in assigned areas within the facility. This includes but is not limited to: floor care (mopping, sweeping, buffing, stripping and waxing), cleaning bathrooms, emptying trash, separating recyclables, emptying garbage, wiping windows and walls, and cleaning stairways. - Carry out minor repairs and building maintenance throughout assigned areas of the facility. - Responsible for exterior snow and ice removal as well as upkeep of the grounds. The Intake Specialist will; - Screens and assesses applicants applying for emergency shelter, SUS/Palladia Veterans Services & Homebase. - Serves as the primary initial SUS contact point for Veterans and their families. - Navigates applicants for program services through intake process, documentation standards and necessary supports.   ESSENTIAL JOB FUNCTIONS - Schedule and conduct intake and follow-up appointments with program applicants, including screenings via telephone, in person, and in the field at Department of Homeless Services (DHS) installations, homeless shelters and other outreach/recruitment locations. - Understand and communicate the programs’ eligibility and admission criteria. - Demonstrate sound understanding of Homebase and Veterans Division programs. - Assess households’ immediate service needs, providing referrals when necessary for immediate services. - Document intake screenings and maintain records for each applicant and enrolled participant in both DHS CARES and AWARDS systems. - Collect required documentation for program enrollment and scan documents for digital archiving. - Assist applicants with obtaining necessary documentation for program enrollment. - Notify Veteran households of intake decisions. - Provide referrals for Veterans services both in-house and for external supports. - Develop strong linkages with referring workers and agencies. - Assemble intake reports including spreadsheets to summarize data. - Participate in team meetings. - Prepare formal letters and enclosures for distribution to applicants and referral sources.     - Respond to information requests from internal SUS staff and DHS. - Maintain close contact with participants and staff. - Participate in SUS Veterans Service Fair(s) and other company events. - Completion of other tasks as assigned by manager.
