- SCOPE OF ROLE: The Family Specialist is responsible to work collaboratively as part of an integrated behavioral health team to address the needs of the “whole person”, including their physical and emotional health.   - ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct in-depth family histories and family assessment for service planning integrating the needs of all family members individually and the needs of the family unit; make appropriate recommendations to the treatment team. - To increase family stability, the Family Life Specialist will assess family’s behavioral and relational problems and refer them to needed services. - Strengthen the families by offering services such as child care planning, self-sufficiency goal setting, educational assistance for children, working with foster care agencies, crisis intervention, family mediation and conflict resolution, and parenting skills workshops. - Complete all progress notes, treatment planning goals, written assessments, and other required documentation of services rendered in a timely fashion. - Conduct home and office visits as necessary. - Participate with and support client in service planning for family reunification. - Coordinate with other public systems, providing direction and advocacy on behalf of client families, including child welfare agencies, courts, and public assistance as necessary. - Prepares report for Assistant Deputy Director of Mental Health Services and Executive Director, including any problems, and proposals for changes in policies or program design - Other duties as assigned   The Salary for this positon : The Mininmum is $35,000 and the Maximum is $40,000

S:US is seeking a Residential Specialist for our women's shelter in the Manhattan.   This position is located at 205 W. 135th St. New York, NY 10030   The Residential Specialist is responsible for providing a warm ,compassionate welcome to new shelter residents. You will be tasked  with  promoting person-centered services, focusing on supporting the independence of the people we serve.  You will manage the primary coverage in the building 24 hours a day 7 days a week, 365 days a year and at times functioning independently using your judgement to guarantee the safety of the people we serve and the building. The ideal candidate will  be a person of character with humility ,integrity and can communicate skillfully with empathy and honesty.       ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct regular scheduled rounds to ensure safety and residents wellbeing. - Enter activity log information and shift notes in a timely fashion and accurate manner. - Complete necessary admissions paperwork. - Assist in escorting residents to services and activities outside of the facility, such as clinics, income maintenance centers and housing referral. - Meet with Shift Supervisor for individual and group supervision. - Planning, implementing and/or researching self-care and other social activities for residents conduct Resident Support groups - Meet with case managers on regular basis to assist in assigned tasks. - Provide initial orientation to program. - Provide administrative support in areas of office coverage, telephone and clerical work, food services. - Understand and follow emergency procedures, including fire drills. - Perform other duties as assigned by Shift Supervisor and Program Director. - Attend job related trainings and agency meetings as necessary. - Provide meal coordination    

  S:US is seeking a Case Manager for our homeless prevention program.   This position is located at  516 West 181st Street (St. Nicholas Ave) New York, NY 10033       The Case Manager will provide comprehensive case management services to an assigned caseload of clients. Conduct intake process within 24 hours of admission. Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing. Facilitate group and individual counseling, workshops, psych education, and recreational activities. Conduct room inspections to assess client overall ADL skills. Conduct service planning, provide referrals to in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate. Provide crisis intervention and assist in building evacuation during fire drills. Monitor client medication compliance and escort clients on housing interviews as needed. A work schedule that includes some weekend and evening hours is required.
