The Service Coordinator, SSVF will conduct comprehensive assessments of veterans/veteran families’ needs; develop, implement, monitor, and evaluate individualized housing stability plans with each client; provide short-term intensive case management to veteran families to achieve their housing stability goals, and to link each program participant with appropriate community supports to ensure their housing stability.     Responsibilities: - Engage program participants by maintaining regular contact to monitor and track progress and response to services, including home visits and field work. - Develop and implement individualized housing stability plans with veterans/veteran families to address identified housing barriers for Homelessness Prevention and Rapid Rehousing services. - Monitor and evaluate client progress and adjust housing plan and/or service delivery as necessary. - Complete all necessary documentation to ensure compliance with funding requirements, and agency quality assurance standards. - Performs comprehensive assessments on clients according to SSVF and agency standards. - Assist program participants in identifying and accessing healthcare, employment/income support supports and other supportive services as needed, including Department of Veterans Affairs’ services and benefits. - Advocate for veterans/veteran families at service providers and agencies in the community. - Coordinate Rapid Rehousing and Homelessness Prevention services with program participants and the social service team. - Assist veteran families with housing search and placement. - Coordinate the provision of Temporary Financial Assistance with third-parties and agency personnel.   - Cultivate positive relationships with relevant funding and monitoring entities, faith based organizations, social service providers, and other community partners. - Participate in appropriate community collaborations and activities. - Initiates and completes case closure when appropriate. - Participate in SSVF and Veterans Services team meetings.  - Available for after-hours crisis response for intensive case management as needed. - Respond to client needs promptly, accurately and with courtesy and respect.  Model appropriate behavior.  Represent the organization in a positive and appropriate manner to outside resources. - Maintain the highest levels of confidentiality for clients and staff, restricting information to those who need to know only.

The Psychotherapist position is for our Certified Community Behavioral Health Clinic (CCBHC), an integrated dually licensed clinic by OMH and OASAS with OMH as the host. As a CCBHC, we not only offer evidence-based and trauma informed treatment for mental illness/substance use but we also provide expanded care coordination and peer support services to people served. The psychotherapist works/collaborates with other members of the team (Targeted Case Managers, Certified Peers, Psychiatric providers and Registered nurses) to develop a treatment plan that effectively address behavioral, medical and social determinant needs of persons-served.      The ideal candidate will have prior psychotherapy experience in a clinical setting working with a wide-ranging population, including those with: serious mental illness, substance abuse, forensic involvement and/or homelessness.  Some knowledge of community and government resources available to the underserved is beneficial. Fully licensed candidates with 2 years’ experience in an Article 31 setting strongly preferred.  Wellness Works provides paid clinical supervision time and hours toward clinical licensure. The Director of Social Services will; - Manage clinical operations of family shelter based on city, state, and federal regulatory requirements and DHS contractual requirements. - Complete written reports to division management, SUS executive management, and funding source/s as required; clinical progress and highlights, consumer census, housing and employment placement. - Provide supervision, guidance, and performance evaluations to clinical supervisor, housing specialist and program assistant. - Assist in the development of job skills training curriculums suitable to the population served. - Enforce corporate compliance protocols, SUS employee policies, procedures, and protocols, and incident management policies, procedures and protocols. - Maintain CPR/ First Aid and Mandated Reporter Certification. - Monitor the integrity of client data and ensure that internal chart reviews are conducted consistently and regularly. - Participate in professional development and training through agency-wide in-service trainings, outside trainings, and facilitate in-house trainings for staff. - Facilitate periodic staff meetings and case conferences as appropriate.  - Monitor the provision of and documentation of all services provided to clients.  - Participate in division management meetings as needed. - On call 24 hours per day and ensure that there is a contingency plan in place. - Ensure all interactions support the mission statement and core values of SUS. - Additional duties as assigned by the Vice President, and Urgent Housing. - A work schedule that includes some weekend and evening hours will be required.

The Peer Specialist ensures the quality of care for consumers who are living with mental health diagnosis, and provides harm reduction outreach and case management services. The Peer Specialist will interface with representatives from Probation/ Parole in order to encourage consumer compliance with requirements and facilitate communication, act as an advocate for consumers as appropriate; encourage and support self- advocacy. Responsibilities include; develop and facilitate a peer support group for agency consumers, provide referral services through the distribution of materials, information, education and prevention messages to consumers in their homes and/or in group settings.

The Supervisor Maintenance will work at 8 to 12 apartment buildings and at such buildings will be supervisor of each buildings’ maintenance staff (superintendents and porters). These 8 to 12 buildings are located in the Manhattan, the Bronx and Brooklyn and are multifamily apartment rental buildings financed by Low Income Housing Tax Credits where S:US is the owner and property manager. The Maintenance Supervisor will need to work closely with a number of different staff members and teams, including the Program Director at each building, and other social service staff at the buildings, the property management team (located at 1168 Stratford Avenue, Bronx), the S:US Facilities Department and the superintendent and porters at each building.   Job Responsibilities:   - Visits each property at least weekly (and more if needed) and conducts regular inspections of each assigned property to ensure the building is kept to the quality standard of the company: - Supervisors the superintendents and porters at each building to ensure timely completion of work orders and minimize the issuance of municipal violations; - Works with S:US Facilities Department and the superintendent and porters at each building to ensure that violations from HPD, DEP, ECB, FDNY, etc. are addressed and reported to Violations Coordinator; - Works with Program Staff and super and porter to coordinate the "turnover" of vacant apartments in an expeditious manner in order to avoid the loss of any rental income, communicate with SPA Department of S:US re: unit availability and required inspections; - Coordinates with Contracts Manager and Director of Maintenance when outside maintenance or service vendors are needed, and advises Contracts Manager when vendors are not performing satisfactorily; - Ability to be on-call and respond to emergency situations; - Supervises all functions of the physical plant which include plumbing, heating, roofing, and electrical and gas systems; - Conduct supervision sessions with Supers/Porters; - Advises Director of Maintenance or VP of Facilities when outside contractors are needed for repair work beyond the capability of the building staff and coordinates access for contractors and ensures inspections are made of work; - Assists the property management team to ensure all apartment and property inspections are ready prior to and be on site for inspections; - Assists the property management team to review the building inspection reports in conjunction with supervisor, Director of Maintenance and VP of Facilities in order to understand what maintenance problems need to be resolved and improvements that need to be made; and - Represents himself/herself and the property in a professional manner with residents, visitors, and other employees. - Responsible for all Supers handing in the monthly preventive maintenance report; - Responsible for timesheets of all staff under supervision; - Responsible for all Supers submitting their work orders in the Yardi system. - Responsible for adhering to a weekly work schedule for himself/herself and all other staff at the properties; and - Works with the Regional Supervisor, trains the Maintenance Mechanics/Handymen, and Porters/Cleaners in the proper performance of their functions, consistent with company policies, procedures, and standards.

Assists in developing consumer’s ability to maintain individual residents on basic cleaning and safety. this position will be located in the Bronx.   Responsibilities: - Establish and execute a formalized preventative maintenance schedule for the facility, i.e. painting, plastering, plumbing, windows, electrical work, changing locks, masonry, changing lighting fixtures, roofing, stripping and waxing floors, installing shades, etc. - Submit requisitions for necessary maintenance/repair supplies to the Program Director on a weekly basis and control the inventory of supplies - Complete the monthly physical plant and fire safety audit of the facility. - Participate in moving clients in and out of the facility. - Maintain vehicle maintenance/mileage record prior to and upon returning from trips. - Facility Maintenance is responsible for the installation of a Maintenance Start-up Kit for newly acquired apartments. This includes:  smoke detector, carbon monoxide detector, window fire gate, curtain rods, window shades, second deadbolt lock, and fire extinguisher.
