As part of the East Harlem ACT II & III Teams the LPN will take vital signs, such as blood pressure, temperature and weight, follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed, and provide CPR/First Aid .   Job Functions:   - The LPN assesses, monitors, and ensures access to coordinated medical and mental health services. - This position requires you to conduct home visits on a regular basis with 50% fieldwork/travel. ACT Teams are a mobile interdisciplinary team of professionals (psychiatry, nursing, social work, rehabilitation) and peers.   As part of the Behavioral Health team, SUS seeks a Registered Nurse with in the state of New York to provide nursing services within a multi-disciplinary team.   Responsibilities: - Conducts physical health assessments - Assesses the health needs of participants and makes the appropriate referrals to community based medical care  - Manages and administers medication as required - Provides a range of treatment, rehabilitation, and support services to the participants - Meets regularly with each participant to assess health status - Provides health management education - Review treatment and medication history - Communicates with the participant’s primary medical provider to coordinate medical treatment - When necessary refers them to appropriate services as determined as a result of their assessment - Advocate on behalf of participants regarding medical treatment as needed - Provide training to program staff on disease, treatment options, and new advances in disease management - Responsibility for documentation of services provided particularly in development of service plans - Responsible for ensuring and documenting revisions to the plan as the participant’s needs change - Advocating for the rights and preferences of the participant whenever possible

The Regional Director under the guidance of the Vice President or Sr. Regional Director will partner with S:US funders, oversite agencies, and executive leadership to elevate the S:US service deliveries to all stakeholders. They are highly influential in managing all program services including advocacy, housing, case management, and access to other essential services as needed.   ESSENTIAL DUTIES & RESPONSIBILITIES:   Leadership - Works under limited supervision of the Vice President or Sr. Regional Director requiring higher planning skills and project management skills. - Manages and coaches shelter program directors to develop leadership skills and customizes development plans for each individual to optimize consistent competencies throughout their scope of accountability. - Lead directors in adapting and improving service models to be responsive to a changing environment. - Participate in and monitor the recruitment, hiring, and talent development. - Provide program and professional supervision, lead required staff and project management team meetings and trainings, monitor program outcomes. - Participate in division management meetings as needed and facilitate staff meetings and case conferences as appropriate. - Manages special projects and all other assignments as determined by the Vice President or Sr. Regional Director. - Confirms services are delivered in accordance with agency culture, mission, vision and values.                 Data Accountability - Develop, monitors and evaluates all program work plans, outputs and outcomes to ensure goals are met. Identify and implement actions to improve goal attainment as necessary. - Develop and monitor data collection to report consumer outcomes and service effectiveness.                 Compliance - Monitors that each program is meeting regulatory requirements, modeling best practices and integrating new tools and initiatives into its work model. - Enforce corporate compliance protocols, SUS employee policies, procedures, protocols, and incident management policies, procedures and protocols. - Timely submission of reports, inquiries, response to internal and external stakeholders                 Operations - Maintain accountability and operation within established budget. - Remains current and updated on new regulations, policies, industry trends, and best practices. - Monitors quality standards and consumer satisfaction are met and sustained. - Provide routine managerial oversight of programs including expenditure approval, timesheet approval, report submission, staff hiring and performance appraisal, etc. - Develop program policy & procedures governing the provision of services to consumers and day to day program operations. - Monitor the provision of and documentation of all services provided to consumers. - Oversight of training and program specific orientation for new hires                 External Relations - Represents the agency and programs and in a professional manner to funders, consumers, potential consumers, referring agencies, network members, etc. - Market and promote agency programs and services to potential referral sources to maximize capacity. - Perform other duties as required.

Educational; vocational; spiritual; social) and recovery-based supports both within and external to SUS - Teach consumers skills that will permit them maximal independence in maintaining their mental health, income, housing and employment, as applicable. - As needed, serve as an advocate for consumers of the mobile team in obtaining benefits, managing and coordinating their medical and psychiatric treatment and rehabilitation, and overcoming stigma and other barriers to community integration - Maintain up to date, accurate, complete and timely documentation of resident progress in individual client records.

The Case Manager provides and coordinates family services.    - ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct intake process with new family members within 24 hours of admission. Responsibilities: • Assists each consumer in skill development in areas such as independent living, adaptive behaviors, socialization, recreation and activities of daily living. • Supervises consumers in the community, using various experiences to reinforce skills being taught in group session while ensuring their safety. • Leads group sessions with consumers and facilitate the discussions to provide learning skills to be used in their community. • Provides annual and semi-annual progress reports regarding valued outcomes. • Assesses the needs of lower functioning and/or non-verbal consumers through observations and evaluations; their valued outcomes will be developed and addressed. • Provides input and ensures compliance with the valued outcomes in the ISP and Day Habilitation Plan. • Provides weekly documentation of outcomes being worked towards with each individual in accordance with his/her Day Habilitation Plan.   Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays
