SCOPE OF ROLE: ACT is an evidence-based, recovery-oriented mental health service delivery model that utilizes a multi-disciplinary team approach providing intensive outreach-oriented services to individuals with severe and persistent mental illnesses and co-occurring disorders. Utilizing a client-centered approach, the Family Specialist has the lead responsibility for integrating family goals; restoring and strengthening the individual’s unique social group and family relationships. Providing at least 80% of services in the community, the family specialist will work with a team of mental health clinicians to assist individuals in achieving their meaningful goals and independence.   II.
