I. SCOPE OF ROLE: The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require limited on call coverage.   II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Complete medical assessments and communicable disease risk assessments within mandated time frames. • Coordinate delivery and reception of medications with pharmacy. • Track all medications received and administered as well as coordinate return of unused medications to pharmacy • Observe self-administration of medications for detoxification according to established protocols. • Refer for required medical services. • Provide education and consultation to individual clients and/or groups on their health needs. • Assess clients’ medical record and coordinate medical care with multidisciplinary team. • Assure clients’ compliance with prescribe medical regimen. • Complete timely and accurate documentation on medical services rendered. • Keep supervisor informed of all relevant information regarding individuals and the program. • Provide internal training to individuals/staff members. • Participate in weekly protocol meetings. • Other duties as requested by supervisor.

I. SCOPE OF ROLE: ACT is an evidence-based, recovery-oriented mental health service delivery model that utilizes a multi-disciplinary team approach providing intensive outreach-oriented services to individuals with severe and persistent mental illnesses and co-occurring disorders. Utilizing a client-centered approach, the Family Specialist has the lead responsibility for integrating family goals; restoring and strengthening the individual’s unique social group and family relationships. Providing at least 80% of services in the community, the family specialist will work with a team of mental health clinicians to assist individuals in achieving their meaningful goals and independence.   II. ESSENTIAL DUTIES & RESPONSIBILITIES: Clinical Skills • Assumes lead responsibility within the multi-disciplinary team for integrating wellness goals, and services with the task of all team members and for providing wellness psycho-education individually and in groups, when clinically indicated. • Provides clinical services that are client-centered, trauma-informed, recovery-oriented, evidenced based, anti-racist, and gender-responsive. • Advocates for the rights and preferences for individuals within the ACT program. • Attends and participate in daily team meeting to discuss individual’s case management and clinical needs. • Involves family and significant others in the individual’s treatment with the team. • Assess the nature of family and social relationships as well as design interventions required to promote progress towards familial and social wellness. • Facilitate family psycho-education group/sessions to help individual’s families and/or support network learn and have better understanding of mental diagnosis and treatments to support the individual service toward recovery. • Lead the discussion as well as associated interventions around family issues and incorporate findings into service plans. • Completes family needs assessments by identifying history, current relationships and supports, and immediate needs and recommend treatment objectives for the individual’s service. • Provide family conflict resolution strategies   Data and Accountability • Provides 80 percent of treatment contacts in the community, including leading groups when clinically indicated. • Ensures all documentations are completed in a timely manner and remain in compliance with agency and OMH regulations. • Ensures that all progress notes are completed within 24-48 hours of the delivery of service. • Completes documentation including, but not limited to, initial comprehensive recovery-oriented treatment plans and 6 month review plan to show continued progress and/or barriers, 6 month comprehensive assessments, 30 day intake paperwork, and psychosocial. • Provides 24-hour crisis intervention on-call services, on a rotating basis. • Able to effectively collaborate with staff across the agency. • Coordinate services with other specialist on the ACT Team, i.e. Psychiatrist, Substance Use Specialist, Employment Specialist, Nurse. • Follows-up on any aftercare/recommendations for treatment. Escorts/ accompanies individual as needed on appointments (medical, psychiatric, substance abuse).   Compliance • Participates in team meetings, in-service trainings, and in compliance with all online required trainings. • Maintains accurate and up-to-date records, including but not limited to, progress notes, service plans/6- month reviews, assessments, and contact logs- and remain in compliance with agency, OMH and AOT regulations. • Maintains good status in regard to completion of in-service and online trainings. • Ensure all documentation is completed in a timely fashion • Communicate pertinent consumer and operational information to the Team Leader and Senior management • Assists in the planning, developing and implementation of goals and training to further develop participants skills. • Responsible for reporting and documenting all incidents to Team Leader within 24 hours.   External Relations • Maintain relationships with outpatient programs, housing programs, as well as continuing to build relationships with organizations that may be able to assist SUS consumers who need other services. • Models appropriate behavior. Represents the organization in a positive and appropriate manner to outside resources. • Attend case conferences and coordinate discharge planning with hospital. • Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader.     Salary: $55,000-60,000

ESSENTIAL JOB FUNCTIONS: Clinical Skills - Responsible for integrating dual-recovery treatment - Provide expert knowledge and guidance on substance use assessment, diagnosis, and treatment interventions, ensuring best practices are followed - Develop and deliver training programs to educate staff, community workers, and stakeholders on substance use-related topics, including the latest research, treatment modalities, and harm reduction strategies. - Have a strong understanding of substance use disorder assessment, diagnosis, and treatment planning - Conducts assessments for substance abuse services amongst individuals served during initial assessment period and follow-up assessments on a needed basis. - Extensive knowledge of evidence-based practices, treatment modalities, and harm reduction strategies in substance use disorder care. Data and Accountability - Develops, conducts, and monitors treatment interventions for specific target populations and ensure these interventions are incorporated into treatment plans - Performs consumer centered, comprehensive assessments to determine needed services for all consumers; develops both initial and ongoing treatment plans; assigns the most appropriate staff to the individual treatment team and actively participates in all activities in revising treatment plans as the participant’s needs change - Conducts qualitative field visits to ensure consumers needs are concerns are being met in their living environment - Complete Progress Notes, which should be entered into system within 24-48hrs of event date - Support around facilitating groups, as needed Compliance - Ensures program operates in accordance with contractual requirements and other applicable regulations, including program standards and performance improvement plans - Demonstrated ability to develop and deliver training programs and presentations on substance use-related topics - Stay abreast of current substance use policies, regulations, and guidelines at the local, state/provincial, and national levels. Advocate for evidence-based policies and contribute to the development and implementation of effective strategies to address substance use issues. - Participates in quality improvement process, administrative meetings, team meetings, in-service trainings, client staffing and case reviews. Operations - Participate in daily morning meetings and case conferences to discuss overall consumer well being and/or any issues that may arise - Performs on call duties for crisis intervention and rotation of on call telephone - Ensure all documentation is completed in a timely manner in accordance with Agency, & State Regulations - Applies time management skills to work effectively within given deadlines.  Organizes own time and prioritizes workload effectively.  Uses resources wisely.   External Relations - Maintain relationships with outpatient and inpatient programs, housing programs, as well as continuing to build relationships with organizations that may be able to assist SUS consumers who need other services. - Performs other related work as assigned.

SCOPE OF ROLE: The Care Coordinator provides services in the Clinical Home and in the community to HIV-positive clients living with complex medical conditions, severe mental illness, substance abuse and long-term care needs and a history or risk of over-utilizing medical and behavioral health services.
