Position Overview

SCOPE OF ROLE:

The Social worker will be responsible for the overall delivery and coordination of services for Behavioral Health programs. The Social Worker is responsible for interfacing with clinical staff, mental health and medical providers, and relevant community-based organizations to enhance and ensure individuals are in receipt of optimal care and services. The Social Worker will be responsible for the completion psychosocial assessments to assist in permanent stable housing. The Social Worker will be responsible for assessing individuals’ needs and facilitate appropriate linkage to community-based services. Work within a crisis driven Behavioral Health environment with a knowledge of substance abuse, NYC Housing Application (2010E), trauma and dual diagnosis.

Monday – Wednesday 10-6pm, Thursday/Friday 11-7pm
 

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Work from a strength based, person -centered perspective to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.
  •  Assess individuals’ needs and facilitate appropriate linkage to community-based services
  • Must be able to independently assess the psychosocial functioning and needs of clients to formulate and implement a treatment plan, identifying the client’s problems, strengths, weaknesses, coping skills and assistance needed.
  • Responsible for facilitating weekly mental health/ substance abuse workshops and onsite support groups for clients.
  • Work with other members of the team to ensure that appropriate wellness is conducted to engage client’s stability
  • Must have knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  • Must have knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. 
  • Must be able to provide consultation services to other staff about the psychosocial needs of individuals and the impact of psychosocial problems on health care and compliance with treatment. 
  • Must have knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking.
  • Provides crisis intervention and stabilization as neededMay require some after hour’s and or weekend follow-up to emergency situations.
  • Maintains; reviews and signs off on all required documentation/reports and submits all in a timely manner.
  • Identifies program needs and makes recommendations.
  • Ability to work a flexible schedule when needed which consist of weekend or evening hours
  • Comply with all S:US Employee Policies, Protocols, S:US Corporate Compliance Policies, and Department of Mental Health Dept. (OMH) Regulations and Statutes
  • Complete other tasks as deemed necessary by program director
  • Performs related tasks as assigned.

 

Qualifications

·         Current licensure in LMSW required from a school of social work fully accredited by the Council on Social Work Education (CSWE) or current licensure in LMHC

·         Knowledge of emotional/behavioral health, mental health, dual-diagnosis

·         Ability to work independently and as part of a multi-disciplinary team, to assist individuals of diverse ethnicities, backgrounds, ages, and sexual preferences

·         Effective written and oral communication skills.

PREFERRED QUALIFICATIONS & SKILLS

·         Bilingual candidates preferred but not required

·         Basic computer skills (e.g. MS Office suite and Windows)
