SCOPE OF ROLE:
Under the supervision of the Assistant Program Director, the Intake Case Manager will coordinate intake and referral process for adult diversions at various locations in NY, NY. They complete prescreening, intake and eligibility assessments for applicants to the Homebase program sites and offices, as well as in the community. As needed support a caseload of enrolled program participants and support other functions of the homeless prevention services at the program.
