Position Overview

  1. SCOPE OF ROLE:

The Health Navigator position provides services that include connecting HOPWA Program to tenants’ care team. This position provides case management and care coordination, health education, interdisciplinary collaboration and coordination. The Health Navigator works closely with the tenants’ primary care provider and members of the tenants’ medical interdisciplinary team.

 

  1. ESSENTIAL DUTIES & RESPONSIBILITIES:

·       Conducts assessments which should highlight the Tenant strengths, limitations, risk factors and internal/external support and service needs to optimize the tenants’ ability to access and maintain health care services.

  • Act as a liaison between the HOPWA program, community medical clinic and other healthcare providers, coordinating care for a population of tenants with similar complex needs who require assistance accessing health care services or adhering to health care plans. Support participants’ access to S:US healthcare resources and services whenever feasible.
  • Work closely with tenants assigned multidisciplinary team, including medical, nursing and administrative specialists, and case management personnel.
  • Work within HOPWA to provide timely, appropriate, person-centered care in an equitable manner.
  • Work collaboratively with healthcare team and tenants to identify and address systems challenges for enhanced care coordination as needed.
  • Work closely with tenants to assist them in communicating their preferences in care and personal health related goals, to facilitate shared decision making of the tenants’ care
  • Serve as a resource for education and support for Tenants and families and help identify appropriate and credible resources and supports tailored to the needs and desires of the tenant.
  • Escort tenants to Medical Appointments as needed.

Qualifications

    1. REQUIREMENTS:

    REQUIRED EDUCATION AND EXPERIENCE

    ·       Associate Degree in Health Care Required/ bachelor’s degree Preferred

    ·       Effective written and oral communication skills.

    ·       Some knowledge of working with Seniors with ongoing medical issues

    PREFERRED QUALIFICATIONS & SKILLS

    ·       Basic computer skills (e.g. MS Office suite and Windows)
