ESSENTIAL DUTIES & RESPONSIBILITIES:Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. ·         Manage the holistic healthcare, nutrition, and wellness of individuals including tracking appointments, communicating with the CMS, Nurse, relevant doctors and managers.·         Ensures the coordination of all recreation and other community activities that promotes the participation and independence of individuals. ·         Encourages the independence of individuals and promotes relationship building within the community and with natural supports.·         Manages and directs the overall flow of the shift including assigning staff to appropriate individual ratios. ·         Supervise staff (DSPs and Shift Supervisors), while promoting learning and development by providing continual coaching and mentoring. ·         Role model the necessary behaviors that support independence and the aspirational desires of the individuals. ·         Ensures that health and safety requirements are observed including maintaining a stimulating and comfortable physical environment.·         To facilitate individuals’ participation in recreational activities and medical appointments, coordinates all transportation activities for the residence. ·         Manage the personal property of individuals and their ability to access personal items.·         Ensures compliance with regulatory agencies within mandatory time frames. ·         Supports the mission, vision, values of S:US. Services for the UnderServed (SUS) is a New York-based nonprofit organization that provides housing and support services to 25,000 individuals and their families living in New York City and Long Island. SUS believes every New Yorker has the right to lead a life of purpose.    Join our Developmental Disabilities Team in Opening Doors to Independence. We are currently seeking a Program Director for our Pogram located in Manhattan.   The Program Director is responsible for ensuring the overall effective operation of the facility in accordance with applicable Federal and State regulations, Agency policies and procedures, and union contracts.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Day-to-day administrative functions within the facility relative to implementation of SUS policies and procedures, union contracts and regulatory requirements as set forth by funding agents - Directly supervise the Assistant Program Manager on a weekly schedule or as ne - Monitor overall consumer care, ensuring needs are met as required/ prescribed through documented weekly observations of supervisors and program specialists’ implementation of active treatment and strict adherence to Part 633 - Client Righ - Execute agency, state and federal requirements pertaining to custodianship of consumer funds within the facility, including the preparation and submission of weekly account summari - Complete all individual client ledger sheets for in-house accounts on a weekly basis, i.e. posting of deposits, withdrawals and re - Conduct at least monthly physical plant inspections with documentation of finding - Ensure strict adherence to OPWDD Part 624 - Incident Reporting Procedures, including reviewing all incidents, conducting any necessary investigations, and making necessary notifications

NYSTART offers crisis prevention and response services to people who have both developmental disabilities and complex behavioral needs, as well as to their families and those who provide supports.  The NYSTART multidisciplinary treatment team is a dynamic, inspiring and supportive group of professionals who are dedicated to helping families support the people they love and care for by using a systematic and effective approach.  The team prides themselves on building relationships and supports across service systems to help people remain in their homes and communities, and enhance the ability of the community to support them.    As a Clinical Team Coordinator, you will provide groundbreaking crisis intervention and prevention services for individuals with intellectual and developmental disabilities and mental health needs in Brooklyn and Staten Island. This is an exciting opportunity for new and seasoned clinicians to learn and practice the START model - a comprehensive, collaborative bio-psychosocial approach to intervention aimed at mitigating crisis occurrences.   ESSENTIAL DUTIES & RESPONSIBILITIES: Outreach - Provide outreach visits to the individuals’ home, day and vocational settings regularly. - Conduct consultative sessions to Include the individual, their family/caregiver, and all active members of their system of support. - Maintain an active caseload of individuals referred for CSIDD Services (variability occurs depending on activity level of cases). Assessment - Complete individual assessments under the supervision of a Medical Director. - Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans. - Generate intake/assessments, admission/discharge summaries for resource center admissions or inpatient hospital admissions and any other applicable documentation of services provided. - Document all services contemporaneously with the date of service, maintaining up to date records.   Planning - Develop individualized clinical crisis plans and treatment plans based on the results of clinical assessments. - Consult with the team to identify which CSIDD services are appropriate for the individual supported. - Ensure the coordination of support meetings and crisis plan development for individuals enrolled in CSIDD services. - Provide on-call support on a rotational basis as part of a CSIDD team. This requires up to 5 days of on-call duties per month including some weekends. Training & Networking - Develop materials and present cases at Clinical Education Team Meetings as assigned by the Director or designee. - Participate in the Center for START Services National Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended - Perform other duties as required.
