Founded in 1978, S:US works with 35,000 individuals and their families every year to create pathways to rich and productive lives by offering housing, employment, skills-building, treatment, and rehabilitation services.   We are currently seeking a Director of our START Program:   START is a comprehensive model of service supports that optimizes independence, treatment, and community living for individuals with IDD and behavioral health needs. In the 2002 U.S. Surgeon General’s Report on mental health disparities for persons with intellectual/developmental disabilities,    The STARTDirector will provide community-based crisis intervention services for individuals with intellectual and developmental disabilities (IDD) and mental health needs.   Primary Responsibilities: - Supervise Assistant Director, Clinical Director, and Resource Center Director - Establish community linkages - Familiarize and become knowledgeable of regulatory requirements. - Serve as liaison to community network partners - Act as representative of the START team in the community - Coordinate trainings utilizing expertise of psychologists and psychiatrists, team leaders, and specialists within the community - Maintain communication with other regional START directors - Respond to requests from state agencies for data and program information - Provide support clinical team and resource center 24/7/365 - Collaborate with other members of the START leadership team to monitor quality of START service delivery and assure fidelity to the START model - Participate in and complete the START Coordinator Certification training process - Ensure staff onboard meets the ratio of 50% certification rate CSS requirement. - Enter, monitor and assure data entry for all team members in the START Information Reporting System. Use data to inform programmatic decision-making. - Interpret changes/revisions of all regulatory standards as they occur. - Develops and/or revises CSS plans for individuals requiring START services. Ensure timely team submission of BPIR tools and follow-up with tier level from the Regional Office. Monthly treatment plan review and ensure service delivery notes. Service delivery note needs to reference amount of contact received specific to BPIR tier level.  - Ensure program fiscal viability- CSIDD billing structure. Ensure regional office approval for SPA/CSIDD services within five business days. - Maintains and organizes monthly tracker is submitted to local regional office with active cases and tiers level. - Access crisis team services as deemed necessary to stabilize individuals in crisis. - Access stakeholders and community referrals to support NYSTART team and individual enrolled in service. - Ensures operational efficiencies and manage staff caseloads. - Arrange in collaboration with case manager specialty services as needed based on treatment plan recommendations. - Manage SPA/CSIDD NYSTART staff schedules and meeting the need for Tier 1-4 enrollees. Assures recommended contact with those served based on BPIR tier score level. Billing documentation contemporaneous.

The Registered Nurse ensures the health and physical well-being of the consumers.   Responsibilities:   - Participates as appropriate in the development, review and update of an Individual Program Plan as part of the Interdisciplinary Team process - Participates in pre-admission screening and evaluation of ICF-referred Consumers - Develops an initial nursing assessment within 24 hours of placement at a facility - Develops a Nursing Service Plan based on a Nursing Assessment, Physical Exam Report, Evaluation of Consumers Ability to Self Medicate and ITT input, within 30 days of placement, and annually thereafter - Reviews and updates Nursing Service Plans semi-annually and as needed based on needs derived from completed nursing evaluations and quarterly nursing physical assessments - Ensures that nursing care, as prescribed by the physician or as identified by individual needs, is carried out - Participates in the development of discharge plans - Makes referrals to appropriate community resources based on the completed medical portion of the consumer’s Community Service Plan - Assesses consumer’s health condition as needed, and determines the need for referral to physician or emergency room based on observation and/or staff provided information - Ensures control of communicable diseases and infections - Supervises medication administration periodically, ensuring adherence with OMRDD regulations and agency policy - Report all injuries to management immediately to ensure appropriate incident reporting procedures are followed - Completes quarterly Quality Improvement Reports and ensure resolution to any outstanding issues - Reviews and follow up on Quality Improvement Reports completed by the Medical Assistant - Completes semi-annual and annual nursing ITT report - Participates in annual and semi-annual interdisciplinary team meeting along with any special meetings within or outside the residence - Participates in the monthly review and reordering of consumers’ medication together with the primary psychiatrist and facility psychologist - Ensures proper storage of all medication and medical supplies, and the proper disposal of the same (in accordance with OMRDD regulations and/or agency policy) - Reviews and monitors, for the purpose of health maintenance bowel movement records, vital signs, seizure activity and records, menstrual records, and tardive dyskenesia records on an ongoing basis.  Reports irregularities or abnormalities to the consumer’s primary physician as needed - Ensures ongoing health care services are provided as needed and medical appointments are scheduled in a timely manner - Supervises, assists and trains a qualified program specialist to become a Nursing Assistant - Supervises, assists and trains Licensed Practical Nurses to include a review and signing of all written reports for accuracy prior to being presented to ITT Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays

You will be joining a dedicated workforce of 2,400 employees serving over 35,000 individuals and families in New York City. Scope of role The APD is responsible for ensuring that our individuals have the supports and resources necessary to live meaningful lives, and that they align with their needs and aspirations within both their respective homes and communities. The APD is responsible for ensuring that the individuals’ wellness, nutritional, and healthcare needs are met. He/she will lead their assigned shift, providing hands-on coaching and role modeling best practices and behaviors for Direct Care Professionals (DSPs) as well as providing 24-hour assistance in periods of high demand.  In collaboration with a trans-disciplinary team member and under supervision of the Program Director, the APD is expected to promote an environment of respect, learning and independence for the individuals.  He/she will interact with individuals and colleagues in a manner that stems from dignity and with hopes of fostering a true home atmosphere for the individuals.  In addition, he/she will provide coaching to DSPs and Shift Supervisors so that their actions ensure the health and wellness of the individuals, support their independence, self-determination and right to take reasonable risks. The START Coordinator will provide community-based crisis intervention and prevention services for individuals with intellectual and developmental disabilities and mental health needs.   Responsibilities: Daily OnCall Responsibilities: All coordinators will be OnCall approximately 3 times each month and back up at least 3 times per month based on the needs of the program.   There may be times due to vacations, illness, holidays when OnCall responsibilities may increase to 4 times each month to meet the needs of the program.   Weekend OnCall responsibilities: All coordinators will be on a weekend (Friday 9:00 am to Monday 9:00 am) OnCall rotation approximately every 6-10 weeks. There may be times due to vacations, illness, holidays, when weekend OnCall responsibilities may be less than 6 weeks based on the needs of the program.  - Must work a designated number of hours each week - Participate in didactic trainings and additional trainings as required - Provide systemic consultation - Participate in National Center for START Services trainings and events - Prepare agendas and document outcomes for individual mentoring sessions held with Center for START Services - Maintain linkages and relationships with community partners - Assist with referrals for consultation and treatment as needed - Ensure the coordination of support meetings and crisis plan development for individuals served through START - Participate in recurring meetings with START leadership, clinical team, and Resource Center program - Provide home visits and visits to day and vocational settings as needed - Develop Comprehensive Service Evaluations, Cross-System Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans for Resource Center admissions or inpatient hospital admissions and any other applicable documentation of services provided - Maintain an active caseload of individuals referred for START Services (variability occurs depending on activity level of cases and the project scope) - Provide on-call support on a rotational basis as part of a START Team Participate in the START Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays
