The incumbent will provide and ensure nursing supervision to maintain medical/nursing standards as prescribed in Federal and State regulations and agency policy through nursing supervision.   Essential Job Functions: - Manage RN’s and LPN’s in nursing activities, including scheduling, training, evaluating effective work performance.   - Provide guidance to RN’s and LPN’s being visible in the residences and programs to engage and support staff.  - Provides orientation and continuous in-services training to RN’s and LPN’s as it relates to professional development, new-hire orientation facilitating trainings for health and safety modules, revised agency policies and/or revised agency policies and or OPWDD Regulations; leads training with staff via a variety of platforms: live training, virtual etc. - Monitor Pharmacy services – identified service issues; addresses issues as needed. - Monitor Nutritional services.  Ensures annual diet and consistency orders are signed by the health care practitioner.  - Choking prevention – ensures mealtime needs are assessed by a licensed clinician, plan changes added to plan of nursing services, ensures site nurses train program specialists and program/assistant directors.  - Interpret changes/revisions of all medical/nursing regulatory standards as they occur. - Develops and/or revises Plan of Nursing Services- RNs are responsible for the development of an individualized Plan of Nursing Services (PONS) for any individual who requires nursing care, including those who require medication administration for diagnosed medical conditions. - Serve as a clinical subject matter expert to address special medical / nursing needs.  Assesses the quality of care rendered and ensures sound nursing practices - Arrange specialty services and second opinions as needed for health concerns.  This includes arranging for physical therapy and speech therapy assessments and observations. - Educate site RN’s on Precision Care electronic health record utilization. - Ensure site RN’s are updating medical diagnosis and changes in plan of care into Precision Care electronic health record.  Ensures program upload of medical referrals on a weekly basis, update medical record changes, data collection completeness of multiple variables & health indicators (blood pressures, weights, medication usage, medication errors, ER use, hospitalization days, demographics, Infection Control, etc.) - Follows-up and/or processes individuals hospital admission and discharge from hospital/rehab.  Ensures ER or admitting hospital unit has medical history and current medication list.  Coordinates hospital discharges to ensure knowledge and implementation of medication changes, new diagnosis, plan of nursing services revisions, adaptive equipment, determine safe discharge status, review required medical follow-ups, contact with primary care practitioners with admission and discharge status.  Collaborates with the clinical team level of supervision when individual returns to the residence. - Gather data points for monthly, hospitalization, ER visits, new diagnosis, and infection control to the Senior Vice President of DD Health and Clinical Services. - Available 24 hrs. / 7 day via cell phone as per medical emergency procedure/protocol, Telephone Triage Protocol and for consultation as needed. - Conducts internal auditing independently through chart and precision care reviews. Submits report with any concerns Senior Vice President of DD Health and Clinical Services.  Chart reviews must occur at least quarterly to ensure medical follow through and appointment compliance. - Collaborate with BIS and clinical management teams regarding the provision of individual services. - Ensure communication with changes in health status or plans with external providers of care.  - Attend state survey inspections and provides support to the nursing department and clinical team. - Communicate to supervisor any critical health concerns. - Anticipates change in schedule and case load to assist the division with critical health events.  Assists with day-to-day functions of the nursing department as needed. - Report nursing gaps in service and deficits immediately to Regional Director and Senior Vice President of DD Health and Clinical Services.  - Reviews and ensures the integrity of medical and flow charts.  - Follows HIPAA laws and ensures health information is protected. - Any additional responsibilities as deemed appropriate by the Senior Vice President of DD Health and Clinical Services.

The Registered Nurse ensures the health and physical well-being of the consumers and will; - Participate as appropriate in the development, review and update of an Individual Program Plan as part of the Interdisciplinary Team process - Participate in pre-admission screening and evaluation of ICF-referred Consumers - Develop an initial nursing assessment within 24 hours of placement at a facility - Develop a Nursing Service Plan based on a Nursing Assessment, Physical Exam Report, Evaluation of Consumers Ability to Self Medicate and ITT input, within 30 days of placement, and annually thereafter - Review and updates Nursing Service Plans semi-annually and as needed based on needs derived from completed nursing evaluations and quarterly nursing physical assessments - Ensure that nursing care, as prescribed by the physician or as identified by individual needs, is carried out - Participate in the development of discharge plans - Make referrals to appropriate community resources based on the completed medical portion of the consumer’s Community Service Plan - Assesse consumer’s health condition as needed, and determines the need for referral to physician or emergency room based on observation and/or staff provided information - Ensure control of communicable diseases and infections - Supervise medication administration periodically, ensuring adherence with OPWDD regulations and agency policy - Report all injuries to management immediately to ensure appropriate incident reporting procedures are followed - Complete quarterly Quality Improvement Reports and ensure resolution to any outstanding issues - Review and follow up on Quality Improvement Reports completed by the Medical Assistant - Complete semi-annual and annual nursing ITT report - Participate in annual and semi-annual interdisciplinary team meeting along with any special meetings within or outside the residence - Participate in the monthly review and reordering of consumers’ medication together with the primary psychiatrist and facility psychologist - Ensures proper storage of all medication and medical supplies, and the proper disposal of the same (in accordance with OPWDD regulations and/or agency policy) - Review and monitor, for the purpose of health maintenance bowel movement records, vital signs, seizure activity and records, menstrual records, and tardive dyskenesia records on an ongoing basis.  Reports irregularities or abnormalities to the consumer’s primary physician as needed - Ensure ongoing health care services are provided as needed and medical appointments are scheduled in a timely manner - Supervise, assists and trains a qualified program specialist to become a Nursing Assistant - Supervise, assists and trains Licensed Practical Nurses to include a review and signing of all written reports for accuracy prior to being presented to ITT
