S:US is seeking a Program Director for their men's shelter located in Brooklyn.   This position is located at 1000 Blake Avenue Brooklyn, NY 11208     The Program Director will:   - Manage and monitor all aspects of program start-up based on DHS contractual requirements and SUS guidelines. - Manage and monitor daily program operations of the shelter based on city, state, and federal regulatory requirements and DHS contractual requirements. - Manage and monitor ongoing expenditures, resource allocation, and utilization. - Complete written reports to division management, SUS executive management, and funding source/s as required; program progress and highlights, consumer census, housing and employment placement, staffing, and service utilization. - Provide supervision, guidance, and performance evaluations to Director of Social Services, and Facility Manager. - Develop administrative program guidelines and protocols as appropriate. - Assist in the development of job skills training curriculums suitable to the population served. - Develop program policy & procedures governing the provision of services to consumers and day to day program operations. - Enforce corporate compliance protocols, SUS employee policies, procedures, and protocols, and incident management policies, procedures and protocols. - Develop and maintains a crisis/incident management system. - Ensure all health and safety guidelines including staff CPR/First AID Certifications, Fire Safety Coordinator trainings, and Mandated Reporting training are all in compliance. - Maintain CPR/ First Aid and Mandated Reporter Certification. - Monitor the integrity of client data and ensure that internal chart reviews are conducted consistently and regularly. - Participate in professional development and training through agency-wide in-service trainings, outside trainings, and facilitate in-house trainings for staff. - Participate in and monitor the recruitment, hiring, and selection of direct care and supervisory personnel. - Facilitate periodic staff meetings and case conferences as appropriate.  - Monitor the provision of and documentation of all services provided to clients.  - Participate in division management meetings as needed. - On call 24 hours per day and ensure that there is a contingency plan in place. - Ensure all interactions support the mission statement and core values of SUS. - Additional duties as assigned by the Vice President, AIDS Services and Urgent Housing. - A work schedule that includes some weekend and evening hours will be required.

The Quality Assurance Investigator is responsible for conducting investigations involving consumers/employees in the DD, BH, HS, TS and VT Divisions based on incidents/special issues.     - SCOPE OF ROLE: The Support Services functions at S:US are carried out by a team of professionals with differing backgrounds who share deeply in the S:US mission. You will be responsible for conducting investigations in all of our service delivery areas, including Developmental Disabilities, Behavioral Health, and Homeless Services. You will be expected to interview all relevant parties, review documentation, analyze all of the pertinent information, and then write a well thought out, thorough and organized investigation report, all while meeting regulatory time frames for the completion of investigation reports. You will also ensure adherence to the S:US mission, core values, and code of ethics.   - ESSENTIAL DUTIES & RESPONSIBILITIES: Investigation Activities - Conduct investigations involving individuals served by S:US, as well as S:US employees and contractors, in all of our service delivery areas, including Developmental Disabilities, Behavioral Health, and Homeless Services.   - Schedule and conduct interviews with all relevant parties.   - Review and analyze various reports and documents pertinent to the investigation, and follow up on missing pertinent information   - Complete written investigation reports as per established protocols and regulations.   - Participate in arbitration or court hearings based on investigations when required   Audits/Reviews - Participate in internal audit reviews at various programs.   - Conduct site visits to ensure compliance with all regulatory mandates.   - Conduct follow up audits to confirm corrective actions have been implemented and are effective.   - Draft written reports summarizing findings of various audits/reviews.   - Help to develop audit tools to be used in various audits/reviews. The Peer Specialist’s role will involve community outreach on the streets and subways, coordinating participants needs before and after their move from street to home, enhancing their daily living skills, accompaniment to appointments, and advocating on their behalf when faced with discrimination or healthcare inequities. Member choice, harm reduction, non-coercion, flexibility and person-centered care are essential elements of the SOS program model and should be front and center of the care delivered by the Peer Specialist.   Job Responsibilities:   - Persistent and assertive outreach and engagement using strength-based approaches beginning either at known “hang-outs” or “Hot spots” within the transit system or during an inpatient hospital admission or emergency department visit; - Continuously assess the health and social needs of participants through SOS’s conversational and observational assessments and formalized risk assessments tools for those identified as being at high risk; - Work in collaborations with the centralized SOR Hub to identify available housing and to support participants through the process. Tasks may include completing HRA 2010e, applying for housing, prepping for interviews, follow up with housing providers, and assistance with moving in (day of move) with obtaining housing supplies and learning the neighborhood; - Participate in hospital discharge planning meetings to identify the best community resources for returning patients; - Collect and report data, as required and work with team leader, data analyst and other SOS teams to use data to inform future care delivery; - Once housed work with participants and their housing providers to resolve clinical issues that are impacting on the participant’s ability manage, and retain supportive housing; - Foster relationship with community provides to ensure that recipients are connected with appropriate services as they transition back into the community; - Appointment navigation including accompaniment to appointments, travel training, reengagement in community care, and addressing barriers to care; - Review documentation and conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing and other social needs in the community; - Obtain historical and collateral information from multiple sources to support participants behavioral and physical health needs; - Monitor, evaluate and record participants progress with respect to care plan goals; - Attend and participate in team meetings and supervisory sessions. - Perform other related duties as assigned.

S:US Is seeking a Porter for their men's shelter in Brooklyn.   This program is located at 1000 Blake Avenue Brooklyn, NY 11208   The Porter will be responsible for maintaining cleanliness ande minor repairs in a residential shelter.   Responsibilities: - Maintain cleanliness in assigned areas within the facility.  This includes but is not limited to; floor care (mopping, sweeping, buffing, waxing), cleaning cafeteria and kitchen areas, bathrooms, emptying trash, separating recyleables, emptying garbage, wiping windows and walls, and cleaning stairways and/or elevators. - Carry out minor repairs and building maintenance throughout assigned areas of the facility.  This includes but is not limited to; painting, plastering, repairing furniture, maintaining smoke detectors and fire extinguishers. - Responsible for exterior snow and ice removal as well as upkeep of the grounds.  This includes but is not limited to sidewalk maintenance, mowing grass, trimming shrubbery, maintaining landscaping, rodent removal, and all other aspects of the building exterior. - Conduct periodic building, dorm, and office inspections for dangerous and hazardous conditions, needed repairs, hygiene, and cleanliness. - Conduct periodic inspections of fire extinguishers, smoke detectors, and sprinkler system to ensure equipment is working properly and in compliance with regulations. - Maintain building maintenance tools and machinery to ensure that all items are in good working condition. - Responsible for laundering bed linens, bathroom towels, kitchens towels, and all other washable items used in the facility by consumers and staff. - Assist with periodic boiler and water heater inspections to ensure proper functioning at all times.  - Assist in moving and storing program supplies, office furniture, office equipment, dorm furniture, consumer belongings, and other items in and around the facility when and where appropriate. - Assist in interior decorating and refurbishing of assigned areas of the facility. - Responsible for insect and bug extermination and rodent removal.  - Drive agency vehicles. - Must wear SUS maintenance uniform and must maintain uniform so that it is clean and presentable at the start of each shift. - Maintain CPR/ First Aid Certification. - Complete other tasks as deemed necessary by the House Manager, Program Director and/or Assistant Director of Facilities. - Attend staff meetings, case conferences, group supervision, and trainings as needed. - A work schedule that includes some weekend and evening hours will be required.

This position is located at 1000 Blake Avenue Brooklyn, NY 11208.   The Senior Case Manager will oversee the Case Managers and Housing Specialist.  He/she will provide supervisory oversight and guidance to Case Manaagers  in the development of initial service plans and service plan reviews, referring clients for in-house and community-based medical, mental health, and supportive social services, and referring clients for in-house and community-based medical, mental health, and supportive social services.  Will provide oversight and training to Case Managers in assisting clients with the financial entitlement and medical benefit application process.  The Senior Case Maanger will provide supervisory oversight and training to Case Managers and Housing Specialists in group and individual counseling, workshops, and recreational activities.  Will schedule and assign groups and activities to Case Managers as needed, and run groups as necessary. The minimum and maximum salary for this position is $17.68 per hour.   This opportunity is available at our program located on Vernon Ave in Brooklyn NY.   Responsibilities: - Provide habilitation services to each client as prescribed in that client’s service plans (CFA, Individual Service plan, Habilitation plan) - Assist each client, as needed, in skill development in areas such as activities of daily living, domestic living, recreation and socialization - With appropriate training and supervision, administer medication to clients as assigned and in compliance with management, medical and nursing policies and procedures - Ensure assigned clients’ nutritional needs are met according to prescribed diet, and assist with feeding as necessary - Report immediately any observed/known deficiencies or own inability in providing habilitation services to clients, as well as all types of abuse and other facility problems, including physical plant/safety issues to management   Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays
