The Housing Specialist will:
- Interview clients to assess their present housing needs and status and submits appropriate applications to community resources.
- Conducts groups to discuss available housing options, how to secure an apartment and how to handle issues related to housing.
- Accompanies residents to housing interviews, housing fairs, and to complete housing applications when necessary.
- Provides housing resources (i.e. phone numbers to landlords, housing programs, etc) to clients and acts as a liaison between landlords and clients.
- Refers residents to get PPD and physicals completed within two weeks of their entrance into the facility.

The Assistant Program Director (APD) is responsible for ensuring that our individuals have the supports and resources necessary to live meaningful lives, and that they align with their needs and aspirations within both their respective homes and communities. The APD is responsible for ensuring that the individuals’ wellness, nutritional, and healthcare needs are met. He/she will lead their assigned shift, providing hands-on coaching and role modeling best practices and behaviors for Direct Care Professionals (DSPs) as well as providing 24-hour assistance in periods of high demand.

- Supervision includes, but is not limited to, ongoing individual supervisory meetings; building inspections, ongoing feedback regarding job performance; coordinating work schedules, timesheets and requests for time off; coordinating work assignments; evaluating paperwork and documentation including the Security Log and Maintenance Work Order Log books; coordinating security and maintenance staff training, development and evaluation. May be required to fulfill duties of residential aides and/or custodial personnel in their absence.
- Responsible for maintaining fire safety which includes:

SCOPE OF ROLE:
The Community Coordinator is committed to removing the client's barriers to care by identifying critical resources for our persons served, helping them navigate through health care services and systems, and promoting consumer health. The Community Coordinator provides housing, wellness, and stabilization services to the person’s served on the SUS ACT Teams. The Community Coordinator supports the Team Providers, Psychiatrist and Nurse Practitioners, by facilitating telehealth video calls between the providers and persons served to mental health care services. The Community Coordinator will also support the team by coordinating service needs, processing HRA applications, scheduling medical and mental health appointments, and assisting with other program requirements. The purpose of the Community Coordinator is to quickly and effectively assist person’s served with transition to housing, coordinate mental health needs with the health center as they graduate from ACT Services, and provide general support based on needs.

- Facilitates telehealth video calls for team providers on a monthly and/or quarterly basis.
- Assist with tele-visits to ensure clients are trained to do telehealth

SCOPE OF ROLE:
Experienced Clinician to work on a NY State Office of Mental Health (OMH) funded hospital-based care transition team that exclusively serves mentally ill inpatients at Bronx Psychiatric Center (BPC). The role will involve addressing the preparatory needs of participants in the early stages of discharge planning, development of daily living skills, and coping mechanisms through group and individual work on the hospital units. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The role will require on call coverage and a willingness to work flexible hours.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Hospital engagement beginning on the unit, involvement in discharge planning, and a needs assessment of community transition supports essential to stabilizing the individual;
- Short term counseling (substance use, coping skills, trauma informed, decision making) and Risk Assessment/Safety Planning;
- Running groups and life skill training for patient population;
- Participate in both the development and implementation of a community care plan that includes formal and informal supports that increase opportunities for improved quality of life. Focus is on creating meaningful and productive daily activities in the community;
- Assessing Housing Supports and Housing Assistance and Placement for those who are homeless. Services may include completing HRA 2010e, applying for housing, prepping for interviews, follow up with housing providers, and assistance with moving in (day of move) with obtaining housing supplies and learning the neighborhood;
- Work with recipients and their housing providers to resolve clinical issues that are impacting on the recipient’s ability to obtain, manage, and retain supportive housing;
- Foster relationship with community provides to ensure that recipients are connected with appropriate services as they transition back into the community and to share or collect collateral information;
- Patient navigation by accompanying to first behavioral health and medical appointment, travel training, reengagement in community care, referral to services with ability to identify and address potential services and barriers to obtain and maintain such services
- Review documentation and conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing and other social needs in the community. Obtain historical and collateral information from multiple sources as it relates to mental and physical health, substance use, and criminal history;
- Develop a short-term person-centered treatment plan to assist the recipients’ towards achieving their goals and modify treatment plans according to changes in recipients’ status.
- Monitor, evaluate and record recipient progress with respect to treatment goals;
- Establish collaborative working relationships with inpatient treatment teams, community providers, and other partners and plan with them for appropriate discharges for recipients;
- Counsel recipients in individual and group sessions to assist them in dealing with substance abuse, mental and physical illness, family issues, housing problems and unemployment;
- Provide ongoing family conferences and psychoeducation to families and others within the support network;
- Provide out of hours crisis intervention services to recipients and their support network, including respite referrals and other diversion and stabilization services;
- Attend and participate in team meetings and supervisory sessions;

Responsibilities:
- Provide habilitation services to each client as prescribed in that client’s service plans (CFA, Individual Service plan, Habilitation plan)
- Assist each client, as needed, in skill development in areas such as activities of daily living, domestic living, recreation and socialization
- With appropriate training and supervision, administer medication to clients as assigned and in compliance with management, medical and nursing policies and procedures
- Ensure assigned clients’ nutritional needs are met according to prescribed diet, and assist with feeding as necessary
- Report immediately any observed/known deficiencies or own inability in providing habilitation services to clients, as well as all types of abuse and other facility problems, including physical plant/safety issues to management

The Residential Specialist staff member will be responsible for ensuring a safe environment and that tenants are adhering to House Rules and program policies, monitoring surveillance systems, and acting as a role model to tenants and encourage program participation. In this role you will provide necessary support to residents for a variety of issues, make hourly observation rounds and document in the communication log, communicate incidents to supervisory staff via written incident reports and communication log, and attend staff meetings. You will also provide frontline crisis intervention, answer phones and route calls as needed, ensure reception area is tidy, and perform other duties as requested by supervisor.

ESSENTIAL DUTIES & RESPONSIBILITIES:
Content Creation across platforms
- Establish and maintain collegial working relationships with programs and support services departments to identify compelling S:US stories, announcements, and expertise for content development.
- Identify, create, and package stories of recovery, resilience, and success from within S:US for content across platforms and for pitches to media.
- Manage the creation and production of print and digital materials, such as advertisements, flyers, brochures, banners, and posters, for distribution to program participants and the public.
- Design and create collateral materials as needed for fundraising purposes, public/community relations efforts, sponsorship ads, and internal events.
- Develop and produce new videos that showcase the S:US story for internal and external stakeholders.
- Support the work of the Communications & Marketing Director as well as the Development & External Affairs Department as needed.
Digital and Print
- Directly monitor and update content on the S:US website to reflect compelling stories, updates, and announcements.
- Manage day-to-day social media engagement including content creation (graphic design and writing) and gathering analytics.
- Plan, write, design, and distribute monthly external emails to funders and supporters for awareness, marketing, or fundraising purposes; manage subscriber email lists and use analytics to monitor engagement.
- Support Development & External Affairs Department in the creative promotion of proposals and events.
Internal Communications
- Oversee the organization’s intranet by posting all staff email announcements, events, updates, documents, and performing daily maintenance.
- Assist in the planning and management of internal communications efforts to ensure the S:US culture is supported and promoted.
- Compose and edit staff emails and announcements, design flyers and event invitations, and create digital graphics.

KEY PERFORMANCE STANDARDS

Position Summary of the Senior Service Coordinator:
- Conduct comprehensive assessments of veterans/veteran families’ needs.
- Directly supervise up to four SSVF Service Coordinators/field staff.
- Develop, implement, monitor, and evaluate individualized housing stability plans with each client.
- Provide short-term intensive case management to veteran families to achieve their housing stability goals.
- Link each program participant with appropriate community supports to ensure their housing stability.
- Assist program management in identifying the training needs of junior SSVF staff.
- Assist the social service team in the adoption of emerging best practices for the delivery of core support services to Veteran participants.
ESSENTIAL JOB FUNCTIONS
§ Engage program participants by maintaining regular contact to monitor and track progress and response to services, including home visits and field work.
§ Develop and implement individualized housing stability plans with veterans/veteran families to address identified housing barriers for Homelessness Prevention and Rapid Rehousing services.
§ Monitor and evaluate client progress and adjust housing plan and/or service delivery as necessary.
§ Provide regular supervision for SSVF direct reports, including support and guidance for workplace performance and behavior, troubleshooting and reducing barriers to services.
§ Complete all necessary documentation to ensure compliance with funding requirements, and agency quality assurance standards.
§ Performs comprehensive assessments on clients according to SSVF and agency standards.

§ Coordinate Rapid Rehousing and Homelessness Prevention services with program participants and the social service team.
