Promote the provision of a safe, comfortable, secure and trusting environment where guests are listened to and respected by staff; practice the Intentional Peer Support {IPS} Draw on knowledge and skills acquired from personal experience with the behavioral health system with a focus on guest's personal recovery process; Create a sense of community through the mutual sharing of lived experiences and offer encouragement and support to participate in daily activities within the BCR. Assist with pre-registration process for both individuals referred from hospitals, other treatment providers and those who self-refer; Assist with intake and registration of new guests, including an explanation of services and expectations, a tour of the premises, introduction to other staff and guests. Assist and collaborate with Health Home and other community provider staff when appropriate to help guests identify, plan for and realize short-term personal goals and priorities while in the Crisis Program. Facilitate individual and group-based educational, recovery, wellness and skill building opportunities including: wellness self-management, WRAP, peer support, harm reduction, and other social or leisure activities. Follow-up with guests to monitor and support progress toward goal-achievement Advocate for and facilitate access to needed health care services, proactively addressing potential barriers such as missed appointments, transportation, fear and stigma, and communication with professional staff. Assist guests with taking care of their ADL needs and participate in the maintenance of a clean-living environment. Responsible for timely and accurate documentation of interactions with guests, services provided, critical information and events, contacts with collaterals and referrals, as required by SUS policies and the OMH Crisis Program Provide household upkeep duties as needed. Support with meal preparations for guests and assist guests with accessing snacks, toiletries and food items daily and encourage guest participation. Participate in data collection and recording related to outcomes, outreaches and other data tracking collection. Participate in supervision, team meetings, trainings, case conferences and other relevant meetings as required by SUS. Provides flexible evening, night, weekend and holiday coverage.

  Services for the UnderServed (S:US) is currently seeking a Diversion Specialist for our homeless prevention program.   This position is located at 516 West 181st Street (St. Nicholas Ave) New York, NY 10033     The Diversion Specialist will coordinate the intake and referral process for adult diversions at DHS identified shelters.    Essential Functions: • Assess shelter applicants directly referred from single and adult family intake centers; serve as the first point of contact. • Enter all Intake and referral information in CARES in a timely manner for Diversions and Aftercare. • Develop individualized housing plan. • Conduct home visits for furnished rooms/apartments. • Provide general case management including coordination and delivery of services relating to housing stability. • Entitlement referral and advocacy. • Meet with participants as needed for follow-up and recommendations. • Meet daily targeted quota for program. • Assist participants in identifying, locating, obtaining and exploring alternatives to shelter (reuniting with family/friends, return to previous living situation, refer to relevant treatment facilities, room rental etc) and retaining suitable housing. • Participate in meetings to promote the program. • Cultivate, develop and maintain productive relationships with referral agencies that are related to retaining housing. • Actively participate in internal and external training programs and committees. • Assure program participants’ rights are protected. • Perform such other duties as may be required. • Maintain case notes for each interaction with clients in CARES. • Complete all individual Diversion folders prior to submitting to Supervisor for review. • Participate in bi-weekly supervision. •Other duties as assigned.

S:US Is currnetly seeking a Shift Supervisor for our families with children shelter located in the Bronx.   This program is located at 240 E 178 Street Bronx, NY 10457   The Shift Supervisor will: - Supervision includes, but is not limited to, ongoing individual supervisory meetings; building inspections, ongoing feedback regarding job performance; coordinating work schedules, timesheets and requests for time off; coordinating work assignments; evaluating paperwork and documentation including the Security Log and Maintenance Work Order Log books; coordinating security and maintenance staff training, development and evaluation. May be required to fulfill duties of residential aides and/or custodial personnel in their absence. - Responsible for maintaining fire safety which includes: - Coordinate Fire Safety plan and training all staff on Fire Safety Procedures and Building Evacuation. - Coordinate monthly fire drills and maintaining fire drill logs. - Periodic building inspections checking for any fire safety hazards or violations and all systems related to fire prevention (centralized fire alarm panel, sprinkler system, smoke detectors and fire extinguishers). - Coordinate servicing and documenting service/inspections of all fire safety equipment. - Provide all new program clients with written fire safety and prevention information. - Perform administrative duties as requested, including but not limited to: - Complete transfer paperwork - Complete intakes with new clients - Compete Incident Reports - Maintain visitor log books - Maintain vehicle sign outs sheets - Maintain roster in CARES and accurately record bedding - Maintain mail logs - Correspond via email - Establish and maintain appropriate staff/client boundaries and direct clients to the appropriate staff or department in order to address their needs. - Communicate regarding incidents and client observations and interactions with Facility Manager, Case Managers, Senior Case Managers, Director of Social Services and Program Director as needed. - Assist with new family arrivals, apartment turnovers and complete intakes and program orientation. - Monitor client behavior to ensure compliance with House Rules, Program Policies, and Procedures. - Monitor facility exterior and interior to ensure client safety, prevent damage and/or theft of agency property, and to prevent fire or other emergency/crisis situations. - May be responsible for cleaning program areas after or before usage. - Assist with move outs, curfew violators and new residents. Complete intakes with new residents. - Provide guidance to clients in proper upkeep of living quarters, personal hygiene and grooming, appropriate attire, attendance at meals, in-house workshops, and participation in other support service activities. - Maintain, prepare and distribute hygiene supplies, donations and living supplies to clients. - Drive program vehicle to drop off/pick up shelter residents at appointments, housing interviews, job interviews, training sites, partnering organizations, etc. as assigned. - Conduct periodic building and grounds inspections and make facility manager aware of findings. - Respond to crisis and emergency situations and contact the appropriate authorities as needed. - Participate in ongoing individual supervision, team meetings, and trainings as appropriate. - Adhere to on call system to facilitate crisis intervention, emergencies, and other situations that require immediate action. - Maintain CPR/ First Aid Certification - A work schedule that includes weekend and evening hours will be required. S:US is seeking a Residential Specialist at our men's shelter in Brooklyn. This position is located at 599 Ralph Avenue Brooklyn, NY 11233.   The Residential Specialist staff member will be responsible for ensuring a safe environment and that tenants are adhering to House Rules and program policies, monitoring surveillance systems, and acting as a role model to tenants and encourage program participation. In this role you will provide necessary support to residents for a variety of issues, make hourly observation rounds and document in the communication log, communicate incidents to supervisory staff via written incident reports and communication log, and attend staff meetings. Peer Specialists intentionally use language in the service of listening to understand and making space for complex personal stories of recovery and resilience. - Advocacy: Peer Specialists advocate with and for program participants both in the larger community and on the team. - Co-Creating Support and Wellness Tools: Collaborate with participants to clarify their personal visions and develop their wellness toolkit. Along with other team members, Peer Specialists support participants in strengthening their self-awareness, building life skills, and connecting to resources and community outside of the S:US team. - Influencing Team Culture: Whenever possible, positively influencing the team culture by advocating for clients, promoting a youth friendly approach and encouraging the use of recovery oriented language. S:US Peer Specialists also work actively within the team itself to help build an environment that operates from a belief in the human potential to grow and an understanding of human diversity. - Team Communication: Peer Specialists maintain open and frequent communication with the team.
