The Veteran Peer Navigator (VPN) must have served in the United States Armed Forces (Active, Reserve, or National Guard service) or be an immediate family member of a Veteran/Service Member. They will provide day-to-day support to veteran households, and assist with outreach, engagement and service delivery to veteran applicants and participants across the five boroughs of NYC. They play an active role in the Veteran program participant’s case as they pursue housing stability. The VPN will function as a role model and peer, using their lived expertise to aid the Veteran household as they navigate through SSVF services and community-based supports. The VPN advocates for participants and supports their unique needs and desired service outcomes. The Veteran Peer Navigator is expected to be highly mobile to engage, meet with, navigate and support participants and applicants through all stages of the SSVF program. They work in-tandem with the other members of the S:US SSVF team to deliver core supportive services within the program and to empower veterans as they work towards housing stability.   - ESSENTIAL DUTIES & RESPONSIBILITIES: - Assist participants in the SSVF program to define, plan and achieve their goals related to SSVF such as homeless prevention/rapid rehousing, income and benefits maximization and other goals for their housing stability. - Provide resources and referral services to community members and possible participants, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and legal services. - Coordinate referrals for internal and external housing, health navigation, employment and core services for the full veteran family. - Assist program participants with accessing needed community services (housing, employment, medical, behavioral healthcare, vocational services, food, clothing, etc.) through the development of a referral network, program-initiated referrals and follow up.  - Maintain a strong knowledge of VA and community resources and support participants in navigating through these services. - Encourage participants’ professional, educational and personal growth when appropriate. - Act as an advocate for Veteran families during their course of services with SSVF. - Attend community outreach activities in order to raise awareness of SSVF services to veterans across our service region and to educate and recruit new program participants. - Accompany Veterans to various appointments at: DSS, VA, SSA, court, shelter, GPD, etc. - Document all engagements with participants and applicants according to VA and S:US standards, including the writing of case notes and contact logs. - Maintain the highest levels of confidentiality and professional boundaries according to S:US and funder standards - Attend and actively engage in all team meetings, supervisions and trainings. - Respond to veteran and program needs promptly, accurately and with courtesy and respect. - Support team members in the delivery of daily services, including providing positive modeling, peer counseling, support and encouragement to participants. - Screen and assess applicants applying for S:US Veterans Services. - Assist with the development of Housing Stability Plans and follow-up re-certifications with participants. - Act as advocate for participants as appropriate; encourage and support self-advocacy. - Participate in SUS Veterans Service Fair(s) and other events. - Willingness to travel to outreach events and to engage Veterans in the field up to 80% of the time. - Willingness and ability to attend occasional events outside of normal business hours. - Represent the agency and programs and in a professional manner to funders, participants of the program, potential customers, referring agencies, network members, and other stakeholders. - Deliver high-quality customer service. - Perform other duties related to the program as defined by the Program Director and management team.

I. SCOPE OF ROLE: The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require limited on call coverage.   II. SCOPE OF ROLE:  The Rapid Re-Housing Support Specialist plays a key direct service role in the S:US Supportive Services for Veteran Families (SSVF) program. They support veteran families’ housing goals and their access to core supportive services, including housing planning, income and benefits maximization, legal aid, and deliver support with permanent housing applications, temporary financial assistance and specialized services. The RRH Specialist supports veteran families’ housing search, placement and stabilization in their apartments. The RRH Specialist will provide day-to-day support to veteran households, and assist with outreach, engagement and service delivery to veteran participants across the five boroughs of NYC and Long Island. The RRH Specialist will work directly with stakeholders and veterans to enroll and support access to SSVF services and Temporary Financial Assistance. They manage the program’s service delivery to veteran households referred directly by local VA Medical Centers’ HUD VASH programs.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Assist participants in the SSVF program to define, plan and achieve their goals related to SSVF such as homeless prevention/rapid rehousing, income and benefits maximization and other goals for their housing stability. - Meet with designated VA staff to review cases and enroll veterans referred by HUD VASH for SSVF Temporary Financial Assistance. - Collect, review and process all required paperwork to screen, enroll and serve Veterans with HUD VASH in the SSVF program. - Act as a close liaison between S:US and the local VA teams. - Process SSVF Temporary Financial Assistance and other services according to agency and VA standards. - Monitor and report on SSVF data as directed by program management. - Maintain a caseload of specialized SSVF cases, mostly comprised of HUD VASH referrals for TFA into the program. - Assist Veteran households with accessing needed community services, including but not limited to public benefits such as SNAP, WIC and TANF, clothing, legal & child care services, educational and training resources, transportation assistance, and other supports through the development of a referral network and close follow-up. - Complete the SOAR program training and certification, and use that system to process households’ SSA applications. - Seek out, foster and interface with representatives from referring agencies in order to facilitate the delivery of program services, tracking referrals as needed. - Develop, assess and monitor individualized housing stability plans in conjunction with SSVF staff. - Track program milestones, including HSP completion dates, re-certifications and retention data as directed by program management. He/she/they will monitor and ensure active adherence to the all plans for each person supported with intellectual/developmental disabilities.    The Shift Supervisor will ensure proper supports are in place and necessary resources are made available to Direct Support Professionals to ensure proper adherence to person supported scheduled of activities.  The Shift Supervisor serves as a role model to staff and people supported and will help to develop best practices to support both the person supported and Direct Support Professional.  This opportunity is available at our program located on East 34th St in Brooklyn. The salary is $18 per hour.     ESSENTIAL DUTIES & RESPONSIBILITIES:   Person Centered Planning - Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. - Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. - Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. - Ensures a positive climate that promotes respect and dignity for people supported. - Provides opportunities for people support to personalize their environment, schedule and experiences in program. - Supports the person’s independence, choice and outcomes. - Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. - Additional responsibilities as deemed necessary and appropriate by the Vice President of Day Services or designee. - Supports the mission, vision, values of S:US.   Regulatory - Provide habilitation services to each client as prescribed in their service plans (CFA, Individual Service plan, Habilitation plan). - Responsible for overall functioning of the facility during the absence of the Program Director. - In the absence of the Program Director, supervises and instructs all Direct Support Professionals on shift.         - Responsible for people supported and agency funds and expenditures during shift (petty cash, recreation etc.). - Ensures coverage of a shift when a Direct Support Professional is absent. ·       Acts as a functioning member of the staff, meeting the needs of the people supported·       Responsible for the physical condition of the program including agency vehicles; ensuring that they are maintained in a safe and sanitary condition. - Participates in staff meetings, supervisors’ workshops, interdisciplinary team (IDT) and committee meetings. - Channels approved information pertaining to staff and agency to Program Specialists. - Provides supervisory counseling and discipline to Direct Support Professionals as necessary - Identifies facility needs and makes the relevant staff aware. - Ensure optimal utilization of the resources available in order to achieve efficiencies.    Quality Improvement - Carries out policies and procedures of SUS, in accordance with OPWDD and other funding sources. - Reports all incidents, ensuring consistent adherence to OPWDD and agency policies - Assesses and monitors staff productivity on an ongoing basis. - Responds appropriately to community, family and agency contacts. - Maintains, develops and supports the person support to staff assignment - Meets with Program Director on a regular basis to discuss pertinent shift and Direct Support Professional’s issues and successes. - Monitors Direct Support Professional’s time sheets on an as-needed basis. - Ensure fire drill completion on shift.

The Residential Aide will work under the supervision of program leadership providing shift coverage, with flexible, rotating shift schedule, and weekend coverage.   - ESSENTIAL DUTIES & RESPONSIBILITIES: - Provide direct crisis intervention to clients and prepares incident reports of such interventions - Monitor client when they are taking their medication if necessary - Inform staff of any prescriptions given to client that must be filled (especially 12am-8am and weekend shifts) - Brief incoming shifts of status of the facility (which includes all clients in and out of facility) - Give verbal and written warnings to clients and hand out appropriate consequences such as extra chores, removal of store and telephone privileges - Inform treatment staff of any concerns via an entry in communication logbook or a note in treatment staff mailbox - Make copies of forms to be utilize for store passes, cigarette distribution, meal checklists, etc. - Serve as staff escort when necessary - Partake in educational training to enhance skills related to job function - Performs various other tasks and responsibilities (i.e. facilitate morning/evening focus group - Participate in staff meetings as required - Provide coverage in the cafeteria during meal times, monitor the client bed area and remove any unnecessary items in accordance with funder rules - Coordinate and oversee morning/dispatches, petty cash (tokens, carfare, and lunch and dinner money) and escorts for clients as assigned - Some Residential Aides will provide escorts for clients as assigned and needed based on driver’s license status and eligibility. Driving eligibility is determined by SUS Driver Eligibility Policy. - Hourly house/room rounds and recording of time and findings in communications logbook on all shifts - Keep supervisor informed of all relevant information regarding the residents and the program - Other duties as requested by supervisor The Salary for this positon : The Mininmum is $16.00/HR and the Maximum is $16.00/HR

The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Peer Specialist engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding and validation with community providers and supports. The role will require on call coverage and a willingness to work flexible hours.
