SCOPE OF ROLE:            The Housing Specialist is responsible for Housing duties related to all residents’ needs.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Interviews clients to assess their present housing needs and status and submits appropriate applications to community resources. - Conducts groups to discuss available housing options, how to secure an apartment and how to handle issues related to housing. - Accompanies residents to housing interviews, housing fairs, and to complete housing applications when necessary. - Provides housing resources (i.e. phone numbers to landlords, housing programs, etc) to clients and acts as a liaison between landlords and clients. - Refers residents to get PPD and physicals completed within two weeks of their entrance into the facility. - Obtains an accurate history of client’s housing situation to assess what services are needed. - Facilitates Tier II interviews and acts as a liaison between Tier II shelter and AEGIS clients. - Submits housing applications for all housing programs available (i.e. Section 8, and NYCHA). - Handles requests for extensions to be submitted to HRA - Refers residents to Public Assistance and monitor their compliance with HRA and the ADVENT program. - Negotiates, assesses and assists client needs with respect to benefits and follows the resident’s case from entry to discharge - Assists and escorts residents in going to HRA/social service agency appointments when necessary. - Participates and attends staff, service plan review and clinical team meetings. - Maintains and ensures completion of accurate records and progress notes in client chart. - Other duties as assigned

The Health Navigator will employ the Critical Time Intervention model to assist individuals/families to access the supports they need and develop long-term helping relationships. Utilize a navigation/chronic care model to advocate for, communicate with, and identify resources for the client as well as encourage and support clients to gain and maintain independence in their healthcare to ensure improvement of health outcomes and quality of life.Provide individual and group counseling and support services to program participants. He/she will work with Social Service team to arrange medical appointments, confirm appointments, and all other activities required to improve client compliance. The Navgator will work with patients with complex medical illness requiring frequent appointments in clinic and to specialists, including confirming appointments, assisting with transportation, and obtaining records from outside medical providers.

The Employment Specialist will conduct assessment interviews to gather information relative to client’s skills, education and interests. He /she will Design and facilitate vocational/employment group and maintain an active employment vocational/educational data bank and serve as Program Representative at Workforce and Employment Initiatives and Meetings.  The specialist will identify barriers to employment e.g.-ESL, Substance Abuse , Mental Health and in collaboration with Case Manager make appropriate referrals. Provide career coaching and assist with resume preparation, job search and filing applications.

The Community Coordinator will work as a member of a team to improve relationships between the Shelter program and the neighboring community including residents, businesses and Community Police Officers. Community Coordinators will focus on safety, security and quality of life issues. The Community Coordinator will assist in problem solving, communication and relationship building. Community Coordinators will identify resources in the community for Shelter residents and will develop collaborative community projects for both Shelter Residents and neighbors to participate in.

The Child Care Specialist for the shelter will be responsible for collaborating with staff and implementing lesson plans and curriculum which are developmentally diverse for children ranging in age from 3 months old – 13 years old.  The Specialist will assist with homework and prepare after school activities, ensure that the childcare area is organized, maintained and safe, facilitate groups and workshops, including a parenting workshop. Will communicate and report all problems, concerns and incidents to Program Director, interact and closely supervise children at all times, participate in parenting workshops as appropriate for the population needs. Residential Aide duties will also be preformed.

Under the supervision of the Trauma Informed Care Supervisor, the Childcare Specialist is responsible for the day to day operation of the child care program. The program is oriented to developmentally sound teaching, non-violent conflict resolution and parent and child bonding.   Job Functions:   - Provide childcare when residents attend in house appointments and groups. - Develop a classroom curriculum on a monthly basis. - Implement the curriculum on a daily basis. - In the absence of the Childcare Specialist, contact Department of Education Liaison to place children in area schools. - Maintain classroom and infant room supplies, furniture, and equipment. - Attends staff meeting and training sessions. - Maintains safety for children at all times. - Accompany children and parents to events when necessary - Accompany children and parents to appointments when necessary - Accompany children to and from area schools when necessary - During familial emergencies drop children off outside catchment area to allow emergency caretakers to retrieve them. - Intervenes to prevent child abuse and follows mandated reporter procedures - Assist in linkage with community organizations for recreational and cultural outings - Assist children in completing homework on a daily - Alert Trauma Informed Care Supervisor of any child that has ongoing behavior problems or suspected learning problems

The Case Manager assesses clients’ needs, plans a course of action, links client to services and monitors progress. The Housing Specialist will;   - Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for consumers. - Assess consumer readiness for, as well as, the barriers to obtaining affordable permanent housing. - Provide supportive social services including individual and group counseling, workshops, education, referrals, and escorting in an effort to prepare consumers for successfully securing and maintaining permanent and affordable housing.   - Provides workshops on ADL skills, money management, utilities and rent payment, and housing interviewing skills. - Assist consumers with identifying and securing permanent affordable housing. Assist consumers with obtaining HASA rent checks, paying broker’s fees, obtaining start up kits, furniture allowances, movers, and moving into new apartment units. - Prepare and assist consumers with the housing application/interview process, including the HRA 2010e application process, the HASA housing application process, and any other specialized housing applications that may become relevant in the future.   - A general working knowledge of the HRA-HASA benefits and entitlement system.  Knowledge of all HASA rental subsidy guidelines including; the 30% rent cap, exceptions to policy, housing and habitability standards, FMR guidelines, and broker fees.   - A basic understanding of and the ability to educate consumers around supportive housing vs. independent housing, and congregate vs. scattered site.   - Provide monthly written reports highlighting consumer length of stay and status, housing placements, lost housing, barriers to housing, and pending housing.   - Must maintain a small caseload and perform the full duties of a Service Coordinator (see SC job description for further details). -   Provide accurate, complete, and timely documentation of assessment information, service planning, the provision of supportive social services, and consumer progression/regression toward service plan goals.   - Provide incident reports and all incident related documentation for members of the caseload as needed. -   Adhere to monthly housing placement goals.
