SCOPE OF ROLE: The Psychotherapist provides care coordination services to individuals who are in early stages of recovery and connecting residents with outpatient care, entitlements, and other community supports.  The psychotherapist will work with Ujima staff and residents in order to improve resident health and housing outcomes by providing and connecting at risk individuals to outpatient clinical services.     ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct and/or review needs assessments for residents in terms of various community supports, such as mental health services (including trauma assessments and Modified Mini Screens (MMS) - Refer residents to any services they need, including mental health and medical care, legal aid, food security, and child support - Inform residents of outpatient care, entitlements, and other community supports that promote integrated physical and mental health, safety, well-being and recovery - Help residents execute goals and treatment plan with Housing Specialist and Vocational Case Managers - Ensure that residents are maintaining support services - Forge and maintain professional relationships with outside service providers to build a wide referral network - Provide individualized and group counseling on-site for residents as needed - Provide crisis intervention services - Support treatment discharge and transition planning. - Encourage residents to develop problem-solving skills through clarification and explanation of options - Participate in treatment review meetings, and attend staff meetings and trainings to enhance skills that are related to job function - Documentation of treatment plan, progress notes and reporting of services delivered in EHR. - Maintain residents’ files in accordance to OASAS regulations - Additional responsibilities as needed to support program operations.

SCOPE OF ROLE: The Peer Specialist in Connections to Care provides first-hand knowledge and insight into care services to support residents in early stages of recovery.  The Peer Specialist promotes problem-solving and decision-making skill development, particularly in the areas of SUD treatment.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Build relationships with residents and their networks (family, spouses, etc.) to support residents’ recovery. - Make connections with professional supports, peer supports, and community resources to help build a care network that residents can be linked to and sharing personal experiences. - Assist residents with accessing and navigating service systems, including behavioral and medical health, criminal-legal, entitlements, and transportation. - Provide peer services on site to people living in the program that are at risk of harm from drugs and alcohol. - Promote problem-solving and decision-making skill development, particularly in the areas of SUD treatment. - Accompany residents on appointments as appropriate and act as advocate/ support self-advocacy - Identify any problems in daily living skills; provide hands-on assistance and/or referral in addressing problems in daily living skills. - Develop and facilitate a peer support group for residents. - Assist with making referrals to harm reduction, substance use, primary care, mental health care, pain management or other as needed. - Provide coordination of treatment and transitional planning as needed. - Follow up with clients who discharge from program. - Attend staff meetings and trainings to enhance skills that are related to job function. - Documentation and reporting of services delivered (in AWARDS).

The Maintenance Worker union ensures the proper maintenance and upkeep of the facility. Assists in developing consumer’s ability to maintain individual residents on basic cleaning and safety.    - Maintain cleanliness in assigned areas within the facility.  This includes but is not limited to: floor care (mopping, sweeping, buffing, stripping and waxing), cleaning bathrooms, emptying trash, separating recyclables, emptying garbage, wiping windows and walls, and cleaning stairways. - Carry out minor repairs and building maintenance throughout assigned areas of the facility. - Responsible for exterior snow and ice removal as well as upkeep of the grounds. This includes but is not limited to sidewalk maintenance, maintaining landscaping, and all other aspects of the building exterior. - Assist Staff with special events by setting the chairs and tables up for use making sure the area is clean and safe for all - Maintain building maintenance tools and machinery to ensure that all items are in good working condition. - If Program does not have a superintendent, Responsible for ordering cleaning supplies, paper supplies, lightbulbs, garbage bags, etc.  and maintaining biweekly inventory. - Assist in moving and storing program supplies, office furniture, office equipment, dorm furniture, consumer belongings, and other items in and around the facility when and where appropriate. - Comply with all SUS Employee Policies, Procedures and Protocols SUS Cooperate Compliance Policies - Willing to assist when EMERGENCIES arise in the Facility - Must wear SUS maintenance uniform and must maintain uniform so that it is clean and presentable at the start of each shift. Scope of Role The Harm Reduction Specialist provides engagement and support services, particularly to program participants around drug and alcohol use, sexual health and self-harm, using a harm reduction model. The emphasis is on meeting individuals “where they are at” and providing non-judgmental, non-coercive services and resources to individuals engaging in risky behaviors. In addition, the Harm Reduction Specialist provides consultation, training, and support to program staff in their work with program participants engaging in risky behaviors. The intention is that the Harm Reduction Specialist will be a leader in promoting Community Access’ commitment to providing services using a harm reduction model.   Core Principles The job responsibilities of all staff extend to understanding and incorporating certain principles into their work and into their relationships with program participants. These principles are: - Program participants’ right to self-determination; - Respectful communication; - Services that support recovery and healing consistent with and nurturing each participant’s cultural background, experience, identity, and values. - Clear professional boundaries to support the limits and possibilities of services.   Essential Job Functions   • Using the model of harm reduction, assess tenants' needs for services, including outreach, counseling, education, referral, etc., at program site and plan services accordingly. • Develop and support strategies to meet the needs of individuals in need of support, including individuals using drugs/alcohol, people living with HIV/AIDS, individuals involved in sex work/survival sex, people with diabetes, individuals at risk for violence, etc. • Attend and participate in Harm Reduction Committee meetings, as scheduled • Contribute to agency-wide projects of the Harm Reduction Committee, to support the infusion of harm reduction throughout the agency • Provide training and education on the harm reduction model, including HIV prevention strategies, for staff and program participants. • Provide service coordination to assigned program participants. • Provide recovery-oriented support services and outreach to assigned program participants, as defined by program and contractual requirements. • Develop and assist in implementing person-centered service plans determined by goals of program participants, including reviewing service plans with program participants, identifying progress and revising plan as needed. • Produce and maintain thorough, accurate and timely documentation, including charts and documentation in service records of interactions with program participants, services provided, important information and/or events, and contacts with other agencies and service providers, as required by agency policies, and relevant contracts and regulatory agencies. • Assist program participants by advocating for quality care from external service providers, including educating providers on special issues, etc. • Assist program participants in maintaining apartment free from unsanitary conditions and safety hazards, utilizing individualized strategies, e.g. teaching, feedback, demonstration and other forms of hands-on interventions, as needed. • Maintain a resources list and provide referrals for treatment for psychiatric disabilities, substance use, and health problems, as directed by program participants. • Assist program participants in identifying vocational and educational goals and opportunities, including assessment, support and follow-up, in coordination with Education and Employment Coordinator. • Assist program participants with conflict resolution, as necessary. • Provide outreach to program participants who are not indicating their goals or who are not engaged, to offer services to them, and to incorporate outreach in their service plans. • Provide crisis intervention, as necessary. • Provide support and training on skills-building, including assertiveness, self advocacy, socialization, and other related issues. • Maintain quality service by establishing and reinforcing program/agency standards in all interactions with program participants. • Create or maintain a special project each year with an emphasis on harm reduction. • Coordinate work orders with operations staff regarding apartment repairs and/or program participant requests. • Attend and participate in supervision, meetings and training sessions, as required and as scheduled. • Provide holiday, evening and weekend coverage, as necessary. • Provide coverage for non-assigned program participants, as needed. • Perform other duties as assigned.   The Salary for this role is Min $55,000 - Max $60,000

The Veteran Peer Navigator (VPN) must have served in the United States Armed Forces (Active, Reserve, or National Guard service) or be an immediate family member of a Veteran/Service Member. They will provide day-to-day support to veteran households, and assist with outreach, engagement and service delivery to veteran applicants and participants across the five boroughs of NYC. They play an active role in the Veteran program participant’s case as they pursue housing stability. The VPN will function as a role model and peer, using their lived expertise to aid the Veteran household as they navigate through SSVF services and community-based supports. The VPN advocates for participants and supports their unique needs and desired service outcomes. The Veteran Peer Navigator is expected to be highly mobile to engage, meet with, navigate and support participants and applicants through all stages of the SSVF program. They work in-tandem with the other members of the S:US SSVF team to deliver core supportive services within the program and to empower veterans as they work towards housing stability.   - ESSENTIAL DUTIES & RESPONSIBILITIES: - Assist participants in the SSVF program to define, plan and achieve their goals related to SSVF such as homeless prevention/rapid rehousing, income and benefits maximization and other goals for their housing stability. - Provide resources and referral services to community members and possible participants, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and legal services. - Coordinate referrals for internal and external housing, health navigation, employment and core services for the full veteran family. - Assist program participants with accessing needed community services (housing, employment, medical, behavioral healthcare, vocational services, food, clothing, etc.) through the development of a referral network, program-initiated referrals and follow up.  - Maintain a strong knowledge of VA and community resources and support participants in navigating through these services. - Encourage participants’ professional, educational and personal growth when appropriate. - Act as an advocate for Veteran families during their course of services with SSVF. - Attend community outreach activities in order to raise awareness of SSVF services to veterans across our service region and to educate and recruit new program participants. - Accompany Veterans to various appointments at: DSS, VA, SSA, court, shelter, GPD, etc. - Document all engagements with participants and applicants according to VA and S:US standards, including the writing of case notes and contact logs. - Maintain the highest levels of confidentiality and professional boundaries according to S:US and funder standards - Attend and actively engage in all team meetings, supervisions and trainings. - Respond to veteran and program needs promptly, accurately and with courtesy and respect. - Support team members in the delivery of daily services, including providing positive modeling, peer counseling, support and encouragement to participants. - Screen and assess applicants applying for S:US Veterans Services. - Assist with the development of Housing Stability Plans and follow-up re-certifications with participants. - Act as advocate for participants as appropriate; encourage and support self-advocacy. - Participate in SUS Veterans Service Fair(s) and other events. - Willingness to travel to outreach events and to engage Veterans in the field up to 80% of the time. - Willingness and ability to attend occasional events outside of normal business hours. - Represent the agency and programs and in a professional manner to funders, participants of the program, potential customers, referring agencies, network members, and other stakeholders. - Deliver high-quality customer service. - Perform other duties related to the program as defined by the Program Director and management team.

The Shift Supervisor will ensure proper supports are in place and necessary resources are made available to Direct Support Professionals to ensure proper adherence to person supported scheduled of activities.   The Shift Supervisor serves as a role model to staff and people supported and will help to develop best practices to support both the person supported and Direct Support Professional.  This opportunity is available at our program located on PineGrove Street  in Queens,NY .   ESSENTIAL DUTIES & RESPONSIBILITIES: Person Centered Planning • Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. • Promotes the independence and self-determination of individuals, utilizing person-centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs. • Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. • Ensures a positive climate that promotes respect and dignity for people supported. • Provides opportunities for people support to personalize their environment, schedule and experiences in program. • Supports the person’s independence, choice and outcomes.   Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. Regulatory • Provide habilitation services to each client as prescribed in their service plans (CFA, Individual Service plan, Habilitation plan). • Responsible for overall functioning of the facility during the absence of the Program Director. • In the absence of the Program Director, supervises and instructs all Direct Support Professionals on shift. • Responsible for people supported and agency funds and expenditures during shift (petty cash, recreation etc.). • Ensures coverage of a shift when a Direct Support Professional is absent. • Acts as a functioning member of the staff, meeting the needs of the people supported • Responsible for the physical condition of the program including agency vehicles; ensuring that they are maintained in a safe and sanitary condition. • Participates in staff meetings, supervisors’ workshops, interdisciplinary team (IDT) and committee meetings. • Channels approved information pertaining to staff and agency to Program Specialists. • Provides supervisory counseling and discipline to Direct Support Professionals as necessary • Identifies facility needs and makes the relevant staff aware. • Ensure optimal utilization of the resources available in order to achieve efficiencies. Quality Improvement • Carries out policies and procedures of SUS, in accordance with OPWDD and other funding sources. • Reports all incidents, ensuring consistent adherence to OPWDD and agency policies • Assesses and monitors staff productivity on an ongoing basis. • Responds appropriately to community, family and agency contacts. • Maintains, develops and supports the person support to staff assignment • Meets with Program Director on a regular basis to discuss pertinent shift and Direct Support Professional’s issues and successes. • Monitors Direct Support Professional’s time sheets on an as-needed basis. • Ensure fire drill completion on shift.

  S:US is seeking a Residential Specialist for our domestic violence shelter in Manhattan.   The Residential Specialist is responsible for providing a warm ,compassionate welcome to new shelter residents. You will be tasked  with  promoting person-centered services, focusing on supporting the independence of the people we serve.  You will manage the primary coverage in the building 24 hours a day 7 days a week, 365 days a year and at times functioning independently using your judgement to guarantee the safety of the people we serve and the building. The ideal candidate will  be a person of character with humility,integrity and can communicate skillfully with empathy and honesty.     ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct regular scheduled rounds to ensure safety and residents wellbeing. - Enter activity log information and shift notes in a timely fashion and accurate manner. - Complete necessary admissions paperwork. - Assist in escorting residents to services and activities outside of the facility, such as clinics, income maintenance centers and housing referral. - Meet with Shift Supervisor for individual and group supervision. - Planning, implementing and/or researching self-care and other social activities for residents conduct Resident Support groups - Meet with case managers on regular basis to assist in assigned tasks. - Provide initial orientation to program. - Provide administrative support in areas of office coverage, telephone and clerical work, food services. - Understand and follow emergency procedures, including fire drills. - Perform other duties as assigned by Shift Supervisor and Program Director. - Attend job related trainings and agency meetings as necessary. - Provide meal coordination.
