S:US is seeking a Residential Specialist for our women's shelter in the Manhattan.   This position is located at 205 W. 135th Street New York, NY 10030     The Residential Specialist is responsible for providing a warm ,compassionate welcome to new shelter residents. You will be tasked  with  promoting person-centered services, focusing on supporting the independence of the people we serve.  You will manage the primary coverage in the building 24 hours a day 7 days a week, 365 days a year and at times functioning independently using your judgement to guarantee the safety of the people we serve and the building. The ideal candidate will  be a person of character with humility ,integrity and can communicate skillfully with empathy and honesty.     ESSENTIAL DUTIES & RESPONSIBILITIES: - Conduct regular scheduled rounds to ensure safety and residents wellbeing. - Enter activity log information and shift notes in a timely fashion and accurate manner. - Complete necessary admissions paperwork. - Assist in escorting residents to services and activities outside of the facility, such as clinics, income maintenance centers and housing referral. - Meet with Shift Supervisor for individual and group supervision. - Planning, implementing and/or researching self-care and other social activities for residents conduct Resident Support groups - Meet with case managers on regular basis to assist in assigned tasks. - Provide initial orientation to program. - Provide administrative support in areas of office coverage, telephone and clerical work, food services. - Understand and follow emergency procedures, including fire drills. - Perform other duties as assigned by Shift Supervisor and Program Director. - Attend job related trainings and agency meetings as necessary. - Provide meal coordination

Then come join the S:US family!     S:US is seeking a Clinical Supervisor for our adult family shelter in Manhattan.   This position is located at East 31 Street, New York, NY 10016     The Clinical Supervisor will: - Provide social work interventions to remove psychosocial barriers to health and wellness. - Collaborates with members of the healthcare team, the patient/family and community liaisons to assure patients’ psychosocial and socioeconomics needs are addressed wherever possible. - Advocate for and empower patients to maintain a level of control over their health and wellness. - Provide and maintain educational materials. - Manage a specific case load of patients referred by clinic staff. - Provide close monitoring of patients through follow up contacts and coordinate services with other providers as necessary. - Provide crisis intervention; assess and coordinate protective service issues. - Document interventions in patients’ medical records. - Maintain knowledge of resources available in the area community. - Maintain current in the field by attending seminars, continuing education courses, workshops, etc. - Provide staff education. - Develop clinical programs and materials. - Interact with community agencies involved in health and social services, represent agency in meetings and events in the community. - Promote positive working relationships with Social Service teams. - Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes. - A work schedule that includes some weekend and evening hours is required. - Any other duties as assigned by the Registered Nurse, designee or as required.

I. ESSENTIAL DUTIES & RESPONSIBILITIES: • Complete medical assessments and Communicable Disease Risk Assessments within mandated time frames • Coordinate delivery and reception of medications with pharmacy • Electronic Health Record data entry and maintenance • Track all medications received and administered as well as coordinate return of unused medications to pharmacy • Observe self-administration of medications for detoxification according to established protocols • Refer for required medical services • Carry out urine screenings in accordance with program policy • Provide education and consultation to individual clients and/or groups on their health needs • Assess clients’ medical record and coordinate medical care with medical director and/or outside clinics • Assure clients’ compliance with prescribe medical regimen • Complete timely and accurate documentation on medical services rendered • Keep supervisor informed of all relevant information regarding the residents and the program • Other duties as requested by supervisor

This position is located at 246 Echo Place Bronx, New York 10457   Salary:  Minimum Salary: $24.04/hr. Maximum Salary:   $24.04/hr.     The Senior Case Manager will: - Provide supervisory oversight and training to Case Managers and Housing Specialists in the housing assessment and documentation process. Provide weekly one on one supervision to Case Managers and Housing Specialists - Provide supervision and guidance to Case Managers in the development of initial service plans and service plan reviews. - Provide supervision to Case Managers in referring consumers for in-house and community-based medical, mental health, and supportive social services. - Provide supervisory oversight and training to Case Managers in assisting consumers with the financial entitlement and medical benefit application process. - Provide supervisory oversight and training to Case Managers and Housing Specialists in group and individual counseling, workshops, and recreational activities. Schedule and assign groups and activities to Case Managers as needed. - Provide recommendations for useful and effective interventions and referrals to Case Managers and report any behavioral issues to supervisor. - Provide supervisory oversight and guidance to Case Managers and Housing Specialists in maintaining up to date, accurate, complete, and timely consumer chart documentation. - Provide administrative supervision including timesheet approval, time off approval, scheduling, as well as professional development including regular individual supervision and formal performance appraisal at least annually. - Directly provide services (including but not limited to those outlined above) to clients in the absence or inability of their assigned case manager. - Manage program census and census data: Manage the intake process; assigns intakes to Case Managers. Will also perform full intake and assessment with new consumers within 24 hours of admission as needed. - Conduct regular audits of all client charts. - Manage tracking sheets related to housing readiness and housing placement - Respond to requests for information and data from DHS. - Reconcile program data in AWARDS, CARES and hard copy charts/ tracking sheets. - Manage and oversee room/dorm visits/inspections to assess consumer personal hygiene, room cleanliness, and overall ADL skills. - Manage crisis intervention services and addresses all emergency situations as appropriate. - Responsible for Incident Management and documentation. - Facilitate/attend staff meetings, case conferences, group supervision, and trainings as needed. - Comply with and enforces all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and Office of Temporary Disabilities Assistance Rules, Regulations, and Statutes. - Maintain program resources including but not limited to: MetroCard’s, petty cash, and client savings - Provide coverage for other Senior Case Manager and/or Director of Social Services when they may be off site or otherwise unavailable. - Maintain CPR/First Aid Certification. - Any other duties as assigned by the Program Director, designee or as required.
