This position is located at 246 Echo Place Bronx, NY 10457.     The Social Worker will: - Responsible for the completion of psychosocial assessments of complex families impacted by multiple systems - Assess family needs and facilitate appropriate linkages for families to community based services - Work within a crisis driven environment with a knowledge base of substance abuse and addiction, domestic violence, child maltreatment, trauma, and mental health conditions - Provide education to other members of the team around clients’ psychosocial stressors and needs, to ensure positive outcomes for families - Attend ongoing professional development by the individual agency program, and the New York City Department of Homeless Services – Division of Family Services - Provide qualitative and quantitative feedback to Department of Homeless Services (DHS) and DHS Service Providers to inform both policy and practice - Be available to families seeking assistance at least five days a week, eight hours a day - Work from a strength based, family focused, client centered perspective and identify and fortify the strengths that each family has - Assist families with specific stressors associated with living in shelter - Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services - Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being - Serve as agency/program liaison to community agencies and/or groups - Provide clear written reports that capture key findings to include provider areas of strength, and recommendations for improvement - Document patient medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes - Complete other tasks as deemed necessary by program director - Ability to work a flexible schedule when needed which consist of weekend or evening hours - Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes

The Supervisor Maintenance will work at 8 to 12 apartment buildings and at such buildings will be supervisor of each buildings’ maintenance staff (superintendents and porters). These 8 to 12 buildings are located in the Manhattan, the Bronx and Brooklyn and are multifamily apartment rental buildings financed by Low Income Housing Tax Credits where S:US is the owner and property manager. The Maintenance Supervisor will need to work closely with a number of different staff members and teams, including the Program Director at each building, and other social service staff at the buildings, the property management team (located at 1168 Stratford Avenue, Bronx), the S:US Facilities Department and the superintendent and porters at each building.   Job Responsibilities:   - Visits each property at least weekly (and more if needed) and conducts regular inspections of each assigned property to ensure the building is kept to the quality standard of the company: - Supervisors the superintendents and porters at each building to ensure timely completion of work orders and minimize the issuance of municipal violations; - Works with S:US Facilities Department and the superintendent and porters at each building to ensure that violations from HPD, DEP, ECB, FDNY, etc. are addressed and reported to Violations Coordinator; - Works with Program Staff and super and porter to coordinate the "turnover" of vacant apartments in an expeditious manner in order to avoid the loss of any rental income, communicate with SPA Department of S:US re: unit availability and required inspections; - Coordinates with Contracts Manager and Director of Maintenance when outside maintenance or service vendors are needed, and advises Contracts Manager when vendors are not performing satisfactorily; - Ability to be on-call and respond to emergency situations; - Supervises all functions of the physical plant which include plumbing, heating, roofing, and electrical and gas systems; - Conduct supervision sessions with Supers/Porters; - Advises Director of Maintenance or VP of Facilities when outside contractors are needed for repair work beyond the capability of the building staff and coordinates access for contractors and ensures inspections are made of work; - Assists the property management team to ensure all apartment and property inspections are ready prior to and be on site for inspections; - Assists the property management team to review the building inspection reports in conjunction with supervisor, Director of Maintenance and VP of Facilities in order to understand what maintenance problems need to be resolved and improvements that need to be made; and - Represents himself/herself and the property in a professional manner with residents, visitors, and other employees. - Responsible for all Supers handing in the monthly preventive maintenance report; - Responsible for timesheets of all staff under supervision; - Responsible for all Supers submitting their work orders in the Yardi system. - Responsible for adhering to a weekly work schedule for himself/herself and all other staff at the properties; and - Works with the Regional Supervisor, trains the Maintenance Mechanics/Handymen, and Porters/Cleaners in the proper performance of their functions, consistent with company policies, procedures, and standards.

Then come join the SUS family!   Services for The UnderServed(S:US) is seeking a Senior Case Manager for our families with children shelter in the Bronx. This position is located at 246 Echo Place Bronx, New York 10457   Salary:  Minimum Salary: $24.04/hr. Maximum Salary:   $24.04/hr.     The Senior Case Manager will: - Provide supervisory oversight and training to Case Managers and Housing Specialists in the housing assessment and documentation process. Provide weekly one on one supervision to Case Managers and Housing Specialists - Provide supervision and guidance to Case Managers in the development of initial service plans and service plan reviews. - Provide supervision to Case Managers in referring consumers for in-house and community-based medical, mental health, and supportive social services. - Provide supervisory oversight and training to Case Managers in assisting consumers with the financial entitlement and medical benefit application process. - Provide supervisory oversight and training to Case Managers and Housing Specialists in group and individual counseling, workshops, and recreational activities. Schedule and assign groups and activities to Case Managers as needed. - Provide recommendations for useful and effective interventions and referrals to Case Managers and report any behavioral issues to supervisor. - Provide supervisory oversight and guidance to Case Managers and Housing Specialists in maintaining up to date, accurate, complete, and timely consumer chart documentation. - Provide administrative supervision including timesheet approval, time off approval, scheduling, as well as professional development including regular individual supervision and formal performance appraisal at least annually. - Directly provide services (including but not limited to those outlined above) to clients in the absence or inability of their assigned case manager. - Manage program census and census data: Manage the intake process; assigns intakes to Case Managers. Will also perform full intake and assessment with new consumers within 24 hours of admission as needed. - Conduct regular audits of all client charts. - Manage tracking sheets related to housing readiness and housing placement - Respond to requests for information and data from DHS. - Reconcile program data in AWARDS, CARES and hard copy charts/ tracking sheets. - Manage and oversee room/dorm visits/inspections to assess consumer personal hygiene, room cleanliness, and overall ADL skills. - Manage crisis intervention services and addresses all emergency situations as appropriate. - Responsible for Incident Management and documentation. - Facilitate/attend staff meetings, case conferences, group supervision, and trainings as needed. - Comply with and enforces all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and Office of Temporary Disabilities Assistance Rules, Regulations, and Statutes. - Maintain program resources including but not limited to: MetroCard’s, petty cash, and client savings - Provide coverage for other Senior Case Manager and/or Director of Social Services when they may be off site or otherwise unavailable. - Maintain CPR/First Aid Certification. - Any other duties as assigned by the Program Director, designee or as required.
