S:US is seeking a Psychiatric Nurse Practitioner for our men's mica shelter in Brooklyn.   This position is located at 1000 Blake Avneue Brooklyn, NY 11208.     The Psychiatric Nurse Practitioner will provide direct psychiatric services for homeless individuals at three separate, general population treatment programs who are not connected to care. The PNP will document medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions and outcomes. The PNP will provide medication management including prescription refills and medication education, write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements and also provide in service staff training and consultation to the social service team. - Meets with Program Director, Assistant Director- Clinical, Program Nurse, and Social Service staff to coordinate shelter residents to receive ongoing psychiatric treatment in the community. - Meet with all new residents within the first 30 days for psychiatric evaluation. - Provide referrals for transitional Psychiatric Treatment to all Residents not currently in care. - Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements.
