SCOPE OF ROLE: The ENGAGE Clinical Coordinator will lead a team of “can-do” people who continually challenge the status quo, merging cutting-edge innovation with groundbreaking research to decrease disparities in mental health and substance use problems by utilizing technology and training to provide high quality and accessible mental health assessments, immediate care, and referrals, as well as promote financial wellness among vulnerable populations. The Clinical Coordinator will be based at an integrated OASAS 822 and Article 31 Outpatient Mental Health Clinic serving adults living with substance use disorders and mental illness; including clinical program development, supervision, training and evaluation of professional staff.   Salary $80,000 - $87,000 Location: East Harlem   ESSENTIAL DUTIES & RESPONSIBILITIES: • Assists in establishing effective and efficient operations in accordance with ENGAGE regulations. • Lead in the clinical coordination of ENGAGE Community Mental Wellness Workers (CMWW). • Work collaboratively with the Project Coordinator on establishing workflows, developing initiatives, and mental health curriculums for staff. • Conducts clinical supervision of CMMW Outreach Specialists. • Directly supervises data reporting from the Team Leaders utilizing the Columbia Tool. • Work with the Columbia ENGAGE Team on Social Determinants of Health component of research that is relevant to served populations. • Issue clinical recommendations to Program Coordinator to ensure proper compliance of program guidelines. • Build relationships with S:US programs to develop inter-agency cross-collaboration. • Pending IPC Supervisory certification, provide counsel and expert advice to the Director and other staff for quality and performance improvement. • Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by S:US' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with S:US policies, OMH and OASAS regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services. • In coordination with relevant members of the SUS management team, conducts outreach and marketing to potential referral sources internal and external to SUS; active liaison with community service providers, hospitals, and other prospective referral sources is may be required. • Coordinates care with SUS nurse practitioners, psychiatrists and administrative staff; closely coordinates with clinicians external to SUS as appropriate. • Assist in managing client crises and onsite and offsite emergencies. • Perform other tasks as assigned.

S:US Is seeking a Director of Social Services for their adult families' shelter in Manhattan.   This position is located at 12 East 31 Street New York, NY 10016.     The Director of Social Services will manage and monitor daily program operations of a mental health shelter based on city, state, and federal regulatory requirements and DHS contractual requirements.   ESSENTIAL DUTIES & RESPONSIBILITIES: - Plans, schedules, organizes, directs, and oversees the provision of all clinical services. - Provides supervisory oversight and training to the Senior Service Coordinators, Entitlements Specialist, Housing Specialist and Program Nurse. - Ensures the collaboration between the Senior Service Coordinators, Entitlements Specialist, Housing Specialist and Program Nurse around coordination of services/case management, barriers to services, and community-based referrals for job skills training, education, and employment as well as progression/regression of consumer behavior, mental health, chemical dependency, and any other issues that may present barriers to housing, employment, and job skills training.  - Meets weekly with the Program Nurse and Psychiatrist to coordinate the psychiatric and medical care of Shelter Residents. - Reviews all admissions and intake screenings. - Monitors daily census reports. - Develops groups, workshops, and recreational activities that will foster consumer self-sufficiency and self-determination, recovery, relapse prevention, daily living skills, socialization skills, and overall wellness.    - Monitors and oversees the assignment of caseloads, group and activity facilitation, housing and job placements, job skills training placements, and referrals for community based social services. - Monitors and oversees the results of room/dorm visits/inspections to assess consumer personal hygiene, room cleanliness, and overall ADL skills. - Responsible for Incident Management, crisis intervention, and all related documentation. - Assists in overseeing and managing building evacuation during fire drills and building emergencies. - Monitors program effectiveness, compliance and consumer outcomes, including but not limited to, productivity standards, reporting to management and funding source/s. - Monitors the establishment and maintenance of linkages with community-based job skills training and education programs in order to build a referral base for consumers to secure job skills training and education. - Monitors the establishment and maintenance of linkages with mental health, primary health care, and substance abuse service providers. - Monitors the establishment of linkages and maintenance of solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for consumers. - Attends staff meetings, case conferences, trainings, and management meetings when needed. - Acts as primary liaison for Care Coordination team and other departments within facility. - Monitors documentation for quality and timely completion; reviews and monitors the development of all Individual Living Plans and ensures they are updated every 30 days. - Develop plans for follow up with program graduates to monitor status for up to one year with documentation of same. - Provides direct services as needed and appropriate. - Complies with and enforces all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, NYC - DHS and NYS Dept. of Social Services Rules, Regulations, and Statutes.  - Performs other duties as assigned.

SCOPE OF ROLE: Provide quality cleanliness oversight to assigned building(s). Screens and assesses applicants applying for SUS Veterans Services and navigates applicants for program services through the intake process, documentation standards and necessary supports.     - Delivers outreach and supportive services to veteran families - Provides engagement and ongoing support - Assists team members in the delivery of daily services, including providing positive modeling, peer counseling, support and encouragement to participants. - Provide outreach through the distribution of materials, informational/educational presentations and marketing to community members and possible participants. - Address crowds of varying sizes and offer program presentations. - Provide resource and referral services to community members and possible participants, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and housing resources. - Develop, assess and monitor strategic outreach plan and resources in conjunction with SSVF staff. - Coordinate outreach in partnership with other SSVF and CoC programs. - Interface with representatives from referring agencies and partnering service providers in order to facilitate the delivery of program services and a steady flow of referrals. - Assist program participants with accessing needed community services (housing, employment, medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up. - Escort and engage Veterans in the community.   - Act as advocate for participants as appropriate; encourage and support self-advocacy. - Maintain a strong working relationship with colleagues and program managers. - Document all services in compliance with agency standards. - Work as a member of the social service team in the development of on-site and off-site socialization, educational, risk reduction and skill building activities with a view towards permanent housing stability. - Liaison to community based services and groups including civic, social service, government, businesses, landlords and non-profit organizations. - Maintain a small caseload of SSVF households, navigating them through the SSVF service cycle and housing planning process. - Participate in ongoing individual supervision, team meetings, case conferences, division meetings and trainings. - Respond to information requests from internal SUS staff. - Participate in SUS Veterans Service Fair(s) and other company events. - Other duties as assigned by management. - Veteran of the U.S. Armed Forces. - Good verbal and written communication skills needed. - Familiarity with MS Office. - Must have an ability to work with a team. - Willingness and ability to travel across various locations regularly a must - Fieldwork expected at least 50% of the workweek.
