I.   The Assistant Program Director (APD) is responsible for ensuring that our individuals have the supports and resources necessary to live meaningful lives, and that they align with their needs and aspirations within both their respective homes and communities. The APD is responsible for ensuring that the individuals’ wellness, nutritional, and healthcare needs are met. He/she will lead their assigned shift, providing hands-on coaching and role modeling best practices and behaviors for Direct Care Professionals (DSPs) as well as providing 24-hour assistance in periods of high demand.   In collaboration with a trans-disciplinary team member and under supervision of the Program Director, the APD is expected to promote an environment of respect, learning and independence for the individuals.  He/she will interact with individuals and colleagues in a manner that stems from dignity and with hopes of fostering a true home atmosphere for the individuals.  In addition, he/she will provide coaching to DSPs and Shift Supervisors so that their actions ensure the health and wellness of the individuals, support their independence, self-determination and right to take reasonable risks.  The APD will maintain operational efficiency of the residence, including fiscal, regulatory, policies & procedures, staff management, and quality improvement.  This position is located at our program in The Bronx on Graff Street and has a minimum and maximum salary of  $52,000.00. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. Outreach Specialist    As part of the SSVF team, this position serves as the primary initial S:US contact point for Veterans and their families.  This position delivers outreach and supportive services to veteran families by providing engagement and ongoing support.  Assists team members in the delivery of daily services, including providing positive modeling, peer counseling, support and encouragement to participants. Screens and assesses applicants applying for SUS Veterans Services and navigates applicants for program services through the intake process, documentation standards and necessary supports.     ESSENTIAL DUTIES & RESPONSIBILITIES: - Provide outreach through the distribution of materials, informational/educational presentations and marketing to community members and possible participants. - Address crowds of varying sizes and offer program presentations. - Provide resource and referral services to community members and possible participants, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and housing resources. - Develop, assess and monitor strategic outreach plan and resources in conjunction with SSVF staff and other SSVF programs. - Interface with representatives from referring agencies and partnering service providers in order to facilitate the delivery of program services and a steady flow of referrals. - Assist program participants with accessing needed community services (housing, employment, medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up.  - Maintain a small caseload of SSVF participants in accordance with SSVF standards. - Assist with the development of Housing Stability Plans and follow-up re-certifications with participants. - Act as advocate for participants as appropriate; encourage and support self-advocacy. - Maintain a strong working relationship with colleagues and program managers. - Document all services in compliance with agency standards. - Work as a member of the social service team in the development of on-site and off-site socialization, educational, risk reduction and skill building activities with a view towards permanent housing stability. - Liaison to community-based services and groups including civic, social service, government, businesses, landlords and non-profit organizations. - Participate in ongoing individual supervision, team meetings, case conferences, division meetings and trainings. - Participate in SUS Veterans Service Fair(s) and other events. - Completion of other tasks as assigned by manager.

This position is located at 882 Dumont Avenue Brooklyn, NY 11207.   The Social Worker will: - Responsible for the completion of psychosocial assessments of complex families impacted by multiple systems - Assess family needs and facilitate appropriate linkages for families to community based services - Work within a crisis driven environment with a knowledge base of substance abuse and addiction, domestic violence, child maltreatment, trauma, and mental health conditions - Provide education to other members of the team around clients’ psychosocial stressors and needs, to ensure positive outcomes for families - Attend ongoing professional development by the individual agency program, and the New York City Department of Homeless Services – Division of Family Services - Provide qualitative and quantitative feedback to Department of Homeless Services (DHS) and DHS Service Providers to inform both policy and practice - Be available to families seeking assistance at least five days a week, eight hours a day - Work from a strength based, family focused, client centered perspective and identify and fortify the strengths that each family has - Assist families with specific stressors associated with living in shelter - Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services - Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being - Serve as agency/program liaison to community agencies and/or groups - Provide clear written reports that capture key findings to include provider areas of strength, and recommendations for improvement - Document patient medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes - Complete other tasks as deemed necessary by program director - Ability to work a flexible schedule when needed which consist of weekend or evening hours - Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes
