SCOPE OF ROLE: The Qualified Intellectual Disability Professional (QIDP) is responsible for the overall effective development, implementation and support of the person supported road map to leading the meaningful life they choose to live. The QIPD is responsible for ensuring each plan is developed and supports Person Centered Thinking and Planning.  The QIDP will help to lead teams in creating meaningful activities and valued outcomes for persons supported in the areas of community integration; socialization; recreation, volunteerism and mindful motivation. Promote, research and provide activities that further enrich people’s lives, while providing options in both the program setting and community environments.  The QIDP ensures that services provided by the DSPs are properly documented and adhere to all OPWDD data collection regulations to ensure proper billing of services render.   The QIDP is to support DSPs in increasing their knowledge and ability to capture service and experiences provided via training and modeling.  The incumbent will support Program Directors during periods of high demand and will provide ongoing coaching and opportunities for learning to the DSPs   ESSENTIAL DUTIES & RESPONSIBILITIES:   Person Centered Planning - Ensures that a person-centered focus is used to provide care and support by all staff who work within the program actively seeking out different approaches. - Ensures a positive climate that promotes respect and dignity for people supported. - Supports the person’s independence, choice and outcomes. - Promotes participation in activities of meaningful daily living, such as vocation, recreation, volunteerism, engagement. - Provides prompt, efficient, and personalized support to meet requirements, requests, and concerns. - Provides opportunities for people support to personalize their environment, schedule and experiences in program.   Regulatory   - Develop Staff Action Plans (SAP) in accordance with services outlined in each person’s Life Plan by developing valued outcomes that are person-centered, meaningful and meet the wants and needs of the person served - Oversees, monitors and coordinate annual and six-month review of Staff Action Plan - Assuring that invitations are being mailed out. - Meetings are being held according to schedule - Attend all Life Plan meetings. During Life plan meetings- advocate for the person’s wants and offer support to the person so their voice is heard. Share valuable information based on observations, conversations and review of data collection to report on progress. - Ensures the follow-up of recommendations stemming from reviews/special meetings - Follow-up with Care Coordination Organizations within 2 weeks of Life Plan meetings, requesting plans - Monitors data collection on an ongoing basis - Participates in audits - Ensure Life Plan dates are accurate and review Life Plans to ensure they are accurate and finalized following the Life Plan review procedure - Ongoing training and support to new/veteran staff and management in regards to compliance and person-centered practices through observations; role modeling and problem-solving. - Provide ongoing training to staff and management on plans as well as when billing issues are identified - Ensures the implementation of SUS policies and procedures, and regulatory requirements as set forth by OPWDD - Coordinates and conducts reviews of people supported status/progress including an assessment of risks to person ’s rights and protection, as required - Additional responsibilities as assigned as needed by Director and/or Vice President, e.g. committee participation, in-service training for the agency, attendance at special meetings, etc.    Quality Improvement - Ensure Medicaid document Compliance - Assuring all individuals clinical time is accurate. - Assuring all individuals attendance is accurate. - Assuring accurate amount of billing is reflected as F/T or P/T and is supported by documentation. - Assuring all necessary signatures are present. - Assuring all dates are accurate. - Upload finalized Life Plans and corresponding documents to the Precision Care system and distribute Staff Action Plans to all parties within a timely manner - Reviews and maintains electronic records to support billing - Reviews and approves Medicaid Billing Documents - Maintains Billing Accuracy and Submission of Quality Improvement Reports - Complete all necessary billing corrections. - Submit responses to QA, Medicaid billing review. - Make all necessary corrections to billing as required by Vice President of Day Services, QA or Fiscal Department

I. SCOPE OF ROLE: ACT is an evidence-based, recovery-oriented mental health service delivery model that utilizes a multi-disciplinary team approach providing intensive outreach-oriented services to individuals with severe and persistent mental illnesses and co-occurring disorders. Utilizing a client-centered approach, the Family Specialist has the lead responsibility for integrating family goals; restoring and strengthening the individual’s unique social group and family relationships. Providing at least 80% of services in the community, the family specialist will work with a team of mental health clinicians to assist individuals in achieving their meaningful goals and independence.   II. ESSENTIAL DUTIES & RESPONSIBILITIES: Clinical Skills • Assumes lead responsibility within the multi-disciplinary team for integrating wellness goals, and services with the task of all team members and for providing wellness psycho-education individually and in groups, when clinically indicated. • Provides clinical services that are client-centered, trauma-informed, recovery-oriented, evidenced based, anti-racist, and gender-responsive. • Advocates for the rights and preferences for individuals within the ACT program. • Attends and participate in daily team meeting to discuss individual’s case management and clinical needs. • Involves family and significant others in the individual’s treatment with the team. • Assess the nature of family and social relationships as well as design interventions required to promote progress towards familial and social wellness. • Facilitate family psycho-education group/sessions to help individual’s families and/or support network learn and have better understanding of mental diagnosis and treatments to support the individual service toward recovery. • Lead the discussion as well as associated interventions around family issues and incorporate findings into service plans. • Completes family needs assessments by identifying history, current relationships and supports, and immediate needs and recommend treatment objectives for the individual’s service. • Provide family conflict resolution strategies   Data and Accountability • Provides 80 percent of treatment contacts in the community, including leading groups when clinically indicated. • Ensures all documentations are completed in a timely manner and remain in compliance with agency and OMH regulations. • Ensures that all progress notes are completed within 24-48 hours of the delivery of service. • Completes documentation including, but not limited to, initial comprehensive recovery-oriented treatment plans and 6 month review plan to show continued progress and/or barriers, 6 month comprehensive assessments, 30 day intake paperwork, and psychosocial. • Provides 24-hour crisis intervention on-call services, on a rotating basis. • Able to effectively collaborate with staff across the agency. • Coordinate services with other specialist on the ACT Team, i.e. Psychiatrist, Substance Use Specialist, Employment Specialist, Nurse. • Follows-up on any aftercare/recommendations for treatment. Escorts/ accompanies individual as needed on appointments (medical, psychiatric, substance abuse).   Compliance • Participates in team meetings, in-service trainings, and in compliance with all online required trainings. • Maintains accurate and up-to-date records, including but not limited to, progress notes, service plans/6- month reviews, assessments, and contact logs- and remain in compliance with agency, OMH and AOT regulations. • Maintains good status in regard to completion of in-service and online trainings. • Ensure all documentation is completed in a timely fashion • Communicate pertinent consumer and operational information to the Team Leader and Senior management • Assists in the planning, developing and implementation of goals and training to further develop participants skills. • Responsible for reporting and documenting all incidents to Team Leader within 24 hours.   External Relations • Maintain relationships with outpatient programs, housing programs, as well as continuing to build relationships with organizations that may be able to assist SUS consumers who need other services. • Models appropriate behavior. Represents the organization in a positive and appropriate manner to outside resources. • Attend case conferences and coordinate discharge planning with hospital. • Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader.     Salary: $55,000-60,000

Peer Specialists intentionally use language in the service of listening to understand and making space for complex personal stories of recovery and resilience. - Advocacy: Peer Specialists advocate with and for program participants both in the larger community and on the team. - Co-Creating Support and Wellness Tools: Collaborate with participants to clarify their personal visions and develop their wellness toolkit. Along with other team members, Peer Specialists support participants in strengthening their self-awareness, building life skills, and connecting to resources and community outside of the S:US team. - Influencing Team Culture: Whenever possible, positively influencing the team culture by advocating for clients, promoting a youth friendly approach and encouraging the use of recovery oriented language. S:US Peer Specialists also work actively within the team itself to help build an environment that operates from a belief in the human potential to grow and an understanding of human diversity. - Team Communication: Peer Specialists maintain open and frequent communication with the team. This includes reporting safety and other concerns to the team. Peer Specialists will also keep notes on visits with program participants as required by the provider agency. S:US Is seeking a Shift Supervisor for our Women's shelter in Manhattan.    This position is located at W. 135 Street New York, NY 10030     SCOPE OF ROLE: The Shift Supervisor provides supervision and training to residential aides and community coordinators.   ESSENTIAL DUTIES & RESPONSIBILITIES: • Supervision includes, but is not limited to, ongoing individual supervisory meetings; building inspections, ongoing feedback regarding job performance; coordinating work schedules, timesheets and requests for time off; coordinating work assignments; evaluating paperwork and documentation including the Security Log and Maintenance Work Order Log books; coordinating security and maintenance staff training, development and evaluation. S:US is seeking a Program Director for our men's shelter located in Brooklyn.   This position is located at 599 Ralph Avenue Brooklyn, NY 11233.     In this role you will:   - Manage and monitor all aspects of program start-up based on DHS contractual requirements and SUS guidelines. - Manage and monitor daily program operations of the shelter based on city, state, and federal regulatory requirements and DHS contractual requirements. - Manage and monitor ongoing expenditures, resource allocation, and utilization. - Complete written reports to division management, SUS executive management, and funding source/s as required; program progress and highlights, consumer census, housing and employment placement, staffing, and service utilization. - Provide supervision, guidance, and performance evaluations to Director of Social Services, Facility Manager and Head Teacher. - Develop administrative program guidelines and protocols as appropriate. - Assist in the development of job skills training curriculums suitable to the population served. - Develop program policy & procedures governing the provision of services to consumers and day to day program operations. - Enforce corporate compliance protocols, SUS employee policies, procedures, and protocols, and incident management policies, procedures and protocols. - Develop and maintains a crisis/incident management system. - Ensure all health and safety guidelines including staff CPR/First AID Certifications, Fire Safety Coordinator trainings, and Mandated Reporting training are all in compliance. - Maintain CPR/ First Aid and Mandated Reporter Certification. - Monitor the integrity of client data and ensure that internal chart reviews are conducted consistently and regularly. - Participate in professional development and training through agency-wide in-service trainings, outside trainings, and facilitate in-house trainings for staff. - Participate in and monitor the recruitment, hiring, and selection of direct care and supervisory personnel. - Facilitate periodic staff meetings and case conferences as appropriate.  - Monitor the provision of and documentation of all services provided to clients.  - Participate in division management meetings as needed. - On call 24 hours per day and ensure that there is a contingency plan in place. - Ensure all interactions support the mission statement and core values of SUS. - Additional duties as assigned by the Vice President, AIDS Services and Urgent Housing. - A work schedule that includes some weekend and evening hours will be required.
