  • Position Overview

    The LPN assesses, monitors, and ensures access to coordinated medical and mental health services as part of a social service treatment team known as HOPWA (Housing Opportunity Persons with AIDS)



    • Conduct home visits on a monthly basis visiting up to 25 consumers
    • Work with consumers in filling prescriptions and coordinating appointments as needed.
    • Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
    • Maintain CPR/First Aid Certification.
    • Maintain up to date, accurate, complete, and timely consumer chart documentation.
    • Attend staff meetings, case conferences, group supervision, and trainings as needed.
    • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
    • Assist full time Registered Nurse
    • Record keeping as per agency regulations
    • Related duties as assigned.


    • New York State Licensed Practical Nurse
    • Experience in direct clinical work with psychiatrically disabled populations preferred.
