  • Position Overview

    The HCBS Specialist perform the following job functions;

     

    • The HCBS specialist will provide Rehabilitation, Vocational, Habilitation and Employment services.
    • Responsible for integrating vocational goals, Employment goals and Habilitation services within the HCBS program
    • Responsible for developing HCBS treatment plans and progress notes
    • Responsible for ensuring documentation is submitted In a timely manner
    • Provides assistance through all phases of the vocational service. 
    • Performing routine clinical services as a member of the HCBS team.
    • Responsibility for the recipients and for charting services.
    • Responsible portion or all of creating the service plan.
    • Responsible for ensuring revisions to the plan as the participant’s needs change.
    • Advocating for the rights and preferences of the participant whenever possible.
    • Adhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate action.
    • Significant travel to engage clients at shelters, in their community and other settings is required; 85%.

    Qualifications

    • BA degree in psychology, social work, or related field is required.
    • Must have two or more years’ experience working with persons who are seriously persistently mentally ill or Co-Occurring with Substance use D/O.
    • Must have excellent verbal, written and interpersonal communication skills. 
    • Proficient in MS Word/Excel.  Ability to research material via the internet. 
    • Bilingual in any language a plus not required. 
    • Driver’s license validate in New York State.
    • Any additional assignments as assigned.
