For an assigned caseload of 18 to 22 clients, the Case Manager for this Men’s shelter will provide comprehensive case management services, conduct intake process with new consumers within 24 hours of admission, and conduct psychosocial, vocational/educational, and housing assessments and documentation of same. The Case Manager will assist consumers with the application process to obtain financial entitlements and medical benefits where appropriate. He/she will facilitate group and individual counseling, workshops, psych education, and recreational activities.
