  • Position Overview

    The Team Leader will;

    • Work with a small but growing number of clients who receive outpatient behavioral healthcare through SUS, and who will receive primary healthcare through Project Renewal.
    • Educate consumers on how to better manage their health and healthcare via evidence based practices focused on prevention.
    • Promote consumer engagement in outpatient behavioral healthcare as well as primary care and wellness services.
    • Complete all tracking and reporting requirements for outcomes and evaluation.
    • Assure consumers get the care they need in a culturally and linguistically appropriate manner.
    • Assist with completion of initial intake assessments.
    • Help patients with transition from inpatient medical settings and set up appropriate follow-up.
    • Serve on the Integrated Treatment Team and work closely with the Project Director/MD and other providers.
    • Conduct prevention and health promotion evidence based programs, in collaboration with the Peer Wellness Coach.
    • Assist with linkage to care coordination.
    • Ensure that the consumer’s behavioral health providers and medical providers are kept informed of changes in the consumer’s condition or treatment.
    • Assist with the arranging of transportation and follow-up services (with specialists, etc.).
    • Take vital signs and draw blood, as directed by Project Renewal’s medical nurse practitioner.
    • Approximately 10% of the work week will be devoted to supporting Project Health initiatives, also located at the Clinic site. The Nurse Care Coordinator will provide HIV testing, hepatitis testing, and hepatitis B vaccinations, as well as related health education, to clients enrolled in Project Health.
