The Licensed Practical Nurse will assist the Registered Nurse and be responsible for supporting the individuals we served by taking vital signs, such as blood pressure, temperature and weight, working with consumers in filling prescriptions and coordinating appointments as needed. The LPN will follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed. The LPN will conduct home visits on a monthly basis visiting up to 25 consumers, refer residents to emergency care, coordinate treatment and discharge planning with hospital staff. Compliance with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes is required.
