  • Position Overview

    The Case Manager will;

    • Provide direct case management services to assigned caseload
    • Develop and monitor individual service plans in conjunction with agency consumers and other related individuals as indicated.
    • Assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network.
    • Assist agency consumers with problem solving and crisis intervention.
    • Accompany agency consumers on medical, psychiatric, and entitlements appointments as necessary.
    • Act as advocate for consumer as appropriate; encourage and support self- advocacy.
    • Identification of and hands on assistance and/or referral in addressing problems in daily living skills.
    • Conduct regular apartment inspections as outlined by SUS and government funders to Support Facilities Management staff in ensuring maintenance of standards.
    • Communicate with Superintendents/Building Management to have apartment repairs rectified.
    • Monitor and follow up on work orders until completion.
    • Supportive counseling to agency consumers as needed.
    • Minimum of monthly face to face visits as well as hospital visits, as necessary.
    • Maintenance of a strong working relationship with all therapeutic support services to ensure the best quality of care.
    • Contact ICM, Parole Officers and other external providers to collaborate on on-going care.
    • Liaison to community based services and groups including civic, social service government, and business organizations.
    • Attend Criminal Court proceedings to provide advocacy as needed.
    • Documentation of all services in compliance with agency standards.
    • Ensure the appropriate reporting, filing and follow-up of incidents involving Mental Health Services clients and/or staff.
    • Participate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainings.
    • Adhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate action.

    Qualifications

    • Five years’ experience in a Human Services related field
    • Two years supervisory experience is preferred not required 
    • Direct service experience with individuals with mental illness, substance use disorder and/or a history of homelessness required
    • Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred 
    • Computer literate, Microsoft Office Suite, AWARDS. Bilingual English/Spanish preferred.
    • Valid NYS driver’s license preferred, not required.
