  • Position Overview

    The SRO Outreach Project works with homeless and formerly homeless people with HIV/ AIDS and seeks to connect them to medical services and stabilized housing; The Care Coordination Program (MCM) employs a navigation model and a chronic care model to advocate for, communicate with, and identify resources for the client as well as encourage and support clients to gain and maintain independence in their healthcare to ensure improved client health outcomes and improved quality of life.

    • Supervises Patient Navigators and provides weekly individual supervision.
    • Conducts orientation activities and enrolls the client into the program.
    • Verifies eligibility and conducts programmatic duplication checks to ensure that similar services are not already provided by another payer.
    • Performs intake assessment including initial social services, logistical and benefits assessment.
    • Facilitate the interdisciplinary conversation with the HIV team.
    • Updates comprehensive care plan with assistance from Patient Navigators.
    • Conducts monthly reviews of staff competency as well as chart-based review of staff work.
    • Meets with medical provider to ensure that MCM consumers receive appropriate services.
    • Collect and record Primary Care Status Measures data (viral load and CD4 count) as assigned.
    • Assist clients with relevant applications and other paperwork for benefits and other support services.
    • Participate in quality assurance/improvement activities.
    • Provides backup to Patient Navigator as needed.
    • Participates in required meetings and trainings.
    • Evening and weekend hours may be required.
    • Reports to the Program Director.

    Qualifications

    • Master’s Degree in social work, human services or related field.
    • Experience working with people living with AIDS, homeless people and/or substance users required.
    • Experience managing staff.
    • Experience working within an outreach model or case management preferred.
    • Bi-lingual (English-Spanish) preferred.
