    Position Summary:

    The Peer Wellness Coach will work with a small but growing number of consumers who receive outpatient behavioral healthcare services from SUS, and who will be receiving primary care and wellness services from a primary care partner as part of the Primary Behavioral Health Care Integration (PBHCI) project.

     

    Responsibilities:

    • Provide mentoring and individualized health coaching to clients to assist them to obtain their health objectives.
    • Co-facilitate support groups/classes to support improved health outcomes.
    • Complete appropriate tracking and reporting requirements for outcomes and evaluation.
    • Maintain appropriate professional standards and provide appropriate follow-up for consumers.
    • Advocate for the needs of the consumer to the Coordination and Integrated Treatment Teams and involved providers of care.
    • Engage consumers and advertise the PBHCI project by giving informal presentations at Consumer Advisory Board (CAB) meetings and other gatherings.
    • Escort and/or link consumers to transportation for appointments.

    Qualifications

    • Graduate of Howie the Harp or other Peer Specialist training program.
    • High School degree preferred.
    • Bilingual skills preferred.
