    Services for the UnderServed (SUS) is a New York-based nonprofit organization that provides housing and support services to 25,000 individuals and their families living in New York City and Long Island.  SUS believes every New Yorker has the right to lead a life of purpose.

    POSITION OVERVIEW:

    Under the supervision of the Program Supervisor, provide aftercare services to for program participants and their families. 

     

    KKEY ESSENTIAL FUNCTIONS:

    • Attend all Manhattan DHS referred NYCHA lease signings.
    • Meet with clients who may need more intense services to tap into resources to adjust to community, attend appointments (other than court) with clients who are in need of additional instructions
    • Complete home visits for Aftercare clients
    • Engage and enroll unsheltered clients as instructed by DHS, ultimate goal is to meet the DHS target of 75 successfully enrolled Aftercare clients
    • Assists families with identifying and establishing relationships with community supports
    • Provide needed services through referrals and follow up on progress.
    • Update clients’ file in CARES with all contacts made with family and other service providers within 48 hours of contact with clients
    • Maintain professional relationship with other service providers.
    • Complete additional tasks and reports as requested by supervisor.
    • The essential functions of the job include but are not limited to the job description. Tasks may be modified, expanded and/or assigned over time.

    Qualifications

    Requirements:

    • BS in related field and one year of qualifying experience OR
    • AA degree in related field and two years of qualifying experience.
    • Bilingual English/Spanish preferred
    • Position requires energy and interest for home visits, case finding and outreach.
    • Experience working with substance abuse, homelessness, families and groups.
    • Excellent written and oral communication skills.
    • Basic computer skills.
