    Position Summary:

    The Registered Nurse ensures the health and physical well-being of the consumers.

     

    JOB FUNCTIONS:                   

    • Participates as appropriate in the development, review and update of an Individual Program Plan as part of the Interdisciplinary Team process.
    • Participates in pre-admission screening and evaluation of ICF-referred Consumers
    • Develops an initial nursing assessment within 24 hours of placement at a facility
    • Develops a Nursing Service Plan based on a Nursing Assessment, Physical Exam Report, Evaluation of Consumers Ability to Self Medicate and ITT input, within 30 days of placement, and annually thereafter
    • Reviews and updates Nursing Service Plans semi-annually and as needed based on needs derived from completed nursing evaluations and quarterly nursing physical assessments
    • Ensures that nursing care, as prescribed by the physician or as identified by individual needs, is carried out
    • Participates in the development of discharge plans
    • Makes referrals to appropriate community resources based on the completed medical portion of the consumer’s Community Service Plan
    • Assesses consumer’s health condition as needed, and determines the need for referral to physician or emergency room based on observation and/or staff provided information
    • Ensures control of communicable diseases and infections, through:
      1. Identification and assessment
      2. Reporting to medical authorities
    • Implementation of appropriate protective and preventive measures
    • Supervises medication administration periodically, ensuring adherence with OMRDD regulations and agency policy
    • Report all injuries to management immediately to ensure appropriate incident reporting procedures are followed
    • Completes quarterly Quality Improvement Reports and ensure resolution to any outstanding issues
    • Reviews and follow up on Quality Improvement Reports completed by the Medical Assistant
    • Completes semi-annual and annual nursing ITT report
    • Participates in annual and semi-annual interdisciplinary team meeting along with any special meetings within or outside the residence
    • Participates in the monthly review and reordering of consumers’ medication together with the primary psychiatrist and facility psychologist
    • Ensures proper storage of all medication and medical supplies, and the proper disposal of the same (in accordance with OMRDD regulations and/or agency policy).
    • Reviews and monitors, for the purpose of health maintenance bowel movement records, vital signs, seizure activity and records, menstrual records, and tardive dyskenesia records on an ongoing basis. Reports irregularities or abnormalities to the consumer’s primary physician as needed
    • Ensures ongoing health care services are provided as needed and medical appointments are scheduled in a timely manner
    • Supervises, assists and trains a qualified program specialist to become a Nursing Assistant.
    • Supervises, assists and trains Licensed Practical Nurses to include a review and signing of all written reports for accuracy prior to being presented to ITT.
    • Ensures the proper maintenance and storage of medical records in accordance with OMRDD regulations and agency policy
    • Ensure all interactions support core values and mission of the agency
    • Carries out all policies of the agency issued by management in accordance with OMRDD and/or other funding agency guidelines
    • Additional responsibilities as deemed appropriate by the Coordinator of Medical Services, Deputy Director, DD Services or designee
    • Be available 24 hours per day by beeper and respond when contacted

    Qualifications

    • Registered Nurse licensed to practice in New York State
    • Specialized training in MR/DD and/or one year experience in community health care services
    • AMAP trainer
    • CPR and First Aid Trained
