    Position Summary: The Direct Support Professional provides individuals with Developmental Disabilitites with the assistance needed to achieve independent living within their homes and in the community.

     

    Responsibilities:

    • Provide habilitation services to each individual as prescribed in that persons Community Habilitation Plan
    • Assist individuals in meeting their valued outcomes as outlined in their Community Habilitation Plan and in their Individualized Service Plan (ISP)
    • Report immediately to Coordinator, Referral Services and Residential Habilitation all types of abuse and any other problems including safety issues or additional needs not outlined in the Community Habilitation Plan
    • Carry out all policies and procedures of the agency issued by the Coordinator, Referral Services and Residential Habilitation in accordance with OPWDD and other funding agency guidelines
    • With appropriate training and supervision, administer medication to individuals as assigned and in compliance with management, medical and nursing policies and procedures
    • Serve as a positive role model and advocate for the individual receiving services

    Qualifications

    • High School Diploma or GED required, some college preferred.
    • Minimum one year experience providing direct services to individuals with a developmental disability. 
    • Must have valid NYS Drive License.
