Position Summary:

The Service Coordinator will engage and assist chronically homeless individuals in meeting the demands of independent living and maintaining permanent housing through guidance and assistance in obtaining the necessary skills and establishing linkages to services that will support self-sufficiency and self-determination.  He/ she will conduct intake process with new consumers within 24 hours of admission.  This includes but is not limited to explanation of resident rights and responsibilities, and grievance procedures.  Will conduct psychosocial, vocational/educational, and housing assessments and documentation of same. The Coordinator will provide accurate, complete, and timely documentation of the provision of supportive social services and consumer progression/regression toward the goals of securing permanent affordable housing and prepare housing applications and assist consumers with the housing application/interview process.

  • Bachelor’s Degree in Human Services field required.
  • one year experience working with chronically homeless individuals or with co-occurring disorders. Masters degree can be substituted for experience
  • Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
  • Experience in group work, case management, referral, and/or community outreach strongly preferred.
  • Bi-lingual-Spanish and a valid NYS driver’s license preferred.
