Position Overview:

The Wellness Coach (WC) will provide direct case management services to assigned caseload as follows: develop and monitor individual service plans in conjunction with agency consumers and other related individuals as indicated. Assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network. Conduct regular apartment inspections as outlined by SUS and government funders to Support Facilities Management staff in ensuring maintenance of standards. A minimum of monthly face to face visits as well as hospital visits, as necessary. The WC will maintain a strong working relationship with all therapeutic support services to ensure the best quality of care, contact ICM, Parole Officers and other external providers to collaborate on on-going care and attend Criminal Court proceedings to provide advocacy as needed.

  • Bachelor’s Degree in Human Services or related field required
  • Direct service experience with individuals with mental illness, substance use disorder and/or a history of homelessness required
  • Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred
  • Bilingual English/Spanish preferred
