Position Overview

Services for the UnderServed (SUS) is a New York-based nonprofit organization that provides housing and support services to 25,000 individuals and their families living in New York City and Long Island.  SUS believes every New Yorker has the right to lead a life of purpose.

Position Summary: The position is responsible for administration and management of NY START Therapeutic In-Home Services.


Essential Job Functions:

  • Implements NY START In-Home Therapeutic Services and supports for children (ages 6-21) with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs.
  • Ensures NY START Therapeutic services are provided in accordance with fidelity to the START model as described in literature published by the National Center for START Services at the University of New Hampshire Institute on Disability.
  • Reviews staff service documentation for clinical appropriateness and accuracy.
  • Conducts retrospective service review to ensure services provided were in accordance with local, state and federal requirements and with fidelity to the START model.
  • Provides clinical and administrative supervision of NY START In-Home counselors/staff.
  • Coordinates with NY START Clinical Teams on triage and admission/discharge from Therapeutic In-Home Services.
  • Coordinates transition planning for individuals utilizing NY START Therapeutic In-Home services.
  • Maintains contact with families and/or providers upon admission to services to ensure satisfaction and monitor quality of service provided.
  • In collaboration with NY START Clinical teams, identifies on-going support and training/treatment needs for families and providers who utilize In-Home Service.
  • Maintains linkages and relationships with community partners.
  • Attends all required meetings with NY START Coordinators, NY START Director, and other NY START staff.
  • Identifies, coordinates and monitors necessary training for In-Home staff.
  • Ensures the collection of required data and documentation on consumer access and utilization of NY START In-Home services.
  • Works with team to compose guest discharge summaries.

Supervisory Responsibility:

  • Direct supervision of NY START In-Home Coaches staff.




Minimum Qualifications:

  • Qualified Professional
  • Master’s Degree obtained from an approved, accredited college/university in Counseling, Social Work, Psychology, or closely related field.
  • Current or eligible for NY license as an LPC, LCSW, LPA, LCAS, or LMFT preferred
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs.
  • Two or more years’ supervisory experience in a mental health/developmental disabilities setting preferred
  • Knowledge of the MH and I/DD systems of care.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
  • Within 12 months of providing services in this capacity, must be approved for certification as a START Coordinator by the University of New Hampshire, IOD Center for START Services.
  • Must possess a current, unrestricted, state-appropriate driver’s license and current personal vehicle insurance.


Essential physical requirements:

  • Must be physically able to lift a minimum of 50 pounds, bend and assist individuals with mobility needs.  
  • Must be able to hear with in normal ranges.
  • Must be able to talk in order to articulate crisis prevention, intervention and de-escalation supports.
  • Must be able to see with or without accommodation to observe individuals.
  • Must be able to drive for work purposes.
