OPWDD Experienced Licensed Registered Nurse
◦NYS Licensed Registered Nurse, RN
◦Experience working persons with IDD a must; experience with mental health concerns preferred
◦Must have excellent verbal, written, and interpersonal communication skills
◦Bilingual English and Spanish speaking is a plus
◦Conducts physical health assessments in the resource center
◦Manages and administers medication as required
◦Provides a range of treatment, rehabilitation, and support services to participants
◦Meets regularly with each guest to assess health status
◦Provides health management education
◦Review treatment and medication history
◦Communicates with each guest primary medical provider to coordinate medical treatment
◦Provide training to program staff on disease, treatment options, and new advances in disease management
◦Responsibility for documentation of services provided
Responsible for ensuring and documenting revisions to the plan as the participant’s needs change
