The Court Advocate will conduct at least 20 screenings per month, referrals and assessment for clients and advocate for clients at court appearances to assist with continuum of treatment with clinical recommendation. The Court Advoate helps to identify key community resources, escort clients to appointments, maintain communication with the Intake Dept. and maintain daily outreach logs, clinical documents. The incumbent will attend interdisciplinary team meetings, case conferences, enter client information and screenings data into AWARDS on the same day as conducted and will work out of 2 locations.
