Position Overview

Position Overview:

S:US is in search of a Licensed RN to work on a new NY State Office of Mental Health (OMH) funded hospital-based care transition team that exclusively serves individuals inpatient at Bronx Psychiatric Center (BPC) and 730.40 designees at Metropolitan Hospital Center (MHC). The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the treatment of human responses to actual or potential health problems through such services as health teaching and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require some on call coverage and a willingness to be flexible.



  • License and current registration to practice as a registered nurse in New York State
  • Prefer at least two years post RN licensure clinical nursing experience working with individuals with mental health needs
