Position Overview

Position Summary:

The Medicaid Service Coordinator advocates & coordinates services as per the need of the consumer in an effort to meet personal goals identified by the consumer.

 

Responsibilities:

  • Develops, implements and maintains consumers’ ISP
  • Maintains contact with consumers as identified in ISP
  • Reviews ISP and Habilitation Plan at least semi-annually
  • Maintains the consumer's primary record
  • Ensures the completion and annual redetermination of the need for the level of care Medicaid eligibility
  • Documents service coordination activities in service coordination monthly notes
  • Facilitates and documents a change in service coordination when requested by the consumer & advocate
  • Secures an advocate for each consumer if they require or choose to have one
  • Maintains an up to date monthly roster of consumers receiving services as part of MSC's caseload
  • Notifies the DDSO of HCBS termination & inform the terminated consumer connecting services
  • Ensures the consumers fire safety needs are met as stated in the ISP at least annually
  • Ensures a face-to-face meeting to be conducted at least monthly, as per the consumers ISP and documented accordingly in the monthly notes
  • Completes with consumer and/or his/her advocate a service coordination agreements and this agreement must be reviewed and up-dated at least every six months and documented in the monthly notes
  • Completes a service coordination agreement activity plan
  • Completes a service coordination observation report once every three months based on an observation of the consumer in an OMRDD certified IRA, CR or Family Care Home and for all Willowbrook class members living in any type if certified residence

Qualifications

  • Bachelors Degree in a health or human services field is required.
  • One year experience working with people with disabilities and participation in an OMRDD approved training curriculum in Services Coordination
  • Driver's License or ability to travel to Brooklyn, Manhattan, Queens, and the Bronx
  • Good writing skills, well organized, articulate, willing to advocate on other's behalf.
