Position Overview

Position Summary:

The Case Manager ensures the best quality of care and services for program consumers in a Certified Community Behavioral Health Clinic (CCBHC). The clinic will provide a range of mental health and substance use disorder services to vulnerable populations with a myriad of needs. The Case Manager assists agency consumers with problem solving and crisis intervention. This role is comprised of 75% field work, travel is required to accompany agency consumers on medical, psychiatric, and public benefit appointments, as necessary.

Qualifications

  • Master’s Degree in Human Services or related field
  • Two years experience with individuals with mental illness, substance abuse issues and/or a history of homelessness required
  • Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
  • Bilingual English/Spanish a plus
  • The ideal candidate will be creative, innovative, open to change and willing to contribute new ideas
