The Case Manager ensures the best quality of care and services for program consumers in a Certified Community Behavioral Health Clinic (CCBHC). The clinic will provide a range of mental health and substance use disorder services to vulnerable populations with a myriad of needs. The Case Manager assists agency consumers with problem solving and crisis intervention. This role is comprised of 75% field work, travel is required to accompany agency consumers on medical, psychiatric, and public benefit appointments, as necessary.
