Position Overview

POSITION OVERVIEW:

  Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility.

 

   KEY ESSENTIAL FUNCTIONS:

 

  Case Management:

  • Provide case management services to a caseload of up to 15
  • Manage and develop systems in the facility.
  • Complete all paperwork for charting as required by funding source
  • Conduct groups periodically or as directed by management

 

Housing Services:

  • Provide Housing Assistance to clients
  • Complete Housing Assessments and Referrals
  • Complete HRA 2010-E’s (CAPS)

 

  ADDITIONAL FUNCTIONS MAY INCLUDE:

  • Attend management trainings, team meetings, and staff meetings.
  • Provide assistance within the facility where needed.
  • Performs other duties as required by the Clinical Supervisor.

Qualifications

EDUCATIONAL / TRAINING REQUIRED:

  • Either CASAC or Bachelor degree Preferred  
  • GED/High School Diploma or Associates Degree Required
  • CASAC-T required

 

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:

 

  • Minimum 2 years’ experience with population in a residential setting or related field

 

COMPUTER SKILLS REQUIRED:

  • Proficient in Microsoft Office and related programs
