S:US provides high-quality, person-centered Rapid Re-Housing and Homeless Prevention services to low-income individuals and families who are experiencing homeless and housing instability.  The Supportive Services for Veteran Families Senior Housing Navigator serves a key new role under the SSVF SNOFA grant award,. They support housing placement and community housing partner engagement for veterans co-enrolled with the SSVF program and VAMC HUD VASH programs.  They will direct liaise with HUD VASH Social Workers and staff at the VAMC as well as housing placement partners in the community, support our landlord recruitment and retention efforts and the delivery of Housing First-oriented supports to participants in the program.  The Senior Housing Navigator works within the SSVF housing placement team to ensure HUD VASH/SSVF enrolled veterans are engaged in daily placement supports and to  maximize participants’ access to safe, affordable permanent housing that meets their needs, preferences and desired outcomes.

 

This candidate will foster internal and external relationships with key stakeholders while demonstrating a strong understanding of the organization’s mission and core values.  The position requires exemplary leadership and organizational skills including excellent verbal and written communication. The Senior Housing Navigator must be committed to the Housing First philosophy of SSVF and ensure that the program’s housing services operate with the utmost level of integrity. The Senior Housing Navigator communicates effectively with participants , VA staff, housing providers and other stakeholders and utilizes best-practices in service provision and strong adherence to practice standards for Rapid Re-housing and Homeless Prevention services. 

 

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

Community Collaboration/Partnerships

  • Coordinate SSVF operations as part of the SSVF/HUD VASH Housing Navigation project, providing regular updates to stakeholders in team meetings and as directed by management.
  • Supports Housing Navigation program implementation efforts across the SSVF service region (NYC).
  • Collaborates effectively with the local Continuum of Care and referring agencies, partner landlords and housing providers, especially HRA Public Engagement Unit, Public Housing authorities, supported housing providers and private landlords
  • Partners with programs to improve the efficiency and quality of the delivery of services.
  • Provides regular feedback to management and liaise with stakeholders regarding program goals and targets, challenges, and key updates as needed.
  • Coordinates with other internal S:US programs to connect Veterans to in-house resources and services.

 

Service Delivery & HUD VASH Coordination

  • Augment housing search and placement supports for veterans selected by the HUD VASH team (who co-enroll in our SSVF program).
  • Enhance access to housing viewings.
  • Assess for housing choice, tenant-screening barriers and strengths.
  • Assist Veterans to locate housing, view units (including transport Veteran to units), collect documents owners require, apply for units and mitigate or overcome any tenant-screening barriers.
  • Conduct landlord engagement and outreach.
  • Assist Veterans to access incentives (landlord & Veteran), move-in costs, and moving logistics.
  • Re-direct/connect Veterans to the HUD-VASH point of contact for clinical care and ongoing case management- trigger the transfer process two weeks into the tenancy.
  • Attend and actively participate in HUD VASH team calls.
  • Creates and deliver appropriate documentation, workflows, reports as needed.
  • Delivers of core services (e.g., Housing Stability Plans, inspections, Rent Reasonableness assessments, housing retention statistics, mandatory paperwork, outcomes, reports, and other agency requirements).

 

Documentation

  • Regularly updates and reviews charts, AWARDS, and other records/systems, helping to ensure that the program follows the rules and regulations of licensing and funding agencies, and in accordance with organizational policies and procedures.
  • Ensures project milestones/goals are met while adhering to approved budgets.
  • Supports timely and accurate documentation of housing services.
  • Works alongside staff as needed for compliance/quality improvement measures for seamless service delivery.
  • Assists with accreditation reviews, monitoring visits and program audits.
  • Completes all required trainings.
  • Contributes to the development of program policies and procedures to ensure effective and efficient delivery of services.
  • Incorporates the use of best practices for ongoing quality improvement of the S:US SSVF program.

 

Accountability

  • Prioritizes and manages multiple projects simultaneously and following through on issues in a timely manner.
  • Responds to participant and stakeholder needs promptly, accurately and with courtesy and respect.
  • Models appropriate behavior and represents the organization in a positive and appropriate manner.
  • Promotes the SSVF Housing First model of service delivery. Develop and promote an awareness of critical barriers to attaining or maintaining permanent housing, and rapid response to those barriers including assistance with legal services as allowable and access to other CoC services as needed
  • Reports on HUD VASH meetings, agendas and prepares responses and recaps for management and stakeholders to ensure all team tasks and goals are being accomplished as expected.
  • Effectively communicates with the Veterans Services team by providing constructive feedback to Veterans Services staff regarding housing services.

 

High-Quality Service Delivery

  • Assists in the implementation and development of best practices in the field.
  • Maintains a caseload of SSVF participants, as needed, in accordance with agency SSVF standards.
  • Completes Housing Assessments, inspections, landlord-tenant interventions when needed.
  • Delivers high-quality customer services to all stakeholders.
  • Conducts fieldwork up to 75% of the work week.
