I. SCOPE OF ROLE: 
Experienced Case Manager to work in a NYS Office of Alcoholism and Substance Abuse Services 
(OASAS) funded program. The candidate will work to address the preparatory needs of participants in 
the early stages of recovery, development of daily living skills, and coping mechanisms through group 
and individual work in the program.
II. ESSENTIAL DUTIES & RESPONSIBILITIES: 
• Provide case management services to a caseload 
• Manage and develop systems in the facility 
• Complete all paperwork for charting as required by funding source 
• Conduct groups periodically or as directed by management 
• Provide Housing Assistance to clients 
• Complete Housing Assessments and Referrals 
• Complete HRA 2010-E’s 
• Attend management trainings, team meetings, and staff meetings 
• Provide assistance to clients within the facility when needed 
• Performs other duties as required by the Program Director and Clinical Supervisor 
• Other duties as assigned
III. REQUIRED EDUCATION AND EXPERIENCE 
• Full CASAC or Bachelor degree OR 
• CASAC-T w/ Bachelor degree 
• 2 years of case management work experience in a social service agency, preferably serving mental 
health or substance using populations
PREFERRED QUALIFICATIONS & SKILLS 
• Knowledge of substance use, mental illness, treatment options, and the needs of an individual 
integrating back into the community 
• Excellent written, verbal and computational skills to present and document records in accordance 
with program standards
