Position Overview

SCOPE OF ROLE:
The Registered Nurse collaborates with physicians and the multidisciplinary team to ensure residents receive timely care that meets evidence-based practice standards. The role integrates utilization management, care coordination, and discharge planning functions to assist residents in achieving positive clinical outcomes and safe transitions to the community. The Registered Nurse provides resident/family support and education and maintains strong relationships with community providers

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Implement and establish plan of care in accordance with scope of practice in collaboration with the physician, clinical team members, social services, physical therapy, the resident and the resident's family by tailoring care to meet individual needs of the resident.
  • Monitor resident’s healthcare/medical needs and provide coaching, training and feedback, to assists in daily management of the residents
  • Observe, audit and reviews each resident's condition, status, and care requirements and act to assure care needs are met by connecting residents to appropriated medical services in the community.
  • Visit and provide services to participants in residential and scattered sites
  • Contribute to participant care conferences and evaluate the interdisciplinary plan of care for each resident.
  • Monitor all medication room and support medication coordinators in multiple sites.
  • Ensure all residents are connected to providers in the community (PCP, Therapist, Psychiatrist, etc.)
  • Reports changes in condition of participants that requires nursing intervention notifying the physician, case managers and family
  • Attend medical visits while building rapport with PCP and other care team members for advocacy and to ensure residents comply with medical regimen.
  • Visit residents during hospitalization, attend and contribute to discharge planning meeting to assist with safe discharge.
  • Documents all pertinent information in the interdisciplinary notes and on the 24-hour report timely and accurately
  • Address any emergent resident needs; anticipates changes in condition and makes appropriate clinical decisions.

 

 

 

 

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

  • Associate Degree in Nursing or a Bachelor of Science Degree in Nursing
  • Must possess an active RN license in the NY state
  • 1+ years working in shelters, behavioral health or with the mental health population, including experience with medication administration and management and care plan development.

PREFERRED QUALIFICATIONS & SKILLS 

  • Ability to creatively problem solve in both resident care and employee management situations
  • Must be a team player, well organized, and flexible.
  • Ability to effect change, perform ongoing evaluation and critical analysis, plan and organize effectively and promote client/family autonomy.
  • Effective written and oral communication skills
  • Basic computer skills (e.g. MS Office suite and Windows)
