Position Overview

  1. SCOPE OF ROLE:

The Case Manager provides comprehensive case management services for individuals transitioning from the Rikers Island Correctional Facility, facilitating their reintegration into stable housing. This position entails the development of individualized housing and service plans designed to ensure that clients attain permanent residence as they re-enter their communities as well as assisting community residents facing housing instability.

 

  1. ESSENTIAL DUTIES & RESPONSIBILITIES:
    • Conduct initial prescreening and intake assessments for prospective program participants.
    • Provide case management for clients and their families, which includes arranging, coordinating, monitoring, and delivering services to meet their permanent housing needs.
    • Foster connections with community service providers to assist participants in obtaining and maintaining housing stability.
    • Evaluate clients for eligibility for subsidies and provide assistance in the application submission process.
    • Encourage compliance with appointments and promote self-sufficiency among clients.
    • Monitor and evaluate client progress regarding their housing situation and integration within the community.
    • Submit Weekly program reports, including client data and progress towards targets.
    • Maintain case records, ensuring that all relevant information is accurate and organized appropriately.
    • Connect clients with financial service providers for assistance with household budgeting, financial counseling, credit repair, and financial workshops.
    • Assist participants in obtaining employment services, such as job training, job readiness programs, and employment workshops.
    • Provide mediation and counseling services for clients returning to their pre-arrest living arrangements with family members, friends, and/or landlords. Provide crisis intervention as needed.
    • Advocate for public benefits, including rental assistance, food stamps, and Medicaid.
    • Arrange case conferences with all primary agencies involved in a client’s stability and obtain case records when necessary.
    • Prepare and submit Financial Assistance Requests on behalf of program participants.
    • Participate in both internal and external training programs.
    • Conduct home visits as required.
    • Ensure timely completion of intakes, assessments, service plans, case notes, and referrals.
    • Document all interactions promptly.
    • Routinely required to work on Riker’s Island as well as at the Manhattan location of Homebase.
    • Perform such other duties as may be required by Program management.

 

Qualifications

  • REQUIRED EDUCATION AND EXPERIENCE
    • Master of Social Work (MSW) preferred, or bachelor’s degree (BS/BA) with enrollment in an accredited MSW program.
    • Bilingual candidates are preferred.
    • Strong understanding of both the shelter system and the criminal justice system is necessary.
    • Experience working with individuals facing homelessness, substance abuse issues, mental health challenges, and families is required.
    • Basic computer skills and familiarity with Microsoft Office applications are essential.
    • Excellent written and verbal communication skills are a must.
